Characterization of long-term gait deficits in mouse dMCAO, using the CatWalk system.

Evaluation of functional outcome is widely used across species to assess the recovery process following various pathological conditions, including spinal cord injury, musculo-skeletal injury, mithochondrial disease, neuropathic cancer, Huntington's disease, chronic pain, cortical lesion, and olivocerebellar degeneration among others. The Stroke Therapy Academic Industry Roundtable (STAIR) recommends multiple endpoints for behavioral studies in pre-clinical stroke research, to demonstrate their clinical relevance. One of the more challenging tasks in experimental stroke research is measuring lo

Reading on the right when there's nothing left? Probabilistic tractography reveals hemispheric asymmetry in pure alexia.

We present a patient with reading inexpertise and right hemianopia following left posterior cerebral artery (PCA) stroke. We examine the extent of disruption to reading performance and the extent of white matter tract damage relative to a patient with more limited PCA infarction and isolated right hemianopia. We show white matter disconnection of the temporal occipital fusiform cortex in our pure alexia patient. Connectivity-based laterality indices revealed right hemisphere laterality in the alexia patient; this was not associated with improved reading function. We speculate that the degree o

Severe cerebral vasospasm after traumatic brain injury.

Severe traumatic brain injury is associated with both acute and delayed neuro- logical injury. Cerebral vasospasm is commonly associated with delayed neurological decline in aneurysmal subarachnoid hemorrhage patients. However, the role played by vasospasm in traumatic brain injury is less clear. Vasospasm occurs earlier, for a shorter duration, and often without significant neurological consequence among traumatic brain injury patients. Detection and management strategies for vasospasm in aneurysmal subarachnoid hemorrhage are not easily transferrable to traumatic brain injury patients. We pr

Isolation and purification of self-renewable human neural stem cells for cell therapy in experimental model of ischemic stroke.

Human embryonic stem cells (hESCs) are pluripotent with a strong self-renewable ability making them a virtually unlimited source of neural cells for structural repair in neurological disorders. Currently, hESCs are one of the most promising cell sources amenable for commercialization of off-shelf cell therapy products. However, along with this strong proliferative capacity of hESCs comes the tumorigenic potential of these cells after transplantation. Thus, the isolation and purification of a homogeneous, population of neural stem cells (hNSCs) are of paramount importance to avoid tumor formati

Microembolic signal monitoring of TOAST‑classified cerebral infarction patients.

The aim of the present study was to investigate the effects of microembolic signals (MES) on post‑stroke neurological deficits, stroke recurrence and survival in patients with acute cerebral infarction (CI). Patients with acute CI were enrolled consecutively and classified etiologically into the following 4 subtypes using the Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification: i) Cardioembolism (CE); ii) large‑artery atherosclerosis (LA); iii) small‑vessel disease (SVD); and iv) stroke of other etiology, including other and undetermined etiologies. The MES of cerebral arteries

Capsular warning syndrome and crescendo lacunar strokes after atherosclerotic stenosis of the recurrent artery of Heubner.

The stereotype of repetitive transient cerebral ischemia causing unilateral motor, sensory, or sensorimotor deficits that simultaneously affect the face, arm, and leg, clinically localized to the internal capsule, fits with the description of capsular warning syndrome (CWS). A high proportion of individuals with these symptoms develop subsequent capsular stroke, despite various proposed preventative measures. It has been postulated that the mechanism for such strokes is that of small-vessel single-penetrator disease. We present a patient with repetitive CWS intermingled with crescendo capsular

Multimodality MRI and MRA for decision making in minor stroke: a case with internal carotid and distal middle cerebral artery occlusion.

We report the case of a 65-year-old man who presented with mild, rapidly improving stroke symptoms. Acute magnetic resonance imaging disclosed no diffusion abnormalities but a tandem internal carotid artery/distal middle cerebral artery occlusion associated with a large corresponding deficit on perfusion imaging. In addition, there was a cross-flow to the middle cerebral artery via the anterior communicating artery. Therefore, intravenous thrombolysis was initiated that led to rapid reopening of the middle cerebral artery and left the patient free of symptoms. Our observation highlights the po

Balloon angioplasty of the A2 segment of the anterior cerebral artery for recurrent, symptomatic atherosclerotic disease.

Intracranial atherosclerotic disease accounts for 5-10% of ischemic strokes in the US. Lesions located in the anterior cerebral artery territory are infrequently reported. Patients in whom medical therapy fails are at a high risk for recurrent ischemic events, in which case intracranial angioplasty or stenting may be a reasonable therapy. There is a paucity of literature describing angioplasty of fixed atherosclerotic lesions affecting the anterior cerebral artery territory, and especially the A(2) segment. This case illustrates that this vessel segment may be treated with balloon angioplasty.

Cognitive functions in patients with MR-defined chronic focal cerebellar lesions.

The aim of the present study was to examine cognitive functions in a group of chronic patients with focal cerebellar lesions. Both effects of localization (anterior vs. posterior lobe) and side (left vs. right cerebellar hemisphere) were of interest. Fourteen patients with infarctions within the territory of the posterior inferior cerebellar artery (PICA) and seven patients with infarctions within the territory of the superior cerebellar artery (SCA) participated. The affected lobules and nuclei were assessed based on 3D MR imaging. The right cerebellar hemisphere was affected in eight PICA an

An investigation of attentional contributions to visual errors in right "neglect dyslexia".

One type of error that is sometimes produced by patients with acquired dyslexia is the substitution of an orthographically similar word with letters that overlap the target either in early or late letter positions. When such errors affect the left sides of words, they are usually produced by patients with focal right hemisphere lesions who typically show evidence of left neglect in non-reading tasks. This pattern has thus been termed "neglect dyslexia". When the right sides of words are affected, however, patients frequently fail to show any signs of neglect in tasks other than reading. This s

Pretreatment with eplerenone reduces stroke volume in mouse middle cerebral artery occlusion model.

Eplerenone, a mineralocorticoid receptor antagonist, is reported to be effective to prevent end-stage cardiovascular damage induced by aldosterone. However, the effect of eplerenone on brain damage is not fully understood. Here, we investigated whether pretreatment with eplerenone attenuates stroke size in mice subjected to middle cerebral artery occlusion. Middle cerebral artery occlusion with a microfilament technique induced focal ischemia, to approximately 25% of the total area in a coronal section of the brain. Treatment with eplerenone at a dose of 1.67 mg/g chow significantly reduced th

Percutaneous reconstruction of the innominate bifurcation using the retrograde 'kissing stents' technique.

INTRODUCTION: Obstructions of the supraaortic vessels are an important cause of morbidity associated with a variety of symptoms. Percutaneous transluminal angioplasty has evolved as an effective and safe treatment modality for occlusive lesions of the supraaortic vessels. However, the endovascular management of an innominate bifurcation has not previously been reported. METHODS: A 53-year-old female with a history of systematic hypertension, diabetes mellitus and hypercholesterolemia presented with left hemiparesis and dysarthria. Angiography of the innominate artery showed a stenosis of the

Post-ischaemic treatment with the cyclooxygenase-2 inhibitor nimesulide reduces blood-brain barrier disruption and leukocyte infiltration following transient focal cerebral ischaemia in rats.

Several studies suggest that cyclooxygenase (COX)-2 plays a pivotal role in the progression of ischaemic brain damage. In the present study, we investigated the effects of selective inhibition of COX-2 with nimesulide (12 mg/kg) and selective inhibition of COX-1 with valeryl salicylate (VAS, 12-120 mg/kg) on prostaglandin E(2) (PGE(2)) levels, myeloperoxidase (MPO) activity, Evans blue (EB) extravasation and infarct volume in a standardized model of transient focal cerebral ischaemia in the rat. Post-ischaemic treatment with nimesulide markedly reduced the increase in PGE(2) levels in the isch

Insular cortical ischemia is independently associated with acute stress hyperglycemia.

BACKGROUND AND PURPOSE: Acute poststroke hyperglycemia has been associated with larger infarct volumes and a cortical location, regardless of diabetes status. Stress hyperglycemia has been attributed to activation of the hypothalamic-pituitary-adrenal axis but never a specific cortical location. We tested the hypothesis that damage to the insular cortex, a site with autonomic connectivity, results in hyperglycemia reflecting sympathoadrenal dysregulation. METHODS: Diffusion-weighted MRI, glycosylated hemoglobin (HbA1c), and blood glucose measurements were obtained in 31 patients within 24 hou

Aortic atherosclerosis and perioperative stroke in patients undergoing coronary artery bypass: role of intra-operative transesophageal echocardiography.

Intra-operative transesophageal echocardiography was performed in 126 patients undergoing coronary artery bypass grafting. Significant protruding atheromas (grade IV and V; Katz et al., 1992) were present in 12 patients (9.5%). Protruding atheromas had significantly higher incidence in patients above 60 years in age. Preoperative assessment with chest roentgenography and angiography, as well as intra-operative assessment by surgical palpation proved to be insensitive in detecting aortic atheromas. Out of four patients with grade V atheromas, two (50%) developed right hemiplegia postoperatively

The problems of surgical treatment for cardiac myxoma and associated lesions.

Twenty-four patients with cardiac myxomas consisting of 22 left and 2 right atrial myxomas were operated on. All myxomas were removed with an excision of the attachment walls using a cardiopulmonary bypass. Two myxomas required a partial cardiopulmonary bypass from the femoral vein to the artery prior to operation because they were on the verge of becoming stuck in the atrioventricular valves and potentially causing shock. For embolic complications of myxoma, the embolus of the external carotid artery was extirpated before undergoing cardiac surgery. In a patient with pulmonary infarction, the

Individualized off-pump all internal thoracic artery revascularization.

OBJECTIVE: We developed an individualized off-pump approach in an all internal thoracic artery (AITA) composite graft revascularization (AITACR) program to minimize postoperative neurological complications and to obtain the best long-term results possible. Early results of the individualized approach are reported. PATIENTS AND METHODS: The operative method (on-pump or off-pump) was determined based on institutional selection criteria. Early neurological outcomes were evaluated in 157 men and 42 women; the mean age was 67.3 +/- 9.3 years. RESULTS: Fifty-nine underwent off-pump procedures and

Aortic Arch Mural Thrombus Concurrent with Deep Surgical-Site Infection after Colorectal Cancer Surgery in an Enhanced Recovery after Surgery Program: A Case Report.

INTRODUCTION: Aortic mural thrombus (AMT) in a non-atherosclerotic aorta is rare but potentially catastrophic and may be difficult to distinguish from septic aortic pathology when it occurs alongside a deep postoperative infection. Enhanced recovery after surgery (ERAS) shortens hospital stay and shifts the recognition of serious complications to the early post-discharge period. We report the case of a patient who underwent colorectal cancer surgery within an ERAS protocol who developed a large AMT on POD 10, coincident with Streptococcus anginosus-positive deep surgical-site infection (SSI) b
