Fractures of the anteroinferior glenoid rim, termed bony Bankart lesions, have been reported to occur in up to 22% of first time anterior shoulder dislocations. The primary goal of treatment is to create a stable glenohumeral joint and a good shoulder function. Options for therapeutic intervention are largely dependent on the chronicity of the lesion, the activity level of the patient and postreduction fracture characteristics, such as the size, location and number of fracture fragments. Non-operative treatment can be successful for small, acute fractures, which are anatomically reduced after

INTRODUCTION: Morquio syndrome is caused by an inherited autosomal recessive enzyme deficiency. It presents with numerous musculoskeletal anomalies, among which atlantoaxial instability is highlighted, due it being life-threatening. Occipital-cervical arthrodesis surgery (and decompression) is indicated in these cases.
OBJECTIVE: The cases of 2 patients with Morquio syndrome that required this type of surgery are presented, along with a review of the literature.
PATIENTS AND METHODS: Two patients: one boy and one girl, with Morquio syndrome and high cervical instability, with neurological si

BACKGROUND CONTEXT: Recent reports have suggested that excessive motion of the lumbar spine might be associated with low back pain and accelerated disc degeneration and may negatively influence the outcome of posterior lumbar interbody fusion (PLIF) surgery. These findings suggest that generalized joint laxity (GJL) might be a negative factor affecting PLIF outcome, although this relationship has not been well studied. In addition, the impact of GJL on adjacent segment pathology (ASP) after PLIF has not been reported.
PURPOSE: To explore the relationship between GJL and the outcome of single-

Traumatic cartilage lesions of the shoulder, such as glenolabral articular disruption (GLAD), have previously been recognized in patients with shoulder instability. We describe a new lesion in which the entire anteroinferior quadrant of the glenoid articular cartilage is sheared off in association with an adjacent labral tear. Both patients were teenage athletes who were being treated arthroscopically for shoulder instability. Preoperative imaging showed some evidence of an articular cartilage lesion. One patient was treated with removal of an irreparable fragment and microfracture, while in t

BACKGROUND: Arthroscopic remplissage is a novel procedure recently advocated for the treatment of large Hill-Sachs lesions with recurrent anterior glenohumeral instability. We have shown previously that infraspinatus tenodesis and Bankart repair reduce the risk of recurrent instability in high-risk patients. The ability to perform this procedure arthroscopically and without the need for bone grafting or an open approach makes this an appealing alternative to more traditional techniques.
PURPOSE: To evaluate and characterize the postoperative appearance of the remplissage procedure on high-res

In posttraumatic shoulder dislocation, the size of a glenoid rim defect can determine the probability of recurrent dislocations. Most attempts to assess the size of such defects are based on the width of a defect in relation to glenoid length or diameter. So far, no method exactly calculating the area of articular surface missing has been reported. With the use of software originally designed for architectural purposes, the exact percentage of a glenoid defect was measured on three-dimensional computed tomography images.

Published reports on patients with skeletal fluorosis undergoing total knee arthroplasty are rare. Skeletal fluorosis is a chronic condition that occurs secondary to the ingestion of food and water that contain high levels of fluoride. Although fluorosis may be described as osteosclerotic and marble-like in appearance, features may also include characteristics of osteomalacia and osteoporosis. This article describes the case of a 67-year-old man with skeletal fluorosis who underwent total knee arthroplasty complicated by fracture. An intramedullary guide was used for the proximal tibia and dis

BACKGROUND: Cervical orthoses are commonly used for extrication, transportation, and definitive immobilization for cervical trauma patients. Various designs have been tested frequently in young, healthy individuals. To date, no one has reported the effectiveness of collar immobilization in the presence of an unstable mid-cervical spine.
STUDY OBJECTIVES: To determine the extent to which cervical orthoses immobilize the cervical spine in a cadaveric model with and without a spinal instability.
METHODS: This study used a repeated-measures design to quantify motion on multiple axes. Five lightl

PURPOSE: Securing the airway of a patient with a potentially unstable cervical spine (C-spine) is a complex and challenging task. The objective of this continuing professional development module is to review the current knowledge essential for airway management in the face of potential C-spine instability and, at the same time, to underline areas of uncertainty and limitations in the literature.
PRINCIPAL FINDINGS: In low-risk patients-defined by strict criteria derived from large multicentre studies-the C-spine can be considered stable or "cleared" without imaging. In all other patients, at

BACKGROUND: Generalized joint hypermobility is a highly prevalent condition commonly associated with joint injuries. The current literature has conflicting reports of the risk of joint injury in hypermobile sporting participants compared with their nonhypermobile peers. Systematic reviews have not been conclusive and no meta-analysis has been performed.
PURPOSE: This review was undertaken to determine whether individuals with generalized joint hypermobility have an increased risk of lower limb joint injury when undertaking sporting activities.
STUDY DESIGN: Systematic review with meta-analys

Classic Ehlers-Danlos syndrome is a heritable connective tissue disorder characterized by skin hyperextensibility, fragile and soft skin, delayed wound healing with formation of atrophic scars, easy bruising, and generalized joint hypermobility. It comprises Ehlers-Danlos syndrome type I and Ehlers-Danlos syndrome type II, but it is now apparent that these form a continuum of clinical findings and differ only in phenotypic severity. It is currently estimated that approximately 50% of patients with a clinical diagnosis of classic Ehlers-Danlos syndrome harbor mutations in the COL5A1 and the COL

BACKGROUND: Balancing both the lateral/medial and extension/flexion joint gaps is a prerequisite for soft tissue balance in total knee arthroplasty. The purpose of this study was to quantify the effects of patellar positioning and quadriceps load during total knee arthroplasty on knee joint gap measurements.
METHODS: Eight fresh-frozen cadaveric knees ranging in age from 65 to 85 years old were used. Using a medial parapatellar approach, posterior cruciate ligament sacrificing total knee arthroplasty was performed. The specimens were mounted on a custom knee testing system that allowed the fe

STUDY DESIGN: A cadaveric simulation model of the lumbar spine was used to study the intervertebral motion characteristics of the lumbar spine after bilateral laminotomy and facet-sparing laminectomy.
OBJECTIVE: To assess differences in motion patterns and lumbar spine stiffness after bilateral laminotomy versus laminectomy.
SUMMARY OF BACKGROUND DATA: Spondylolisthesis after facet-sparing laminectomy has been reported with a frequency of 8% to 31%. Bilateral laminotomies have been shown to be effective in decompressing the spine, without resection of the posterior osteo-ligamentous complex.

When stabilization surgery fails, both patient and treating physician face disappointment as well as additional stress in attempting to solve this difficult clinical challenge. The treating physician must: (1) review the basics of what constitutes stability, (2) confirm the correct diagnosis by performing a thorough examination supplemented by appropriate imaging, (3) determine the reason for failure, (4) determine the expectations and needs of the patient, and (5) decide which operative or nonoperative approach provides the best potential result for the patient. This review article will provi

PURPOSE: Meniscal allograft transplantation (MAT) has become an accepted treatment option for patients undergoing meniscectomy with recalcitrant pain in the corresponding compartment. Whether MAT can oppose cartilage degeneration is unclear. Our purpose was to perform a systematic review of available literature to answer the following: (1) Does MAT prevent advancing chondrosis? (2) Who is the ideal candidate for MAT? (3) What is the survival time for allograft in a stable knee? (4) Can MAT be successful when performed with concomitant procedures? (5) Is there an outcome difference between medi

Constrained acetabular liners were developed for the surgical treatment of recurrent instability by holding the femoral head captive within the socket. This article summarizes the data describing constrained component designs, indications, outcome, and complications. Different designs accept head sizes of varying diameter and have differing amounts of rim elevation and offset, allowing slight variations in the range of movement allowed. Complications of constrained acetabular components can be divided into three categories. The first category is directly related to the constraining mechanism s
