Demystifying Split Liver Transplantation in Adults: Factors Associated with Early Graft Survival in the New Era.

BACKGROUND: Split liver transplantation (SLT) can expand the donor pool by generating 2 grafts from 1 organ. However, SLT only consists of <5% of United States liver transplants. Factors determining adequate SLT prognosis remain understudied. METHODS: A retrospective study of adult deceased-donor liver transplant recipients (January 2015 to September 2025) was conducted using the United Network for Organ Sharing/Organ Procurement and Transplantation Network database. The primary outcome was graft survival (GS) at 3 and 12 months. SLT recipients were analyzed by graft laterality, splitting tec

Elevated Preoperative Systemic Immune-Inflammation Index Independently Predicts 30-Day Mortality After Living Donor Liver Transplantation.

Background: Systemic inflammation significantly impacts graft survival and clinical outcomes following living donor liver transplantation (LDLT). The systemic immune-inflammation index (SII), which integrates peripheral neutrophil, platelet, and lymphocyte counts, has shown prognostic value in various clinical settings, but its role in LDLT has not been thoroughly investigated. Methods: This retrospective cohort study included 378 consecutive adult patients with end-stage liver disease (ESLD) who underwent primary LDLT between March 2016 and February 2025. The SII was calculated as (neutrophil

Outcomes of Liver Transplantation Without Alcohol Abstinence in Alcohol-Associated Hepatitis With Original MELD Score ≥30.

BACKGROUND Achieving alcohol abstinence before standard liver transplantation (SLT) for alcohol-associated liver disease (ALD) is commonly accepted. However, the 3-month mortality rate for alcohol-associated hepatitis (AH) patients with extremely high Model for End-Stage Liver Disease (MELD) scores (≥30) exceeds 50%, suggesting insufficient time for these patients to achieve sobriety. Current data on outcomes for non-abstinent individuals in this high-risk group undergoing early liver transplantation (ELT) is limited. MATERIAL AND METHODS This single-center observational study (May 2002 to Oct

Recipient Social Determinants and Geographic Access in DCD Liver Transplantation: Early Lessons During the Perfusion era.

BACKGROUND: Normothermic machine perfusion (NMP) and normothermic regional perfusion (NRP) have expanded donation after circulatory death (DCD) liver transplantation (LT). However, cold ischemia time (CIT) potentially restricts access to DCD LT for recipients living far from transplant centers. The impact of perfusion technology on DCD LT access for geographically distant patients remains underexplored. METHODS: All adult DCD LT at a transplant center from January 2017 to December 2024 were reviewed. Recipients were categorized by perfusion method:static cold storage (SCS) only, NRP, or NMP.

Intraoperative renal oxygen saturation trajectories are associated with early postoperative acute kidney injury after liver transplantation.

Acute kidney injury (AKI) is frequent after liver transplantation, yet kidney-specific real-time intraoperative monitoring is limited. We hypothesized that intraoperative renal regional oxygen saturation (rSkO₂) trajectories—particularly a rapid decline—are independently associated with AKI within 7 days after liver transplantation. In this single-center prospective cohort (January 2022–January 2024), adults undergoing liver transplantation with baseline eGFR ≥ 60 mL/min/1.73 m² and total bilirubin ≤ 300 µmol/L underwent continuous bilateral renal near-infrared spectroscopy monitoring (INVOS 5

Effect of perioperative Sonoclot monitoring on early thrombosis after pediatric liver transplantation: a prospective observational study.

BACKGROUND: Postoperative thrombosis is the most serious and common complication for children. The Sonoclot device is used to monitor coagulation function in transplant patients. However, no Sonoclot-based threshold for identifying children at risk of thrombosis has been established. This study aimed to identify the Sonoclot parameter and its optimal cutoff value for predicting early postoperative thrombosis in pediatric living-donor liver transplantation. METHODS: This was a single-center, prospective and observational study. The study was performed in Tianjin First Central Hospital, Tianjin

Liver Transplantation for Biliary Atresia: Sex-Based Equity but Persistent Racial and Donor-Related Risk Factors.

OBJECTIVES: Liver transplantation can be salvage or primary therapy for children with biliary atresia. In this study, we investigated the effects of sex, race, and donor type on outcomes. MATERIALS AND METHODS: We used the United Network for Organ Sharing database to perform a retrospective analysis of 1382 pediatric transplant recipients with biliary atresia, in which we compared sex, race, and donor type using standard statistical tests. We analyzed graft failure and mortality with Kaplan -Meier and Cox regression. RESULTS: No significant sex -based differences were found. Black (69.6 % )

Cardiac Allograft Immunoprotection in Combined Heart-Liver Transplantation: Mechanisms and Clinical Translation for the Sensitized Candidate.

Highly sensitized heart transplant candidates face substantially restricted access to transplantation and adverse waitlist outcomes, and no pharmacological desensitization strategy has consistently mitigated posttransplant rejection risk. A growing number of these candidates carry an independent indication for liver transplantation. For this population, combined heart-liver transplantation addresses both organ failures, and evidence demonstrates that the liver allograft confers immunological protection to the cardiac allograft through multiple convergent mechanisms. Clinical data confirm that

Intraoperative Terlipressin During Liver Transplantation Is Associated with Reduced Vasoactive Requirements and Lower Postoperative Troponin Release.

Background/Objectives: Intraoperative hemodynamic instability during liver transplantation (LT) is common and results from cirrhosis-related circulatory dysfunction and profound hemodynamic changes during graft reperfusion. High catecholamine requirements may contribute to secondary organ injury, including myocardial damage. Terlipressin, a selective vasopressin V1 receptor agonist, has been shown to improve hemodynamic stability during LT; however, the impact of a short, targeted intraoperative infusion on cardiac biomarkers remains unclear. Methods: This retrospective single-center study inc

Upper-Limb Compartment Syndrome Following Rapid Infusion Catheter Extravasation During Orthotopic Liver Transplantation: A Case Report.

Rapid infusion catheters (RICs) provide high-flow peripheral venous access for major haemorrhage but can cause severe tissue injury if pressurised infusion becomes extravascular. We report upper-limb acute compartment syndrome during orthotopic liver transplantation following transfusion through an 8.5 Fr Arrow RIC (Teleflex Incorporated, Morrisville, NC, USA). A female in her late 60s with decompensated cryptogenic cirrhosis underwent urgent transplantation. Four hours before surgery, an attempt to place an 8.5 Fr RIC in the left antecubital fossa was abandoned because of guidewire resistance

Hypothermic Oxygenated Perfusion Versus Static Cold Storage in Liver Transplantation: A Meta- Analysis of Randomized Trials.

BACKGROUND: Hypothermic oxygenated machine perfusion (HOPE) is an alternative to static cold storage (SCS) for liver graft preservation, aiming to reduce ischemia-reperfusion injury (IRI) and improve post-transplant outcomes. METHODS: We systematically reviewed randomized controlled trials (RCTs) comparing HOPE with SCS in adult liver transplantation (MEDLINE, EMBASE, CENTRAL to July 2026). Outcomes included early allograft dysfunction (EAD), primary non-function (PNF), major postoperative complications, biliary complications, including non-anastomotic strictures (NAS), graft survival, and re

Normothermic machine perfusion versus static cold storage for liver transplantation: a systematic review and meta-analysis.

PURPOSE: To compare the effectiveness of normothermic machine perfusion (NMP) with static cold storage (SCS) in preserving transplantable livers, focusing on early postoperative complications. METHODS: A PRISMA 2020-compliant search of PubMed, Embase, Scopus, Cochrane Library and Web of Science identified comparative studies of NMP versus SCS. Five studies met the criteria (two randomized controlled trials, and three observational cohorts; 630 recipients). Random-effects meta-analysis was performed in RStudio, reporting risk ratio (RR), 95% confidence interval (95%CI), p-value and heterogenei

Normothermic Machine Perfusion, Patient-Centered Recovery, and Graft Survival After Donation-After-Circulatory-Death Liver Transplantation.

BACKGROUND: Normothermic machine perfusion (NMP) may reduce preservation injury in donation-after-circulatory-death (DCD) liver transplantation, but its association with patient-centered recovery and longer-term graft survival remains uncertain. STUDY DESIGN: We retrospectively analyzed 306 consecutive adult DCD liver transplant recipients at a single center from January 2019 through December 2022, comparing NMP (n=95) with static cold storage (SCS; n=211). The primary endpoint was days alive and out of hospital at 365 days (DAOH365). Quasi-Poisson regression evaluated factors associated with

The impact of early allograft dysfunction severity on graft and recipient outcomes in pediatric liver transplantation.

INTRODUCTION AND OBJECTIVES: Early allograft dysfunction (EAD) substantially compromises graft and recipient outcomes; the clinical consequences of EAD vary according to severity. Research focusing on EAD in pediatric liver transplantation (pLT) remains notably limited. This study sought to validate the EAD severity grading models in pLT and assess the impact of varying severity grades of EAD on both graft and recipient outcomes. PATIENTS AND METHODS: This retrospective study enrolled 360 patients who underwent pLT (living-donor liver transplantation (LDLT)=299, deceased-donor liver transplan

Overall Outcomes of Liver Transplantation Using Upfront Normothermic Machine Perfusion: A Real-World Single-Center Experience.

OBJECTIVE: To determine whether normothermic machine perfusion (NMP) mitigates the historical survival disadvantage of donation after circulatory death (DCD) liver transplantation compared with donation after brain death (DBD) livers. SUMMARY BACKGROUND DATA: NMP has improved outcomes in DCD LT compared with static cold storage (SCS), but whether it eliminates the historical disparity relative to DBD transplantation remains uncertain. METHODS: We performed a retrospective cohort study of adult LT at a high-volume US center between 2019 and 2024. Outcomes of DCD-NMP grafts were compared with
