Cord blood platelet gel for the treatment of inherited epidermolysis bullosa.

The management of patients affected by epidemolysis bullosa requires an integrated approach involving different specialties. A cornerstone of clinical management is the prevention and treatment of mechanobullous ulcerations of the patient's skin, which significantly impact the quality of life and can be the cause of septic and neoplatic complications. This article describes the preliminary clinical evaluation of the use of allogeneic cord blood platelet gel, a novel blood component obtained from umbilical cord blood of healthy, term neonates, for the treatment of skin ulcers in patients with d

Ominous triad triggered by high-dose glucocorticosteroid therapy.

Glucocorticosteroids (CS) play a key role in the treatment of numerous diseases. Nonetheless, they can be accompanied by several adverse effects. We present the case of a 51-year-old woman who was treated with high-dose CS for a relapse of her multiple sclerosis. After 5 days of treatment, the patient developed severe diabetic ketoacidosis, hypertriglyceridemia and acute pancreatitis-a potentially life-threatening triad which has previously been described, in our case, however, for the first time as a complication of CS therapy. Our patient's condition was further aggravated by a circulatory s

Guidelines for specialized nutritional and metabolic support in the critically-ill patient: update. Consensus SEMICYUC-SENPE: nutritional assessment.

Current parameters to assess nutritional status in critically-ill patients are useful to evaluate nutritional status prior to admission to the intensive care unit. However, these parameters are of little utility once the patient's nutritional status has been altered by the acute process and its treatment. Changes in water distribution affect anthropometric variables and biochemical biomarkers, which in turn are affected by synthesis and degradation processes. Increased plasma levels of prealbumin and retinol -proteins with a short half-life- can indicate adequate response to nutritional suppor

A case study of Muslims' perspectives of expanded terminal sedation:addressing the elephant in the room.

BACKGROUND: Recently, the concept of expanded terminal sedation emerged to describe using sedation at the end of life in cases beyond the usual use. Using this sedation could be a stressful ethical encounter for healthcare providers. CASE: In this paper, we describe a case of a Muslim palliative care nurse who cared for a patient with cancer who requested expanded terminal sedation. The palliative care nurse described that his initial response to the expanded terminal sedation order was refusing to start the sedation because he believed the patient was not terminally ill and was concerned abo

The impact, perceptions and needs of parents of children with epidermolysis bullosa.

BACKGROUND: Epidermolysis bullosa (EB) is a rare, incurable genodermatosis that presents with blistering and skin fragility. Complications can be localised or generalised, limited to the skin or have systemic effects resulting in death. Caring for a child with this painful condition can have a profound effect on the quality of life of parents and the family. There is currently no published research on the lived experience of parents caring for a child with EB in a resource-limited environment in Africa. METHOD: This qualitative research used interpretative phenomenological analysis with the a

Metformin-Induced Lactic Acidosis Following Intentional Overdose: A Case Report and Mini Review.

Metformin-induced lactic acidosis (MILA) is a rare but potentially life-threatening condition. It is mainly related to metformin accumulation secondary to overdose or severe kidney failure. We report a case of MILA in a 17-year-old non-diabetic female, who attempted suicide with a high dose of metformin (30 tablets of metformin 500 mg). On arrival, she was fully conscious with normal vital signs. She was admitted to the intensive care unit for close observation. Her laboratory investigations showed development of high anion gap metabolic acidosis, worsening lactatemia, and a drop in blood pres

Eating well for ageing well: the role of diet and nutrition in promoting healthspan and longevity.

Healthy ageing and longevity have emerged as urgent public health priorities amid global population ageing and declining birth rates. This review synthesises empirical evidence highlighting the essential role of diet and nutrition in preventing chronic diseases and supporting functional independence in later life. The review was organised using a problem-solving approach to address three core questions: 'What' evidence links food and nutrition with positive health outcomes; 'Why' do specific dietary components affect biological ageing - via mechanisms such as nutrient sensing, inflammation mod

Chronic Intestinal Pseudoobstruction.

Patients with chronic intestinal pseudoobstruction (CIP) experience a constellation of symptoms including abdominal pain, nausea, fullness, and malaise which fluctuates in severity and invariably result in a diminished quality of life. Though surgical resection or transplantation may be an option for some, there currently is no cure for CIP. Thus, management strategies utilize pharmacologic, intravenous, endoscopic, and surgical techniques to promote transit, minimize painful bloating, reduce complications of stasis, and improve quality of life.
