Provox Life HMEs for Pulmonary Rehabilitation in HME-Naïve Laryngectomy Patients: A Randomized Controlled Trial.

OBJECTIVE: To evaluate whether Provox Life Heat and Moisture Exchangers (HMEs) and attachments enable HME-naïve laryngectomized patients to establish a 24-h HME routine, and whether the resulting effect on pulmonary symptoms is sustained over 9 months, compared with standard care. METHODS: In this prospective, randomized controlled trial, 79 HME-naïve patients post-total laryngectomy at two Brazilian centers were randomized to the Provox Life group (n = 41) or a control group without HMEs (n = 38). Data were collected at baseline and at 3, 6, and 9 months. The primary endpoint was the Cough S

External validation and comparative performance of Marshall, Rotterdam, and Helsinki computed tomography scores in adult traumatic brain injury.

PURPOSE: Computed tomography (CT)-based scoring systems enable standardized quantification of intracranial pathology following traumatic brain injury (TBI). Despite widespread clinical application, external validation studies comparing the Marshall, Rotterdam, and Helsinki CT scores remain limited, particularly in middle-income settings. This study aimed to externally validate these three CT scoring systems and evaluate their incremental prognostic value beyond clinical variables. METHODS: This retrospective cohort study included adult patients with traumatic brain injury requiring hospitaliz

Outcomes of post-approval non-interventional safety studies in pregnancy: wide variation demonstrates need for further standardization.

BACKGROUND: Upon product approval, sponsors are often required by regulatory authorities (e.g., the United States [US] Food and Drug Administration [FDA] or the European Medicines Agency [EMA]) to conduct post-approval safety studies in pregnant populations. Existing guidance from regulatory authorities provide important recommendations of study outcomes to evaluate in such studies, however harmonized guidance on a priority list of outcomes that all studies must evaluate is currently lacking. This creates inefficiencies in study conduct and limits comparability. OBJECTIVES: The article aims t

Beyond the classic ketogenic diet: A systematic review of alternative and adjunctive therapies for GLUT1 deficiency syndrome.

BACKGROUND: Glucose transporter type 1 deficiency syndrome (GLUT1-DS) is a rare, paediatric onset, neurometabolic disorder caused by impaired cerebral glucose transport, leading to chronic brain energy deficiency. The classic ketogenic diet (CKD) is its established cornerstone therapy but it is not universally effective, well tolerated or sustainable. A growing number of alternative and adjunctive therapies has been explored, but their clinical roles remain unclear due to the fragmented and heterogeneous nature of the evidence. OBJECTIVES: Systematic synthesis and appraisal of clinical eviden

Gonadotropin therapy in idiopathic hypogonadal non-obstructive azoospermia (APHRODITE Groups 3-4): a multicenter randomized controlled trial.

INTRODUCTION: Idiopathic non-obstructive azoospermia (NOA) is a severe form of male infertility with limited non-surgical treatment options. Microdissection testicular sperm extraction (micro-TESE) remains the standard approach for sperm retrieval, yet success rates vary substantially, and many patients undergo surgery without successful sperm recovery. Hormonal optimization using human chorionic gonadotropin (hCG) with or without follicle-stimulating hormone (FSH) has been proposed to improve spermatogenesis in hypogonadal men, but evidence from randomized controlled trials is lacking. This m

Complementary and Integrative Health Approaches for Low Back Pain in Veterans: A Narrative Review.

INTRODUCTION: Chronic low back pain (cLBP) is the most prevalent chronic pain condition among veterans, contributing to disability, reduced quality of life, and opioid dependence. Complementary and integrative health (CIH) approaches offer evidence-based, non-pharmacological alternatives that may improve pain management, enhance patient satisfaction, lower healthcare costs, and reduce opioid dependence. MATERIALS AND METHODS: The objective of the study was to synthesize current evidence on CIH approaches for veterans with cLBP, evaluate their impact on pain, function, and well-being, and iden

Phospholipid-Based Delivery System Optimizes the Solubility and Systemic Exposure of Palmitoylethanolamide and Supports Clinical Benefits in Chronic Neuropathic Low Back Pain.

Background: Chronic neuropathic low back pain (LBP) is a prevalent health condition and difficult to treat. Conventional therapies often provide limited relief and raise safety concerns. Supplemental palmitoylethanolamide (PEA), an endogenous fatty acid amide with analgesic and anti-inflammatory properties, has shown benefits in neuropathic pain, but its application as a supportive strategy has been limited by poor oral bioavailability. Objectives: This study aimed to investigate a phospholipid-based palmitoylethanolamide formulation (PEA-PL, Cronilief™), developed using Phytosome™ delivery te

Does Hi-PoAD technique work for congenital scoliosis: a single centre case series.

PURPOSE: To evaluate the efficacy of the Hi-PoAD technique in treating select cases of Congenital scoliosis. METHODS: A retrospective study was conducted, including 38 patients with congenital scoliosis who were operated on by posterior single-stage surgery using the Hi-PoAD technique, and were actively followed for a minimum follow-up time of two years. This special technique involved a combination of high-density pedicle screws, Ponte osteotomies, asymmetric rod contouring, and direct vertebral rotation. All cases were performed between 2016 and 2022 at a single tertiary care hospital by a

Validating methods for inferring co-occurring diseases: a flexible framework for simulating synthetic data.

BACKGROUND: The validation of methods is an integral part of statistical research, defining conditions under which methods yield reliable results. Empirical validation requires a solid data basis to control and manage relevant characteristics like sample size, dimensionality, and underlying dependency structures. Real-world data often fails to meet these requirements, particularly in medical contexts where privacy regulations restrict availability. For this reason, synthetic data is an effective alternative for method validation. However, generating synthetic data is demanding when it must pre

Impact of preoperative halo-gravity traction on radiographic and surgical outcomes following posterior spinal fusion in osteogenesis imperfecta: a comparative study.

PURPOSE: Osteogenesis Imperfecta (OI) is a rare connective tissue disorder often associated with severe, brace-resistant scoliosis. Posterior spinal fusion (PSF) with pedicle screws can achieve up to 60% coronal correction, while preoperative halo-gravity traction (HGT) may provide additional benefits but carries potential risks. This study evaluated whether HGT offers perioperative or radiographic advantages compared with PSF alone in pediatric OI patients. METHODS: Thirty-six patients treated between 2002 and 2020 with ≥ 2 years' follow-up were retrospectively analyzed. Patients were divide

Locking plate fixation compared with tension-band wiring for patellar fractures: a multicentre randomized controlled trial.

AIMS: Patellar fractures are associated with considerable postoperative morbidity, and tension-band wiring has high complication rates and suboptimal outcomes. Locking plate fixation is a recent surgical innovation which confers superior biomechanical stability. However, there is little information about the clinical outcomes of this method of treatment. The aim of this study was to compare locking plate fixation with tension-band wiring in patients requiring surgical treatment for a patellar fracture. METHODS: In this patient- and assessor-blinded, multicentre randomized trial, 122 adults wi

Early SLE may follow a fast comorbidity/multimorbidity trajectory associated with flare burden and glucocorticoid exposure.

BACKGROUND: Patients with systemic lupus erythematosus (SLE) carry a substantial comorbidity load associated with worse prognosis, yet most evidence pertains to long-standing disease. How comorbidities-particularly multimorbidity as a marker of overall health burden-evolve at early stages, and whether conditions co-aggregate into clinically meaningful clusters, remains unclear. METHODS: An inception cohort (≤2 years from diagnosis) of 311 patients with SLE with 5-year monitoring of 140 comorbidities, disease activity, treatments, organ damage and adverse events/hospitalisations. Initial Experience With Special Medical Physics Consultations for Proton Reirradiation.

PURPOSE: To reduce the volume of tissues receiving additional radiation dose, proton therapy is often used in patients receiving reirradiation (reRT). Standardized workflows to systematically assess cumulative dose and potential risks of reRT are limited. We present our institutional implementation of a proton reRT SMPC, characterizing its workflow and initial clinical impact. METHODS AND MATERIALS: Using published literature, physicians agreed upon institutional organs-at-risk (OARs) equivalent dose in 2-Gy fraction (EQD2) goals; constant relative biological effectiveness of 1.1 was applied

From insulin resistance to incretin receptor agonism in the prevention of type 2 diabetes mellitus in obese individuals.

Type 2 diabetes mellitus (T2DM) represents one of the most significant metabolic challenges of the modern era, with more than 828 million people worldwide living with diabetes. Obesity and insulin resistance are major modifiable contributors to T2DM risk and progression, although the disease is heterogeneous and arises from complex interactions among β-cell dysfunction, hepatic glucose dysregulation, adipose-tissue dysfunction, altered incretin biology, chronic low-grade inflammation, and genetic susceptibility. In individuals with obesity and insulin resistance, progression through prediabete
