Ectopia lentis (EL) is a condition that can either herald underlying systemic conditions, or be isolated. The recent expansion in the genetics of these conditions has furthered the understanding of the underlying molecular aetiology. It is becoming apparent that novel genes, and in particular the ADAMTS (a disintegrin and metalloproteinase with thrombospondin motifs) family, are important in ocular development. PURPOSE OF REVIEW: To outline current options for managing astigmatism during cataract surgery and update readers on new techniques for improving the final refractive outcome in these patients.
RECENT FINDINGS: Recent studies continue to show the effectiveness of peripheral corneal relaxing incisions (PCRIs) for correcting astigmatism in combination with monofocal, multifocal, and toric intraocular lens (IOL) implants. The options in toric IOLs are expanding. Intraoperative aberrometry is a new tool that can improve the accuracy of PCRIs and toric IOLs.

PURPOSE: To examine psychological factors and other mechanisms for soft contact lens (CL)-related end-of-day symptoms of dryness and associated dropping out from lens wear.
METHODS: A PubMed search for articles related to dryness, comfort, and discomfort in CLs was conducted. Comparisons with rigid lens adaptation have been used to construct and examine a hypothesis that high levels of initial soft lens comfort increase expectations of an absence of lens awareness that result in end-of-day symptoms being more difficult to tolerate.

Methods of intraocular lens (IOL) calculation in non-standard clinical situations have been developed and introduced. These include a formula that improves the calculation accuracy in patients with relatively short anterior-posterior axis and another method for eyes after previous radial keratotomy. A comparative clinical evaluation of the proposed methods has been performed. The results indicate good perspectives for their use in ocular surgery.

A 52 year-old, contact lens-wearing man presented with progressive right eye pain and redness for one month. He had been evaluated and treated for necrotizing scleritis by multiple eye care specialists prior to presentation. We describe the case of a 37-year-old caucasian man with a history of keratoconus and long-term hard contact lens wear. Histological analysis of the corneal button after penetrating keratoplasty revealed staining with Congo red and apple green birefringence detected using polarized light. In the absence of other systemic or ocular pathology, the authors infer this amyloid deposition to be secondary to keratoconus and endeavor to discuss the possible etiopathogenesis of the disease.

PURPOSE: To describe a rare complication of classical EEC technique with posterior chamber IOL implantation in the sulcus.
PATIENTS AND METHODS: This is a retrospective study of 296 eyes which underwent EEC and CP implantation. None was operated with phacoemulsification. The posterior chamber IOL originated from 4 renowned manufacturers.
RESULTS: In 25 eyes, at 2 to 7 month after surgery, we noticed a mydriasis with pupillary areflexia, though the majority (20 eyes) presented a miotic pupil in the days following surgery.

Polymer solutions can fill any potential irregular cavity using minimally invasive techniques and thus have potential uses in ophthalmology. We prepared acrylamide hydrogels containing disulfide bonds by free radical polymerization in aqueous ethanol.
