Involvement of the sacroiliac joints is a hallmark of the spondyloarthropathies, especially in ankylosing spondylitis. The conventional diagnostic imaging of sacroiliitis in early stages might cause problems, because sensitivity of conventional radiographic methods is known to be too low in early stages of the disease. Magnetic resonance imaging of the sacroiliac joints certainly enables one to detect acute as well as chronic inflammatory changes in all stages of the disease. The potential disadvantages of this method are the dependency on the examiner, the lack of standardization, and the rel

A case of a man suffering from psoriasis vulgaris is presented. The onset of the disease was at the age of 5. From the age of 14 all joints were gradually affected by the type of severe, symmetrical psoriatic arthritis which led to aseptic necrosis of the hip joints. In the course of the disease, from 1988 on, proteinuria and nephrotic syndrome were found. The renal biopsy proved amyloidosis which is considered secondary, developed on the basis of psoriatic arthritis. The severe psoriatic arthritis with especially emphasized clinical activity is in this case the pathologic antigenic stimulus f

Pulmonary ventilation was studied in 100 patients with rheumatoid arthritis of an outpatient clinic, using whole-body plethysmography, spirography, and pneumometry. The patients with more or less severe joint involvement, as well as the patients with shorter or longer duration of their disease, did not show any significant difference in their dynamic and static pulmonary fuction. Patients with serious joint destructions showed a bronchial obstruction in 29%. 36 patients who were treated with thioglucose gold had a significantly better expiration capacity. A causal connection with the underlyin

BACKGROUNDS: Wilson's disease (WD) is a rare autosomal recessive disorder of copper metabolism characterized by impaired hepatic copper excretion and progressive multisystem copper accumulation. While hepatic and neuropsychiatric manifestations predominate, arthralgia as an initial presentation is uncommon. We report a child with WD who presented initially with joint pain.
CASE PRESENTATION: A ten-year-old patient experienced recurrent joint pain, predominantly in the lower limbs, and physical examination revealed tenderness in the right ankle joint and hepatomegaly. Laboratory investigations

Osteoarthritis (OA) is a frequently diagnosed joint disease. It is characterized by articular cartilage/bone damage, synovitis, pain, etc., which severely reduce the life quality of patients. Current treatments for OA act to reduce inflammation and lubricate joints but barely halt OA progression and joint damage. Matrix metalloproteinases (MMPs) are key factors for extracellular matrix (ECM)/cartilage/bone degradation and inflammation. Therapies targeting MMPs hold the potential to treat OA efficiently. Recently, MMPs have been studied as the therapeutic targets for OA treatment. MMP inhibitor

Bone marrow edema (BME) of the temporomandibular joint is an abnormal change within the bone marrow that appears as low signal intensity on T1-weighted images and high signal intensity on T2-weighted images in MRI. It is commonly seen in patients with temporomandibular joint osteoarthritis. Studies have shown that BME is closely related to joint pain, disc displacement and bony degeneration, but its specific mechanism is not yet fully understood, potentially involving abnormal mechanical loading, inflammatory responses and imbalances in bone metabolism. Although BME is considered an early sign

Hallux valgus is a common foot and ankle disease, for which numerous surgical procedures were introduced. So, understanding the mechanism of deformity reduction is important to select the proper method. Intermetatarsal angle (IMA) determines the severity of hallux valgus, which is influenced by the translated metatarsal head and the reduction of the first tarsometatarsal joint. We hypothesized that both of the mechanisms simultaneously contribute to the correction of IMA. Hallux valgus (70 feet) operated with a Scarf osteotomy with the Akin procedure were reviewed. Hallux valgus angle (HVA), I

OBJECTIVES: This study aimed to identify disability trajectories and discover early disablement process factors associated with disability trajectories among older adults in China.
STUDY DESIGN: This is a prospective cohort study.
METHODS: Data were obtained from five waves of the Chinese Longitudinal Healthy Longevity Survey (CLHLS, 2005 to 2018). The multitrajectory modelling approach was used to estimate the joint trajectories of activity of daily living (ADL) disability and instrumental activity of daily living (IADL) disability. A multinomial regression model was used to investigate how

SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, osteitis), a rare inflammatory disorder, is an association of distinct skin disorders with pustules with osteoarticular inflammation. Its etiology remains unclear, and various treatment regimens frequently fail to control the disease. An 18-year-old male patient presented to the outpatient department with severe nodulocystic acne on the face with pain at both the wrists and lower back associated with high-grade fever and chills. On physical examination, he had severe tenderness at both wrist joints and lower back, along with swelling o

[Purpose] Ankylosing spondylitis is prevalent in men. Modern and expert consensus documents include physical therapy among the strategies for the treatment of ankylosing spondylitis. This study aimed to describe the physical therapy approach in an athlete with ankylosing spondylitis. [Subject and Methods] The patient, refractory to treatment with anti-inflammatory medication, showed pelvic and lumbar pain and joint, muscle, and functional disorders, which were treated with orthopedic joint mobilization, dry needling, exercise, and whole-body hyperthermia. [Results] After the treatment, pain re

Hallux valgus is the most common deformity of the foot, affecting 2-4% of the population. The disease is mostly genetic, but there are also exogenous factors. Conservative treatment for the use of orthotics is usually ineffective and only surgery leads to a complete cure. The indications for surgery are pain and/or problems with shoe choosing. AP and lateral foot standing X-ray film is necessary for the proper qualifications. Depending on the size of the hallux valgus angle (HVA) and intermetatarsal angle (IMA) deformations are divided into mild, moderate, advanced and severe. In addition, we

Visceroptosis is described in several heritable connective tissue disorders, including the hypermobility type of Ehlers-Danlos syndrome (hEDS), a.k.a. joint hypermobility syndrome (JHS). Clinical features of hEDS comprise joint hypermobility, mild skin hyperextensibility, joint instability complications, chronic joint/limb pain, and positive family history. Uterine and rectal prolapse has been reported in nulliparous women. We report on a family with two patients with hEDS. The proposita, a 38-year-old woman, present bilateral kidney prolapse requiring three nephropexies, gastric ptosis treate

Articular involvement is frequent in SSc. It contributes to disability and compromises patients' quality of life. Different aspects of articular involvement have been described, ranging from arthralgia and arthritis to joint contracture and tendon sheath involvement. Recent cohort studies examining clinical and radiographic aspects of SSc have clarified the frequency of articular involvement and identified subsets of SSc patients with a higher risk of developing joint involvement. They have also highlighted the potential contribution of inflammatory arthritis to early SSc. Some pilot studies h

Calcium Pyrophosphate Dihydrate Deposition (CPDD) disease has characteristic radiographic features including soft tissue calcification, joint space narrowing, bone sclerosis, subchondral cyst formation without osteophyte formation, and large intraosseous geodes. Triangular fibrocartilage calcification is frequently found and isolated scapho-trapezio-trapezoid (STT) arthritis is specific for CPDD. Distal radio-ulnar (DRUJ), isolated midcarpal joint and piso-triquetral joint involvement also occur. 127 patients were reviewed. Seventy-eight had symptomatic STT joint arthritis, for which 36 underw

From 1978 through 1988, 27 severe slipped epiphyses (Grade III) in 23 adolescent patients were treated with a cuneiform osteotomy of the proximal neck of the femur. Average time of followup was 8 years 5 months. The results were graded by the criteria of Southwick, with emphasis on pain, function, range of motion, and radiographic appearance. Arbitrarily, no hips were rated excellent, because the authors thought that this rating should be reserved for normal hips that have not had any disease process nor surgical procedure. There were 19 good, 4 fair, and 4 poor results. The 4 poor results wer

We reviewed the results of the Keller arthroplasty in combination with resection arthroplasty of the forefoot in patients with rheumatoid arthritis. Of the 29 patients (49 feet) in the series, 20 had involvement of both feet and nine had involvement of a single foot. The average age of the patients was 55.4 years, and the average follow-up period was 4.9 years. All feet had resection of the lesser metatarsal heads, resection of the base of the proximal phalanges of the lesser toe, and a Keller arthroplasty of the first metatarsophalangeal joint. The results were satisfactory in 16 feet, satisf

IDA of the shoulder is a condition found predominantly in elderly females. Although the shoulder is primarily involved, other joints such as the hip and knee can be affected, and concurrent OA is common at other joint sites. Clinical features include voluminous, often blood-stained effusions, together with features of rotator cuff rupture and restriction of shoulder movement. Laboratory parameters are usually normal and examination of the synovial fluid reveals large amounts of basic calcium phosphate crystals. The synovium is hypertrophied and vascular and shows fibrin deposition. It contains

In order to improve the therapeutic results of conservative treatment for coxarthrosis and to start functional training soon after surgery, the S-form hip brace of Wakayama Medical College type has been developed with due regard to the instability of the hip joint which is related to the origin of coxarthrosis and to biomechanics of the hip joint. Functional investigation was made by electromyography, etc. Even healthy people tend to develop gradual lack of stability at the hip joint with aging and the tendency is more pronounced in secondary coxarthrosis. This brace has been designed to reinf

Several points may be stressed. (1)When in doubt, perform joint aspiration and look for crystals of micro-organisms. A joint tap is nearly always indicated. (2)Do not rely on a coincidental elevated serum uric acid level. Question the patient regarding drug therapy and other causes of secondary hyperuricemia. (3)Examine all of the patient, looking for tophi (gout), skin lesions (gonococcal infection, psoriasis), erythema nodosum (allergic reactions, fungal infections), and other clues. (4)Monoarticular rheumatoid arthritis is a rare cause of a single hot joint, but it is much more common that
