Cognitive outcome of long-term survivors of multisystem langerhans cell histiocytosis: a single-institution, cross-sectional study.

PURPOSE: Damage to the CNS, including the cerebellum, and to the hypothalamopituitary axis, is documented in Langerhans cell histiocytosis (LCH). Neuropsychologic deficits have been recognized, but this is the first study in which cognitive function has been systematically assessed in a cohort of patients. PATIENTS AND METHODS: Twenty-eight long-term survivors of multisystem LCH (mean age, 15.1 years) were investigated for intelligence, memory and learning, language, and academic attainments. RESULTS: The mean intelligence quotient (IQ) of the entire group was not significantly different fro

Neural substrates of spoken language rehabilitation in an aphasic patient: an fMRI study.

Little is known about the neural counterparts of speech therapy in aphasic patients. An fMRI experiment was performed before and after a specific and intensive speech output therapy in RC, a patient with long-lasting speech output deficit following a left-sided ischemic lesion. Overt picture naming and picture/word rhyming were used as activation tasks in RC and 6 control subjects. The naming task concerned the output lexicon deficit to be rehabilitated while rhyming referred to preserved levels of processing and was used to control for repetition effect. The speech therapy program improved na

Sleep EEG and developmental dysphasia.

To determine the relation between developmental dysphasia and EEG anomalies during sleep, we compared 52 subjects with dysphasia with a control group of 20 children by using the ambulatory EEG method. Whereas 50% of the children with dysphasia experienced paroxysmal activity (PA), only two of the control group did. It is likely that paroxysmal abnormalities and language impairment are related to architectural dysplasia and neuron-migration disturbances. PA is frequent in subjects with receptive developmental dysphasia and may be the cause of language deterioration. When the occurrence of parox

Nonverbal perceptual organization output disability and schizophrenia spectrum symptomatology.

Patients with neurodevelopmental syndromes often receive numerous psychiatric diagnoses before the true nature of their disorder becomes apparent. We present a case in which the neuropsychological evaluation played a significant role in reconceptualizing a patient who had received, at various times, diagnoses of schizotypal personality disorder, schizoaffective disorder, and schizophrenia. The identification of specific cognitive deficits in executive functioning, perceptual organization, visual-spatial problem solving, and abstraction led to: 1) a diagnosis of nonverbal perceptual-organizatio

Do parent-reported early indicators predict later developmental language disorder? A Raine Study investigation.

BACKGROUND: Developmental language disorder (DLD) is one of the most common neurodevelopmental conditions. Due to variable rates of language growth in children under 5 years, the early identification of children with DLD is challenging. Early indicators are often outlined by speech pathology regulatory bodies and other developmental services as evidence to empower caregivers in the early identification of DLD. AIMS: To test the predictive relationship between parent-reported early indicators and the likelihood of children meeting diagnostic criteria for DLD at 10 years of age as determined by

Parents seek early intervention services for a two-year-old without autism.

Sam is a 27-month-old boy who you have followed since birth. He lives with his parents in a small resort town approximately 90 miles outside a major city. Both his parents are professionals in their late 30s and have been highly involved in his care since birth. At the 12-month visit, they were concerned about his difficulty regulating. He was not sleeping through the night and had significant difficulty with baths. His physical examination and growth were normal. His eye contact was good, although it was difficult to see him smile. He had 1 or 2 words and was beginning to walk independently.A

Low-cost fine-tuning of SmolVLA with smoothness regularization and chunk-aware auxiliary supervision for language-conditioned manipulation.

Vision-Language-Action (VLA) models offer a unified interface for language-conditioned manipulation. This paper reports a negative result: a low-cost post-training recipe for SmolVLA with LoRA, a temporal smoothness regularizer, and chunk-aware auxiliary supervision from action-variation pseudo labels does not improve manipulation success under a 30,000-step budget (approximately 1/67 of the original 2,000,000-step SmolVLA recipe). Because the auxiliary classifier is used only during training and the runtime chunk length remains fixed at k = 8, we denote the configuration SmolVLA+LoRA+S&C (smo

Mapping the cognitive landscape: a pilot study of domain-specific outcomes following frontal lobe resection in children with epilepsy.

BACKGROUND: Pediatric frontal lobe epilepsy surgery can achieve seizure freedom, but cognitive outcomes are heterogeneous and poorly predicted by global measures. This pilot study aimed to implement a domain-specific cognitive assessment protocol and perform an exploratory analysis to identify clinical variables that may influence postoperative outcomes, thereby generating hypotheses for future research. METHODS: This retrospective pilot study included 17 pediatric patients with drug-resistant epilepsy (DRE) who underwent frontal lobe resection. All patients completed the Computerized Cogniti

A step toward inclusion: A transparent dataset for automatic text simplification for screen reader users.

GeoSimp is a Spanish-language dataset for automatic text simplification (ATS), comprising approximately 3200 discourse segments derived from peer-reviewed geological scientific articles. The dataset was constructed to address two documented gaps in existing ATS resources: the limited availability of Spanish-language corpora for text simplification and text complexity classification, and the scarcity of domain-specific datasets focused on specialized scientific writing. Source texts were drawn exclusively from articles published in the Revista Geológica de América Central, a peer-reviewed, open

MINERVA OPS: A computational framework for the representation and recognition of orthographic, phonological, and semantic associates.

Remembering is constructive. Thus, a complete account of memory must predict patterns in both veridical and false recognition. Ideally, that account will predict the content of what people falsely remember, not just that they falsely remember. To move towards that goal, we need theories of both representation and retrieval as well as an account that joins those two accounts into a cohesive whole. To work towards that goal, Reid and Jamieson Journal of Memory and Language, 129, 104397, (2023) previously modelled false recognition in the Deese-Roediger-McDermott paradigm by importing vectors tha

Longitudinal Evaluation of Auditory and Speech Performance in Pediatric Cochlear Implant Recipients Using IT-MAIS, MESP, CAP, and SIR Scales.

Considerable variability in auditory and speech outcomes among pediatric cochlear implant (CI) recipients has raised ongoing debate regarding whether early auditory perception directly determines later speech intelligibility, particularly in tonal language environments where pitch cues are critical for communication. This longitudinal study evaluated 64 prelingually deaf children aged 1-5 years over 12 months following unilateral implantation, integrating four standardized behavioral scales-the Infant-Toddler Meaningful Auditory Integration Scale (IT-MAIS), Categories of Auditory Performance (

Plasma Metabolite Associations with Incident Heart Failure with Reduced and Preserved Ejection Fraction.

BACKGROUND: Prior studies on metabolite associations with incident heart failure (HF) used billing code-based definitions and lacked the data on left ventricular ejection fraction needed to determine associations with HF with reduced (HFrEF) and preserved (HFpEF) ejection fraction. OBJECTIVES: Identify potentially causal plasma metabolite associations with HFrEF and HFpEF, ascertained using a validated machine learning- and natural language processing-based algorithm, in up to 38,000 individuals. METHODS: We included MGB Biobank participants who had available metabolomics data and no history

The relationship between radiotherapy dose and cognitive outcomes in functional brain network hubs of glioma patients with predominantly left frontal lobe lesions.

BACKGROUND: Emerging evidence suggests that cognitive dysfunction may result from damage to the brain's functional network. This study explores the dose-dependent susceptibility of functional hubs to radiotherapy (RT) and associations with cognitive outcomes. METHODS: Attention, language, memory, motor, and executive functioning were assessed ≥1-year post-radiotherapy in 39 WHO grade 2 or 3 glioma patients with lesions predominantly located in the left frontal lobe and 50 matched healthy controls. Using resting-state functional imaging, weighted functional graphs were constructed for each par

Changes in Phonological Awareness Following Combined Phonetic-Phonological Intervention in Children With Cleft Palate.

PURPOSE: Phonological awareness (PA) is a foundational skill for literacy development and may be impacted in children with cleft palate with or without cleft lip (CP ± L), as studies have reported associations between atypical speech sound production in this population and reduced PA skills. This study examined (a) the relationship between pre-intervention PA and the presence of nonoral compensatory speech errors in children with CP ± L and (b) the effects of phonetic-based versus combined phonetic-phonological speech and language intervention on PA across syllabic, intrasyllabic, and phonemic

Validated Questionnaires for the Assessment of Italian Patients with Pelvic Floor Dysfunctions: A Systematic Review.

INTRODUCTION: Pelvic floor dysfunction (PFD) is a pathological condition that significantly impacts women's health. It is therefore necessary to correctly diagnose it and offer the most appropriate treatment for her. Patient-reported outcomes (PROs) have been established, which include questionnaires measuring quality of life (QoL) and symptoms. Although several English-language questionnaires are available to assess PFDs and QoL concerns, relatively few have been validated for use in Italian. With a focus on validated PROs in Italian for assessing PFDs, this article aimed to provide a systema

Evaluating the Efficacy of Large Language Models in CPT Coding for Craniofacial Surgery: A Comparative Analysis.

BACKGROUND: The advent of Large Language Models (LLMs) like ChatGPT has introduced significant advancements in various surgical disciplines. These developments have led to an increased interest in the utilization of LLMs for Current Procedural Terminology (CPT) coding in surgery. With CPT coding being a complex and time-consuming process, often exacerbated by the scarcity of professional coders, there is a pressing need for innovative solutions to enhance coding efficiency and accuracy. METHODS: This observational study evaluated the effectiveness of 5 publicly available large language models

Hippocampal resection in temporal lobe epilepsy: Do we need to resect the tail?

INTRODUCTION: Anteromesial temporal lobe resection is the most common surgical technique used to treat drug-resistant mesial temporal lobe epilepsy, particularly when secondary to hippocampal sclerosis. Structural and functional imaging data suggest the importance of sparing the posterior hippocampus for minimising language and memory deficits. Recent work has challenged the view that maximal posterior hippocampal resection improves seizure outcome. This study was designed to assess whether resection of posterior hippocampal atrophy was associated with improved seizure outcome. METHODS: Retro

Validation studies of rheumatoid arthritis patient-reported outcome measures in populations at risk for inequity: A systematic review and analysis using the OMERACT summary of measurement properties equity table.

BACKGROUND: Existing patient-reported outcome measures (PROMs) in rheumatoid arthritis (RA) may be limited in their applicability to populations that are at risk for inequities. We conducted a systematic review to identify and rate evidence in the validation studies for PROMs in populations at risk for inequity. METHODS: A systematic review of MEDLINE and EMBASE was completed. The search strategy was developed to identify measurement property studies for PROMs of interest (selected pain, disease activity, global evaluation and quality of life scales) in patients with RA.
