Three major functional characteristics of the insulin receptor are negative cooperativity, down-regulation, and beta-subunit tyrosine kinase activity. To investigate the inter-relationships among these functions we studied four antibodies to the insulin receptor alpha-subunit. Patients with insulin dependent diabetes mellitus who develop proteinuria may die prematurely, whereas those who do not develop this complication have a comparatively normal life span. The excess mortality in diabetics with proteinuria is from cardiovascular as well as renal disease, but the reason is unclear. A 43-year-old woman with spontaneous episodes of neuroglycopenic hypoglycemia was found to have immune-mediated thrombocytopenic purpura and primary biliary cirrhosis. Hypoglycemia along with hyperinsulinemia suggested insulinoma. Serum c-peptide levels were disproportionately low, raising the possibility of factitious hypoglycemia. Diabetes mellitus is common in the Maltese. Patients in whom diabetes was first diagnosed before the age of 25 years were examined to determine their oral status and to compare their levels of disease with oral disease in non-diabetic age-matched control subjects. No patient was older than 29 years. Compared with control subjects, the diabetic patients showed differences in both caries and periodontal disease experience. Those with poor metabolic control of their diabetes showed oral disease patterns different to the stable diabetics.
