On the blood-brain barrier to peptides: specific binding of atrial natriuretic peptide in vivo and in vitro.

Using the intracarotid bolus injection technique, a saturable binding of [125I]atrial natriuretic peptide (ANP) was found in 8 blood-brain barrier (BBB)-protected rat brain regions as well as in the pineal gland, choroid plexus, neurointermediate and anterior lobes of the pituitary, i.e. structures lacking a BBB. The presence of specific ANP binding on the BBB, here shown for the first time by an in vivo approach, was evidenced concomitantly in vitro by incubation of isolated microvessels. A single-class high affinity binding without regional differences was obtained with Kd = 0.23 nM and Bmax

Effect of a new antithrombotic agent (defibrotide) in acute renal failure due to thrombotic microangiopathy.

8 patients with thrombotic microangiopathy were treated with a new antithrombotic agent, defibrotide. This drug displays considerable fibrinolytic and antithrombotic activity, and induces generation and release of a prostacyclin-like substance from vascular tissue. At admission all patients presented severe renal involvement and coagulation abnormalities. Neurological manifestations were present in 6. Defibrotide administration was followed by recovery of renal function in 6, disappearance of neurological symptoms and coagulation abnormalities in all patients. The use of defibrotide was not as

Laser Speckle Contrast Imaging for Assessment of Cutaneous Perfusion and Treatment Response During Hyperbaric Oxygen Therapy in Chronic Wounds: A Case Series.

Hyperbaric oxygen therapy (HBOT) is an adjunctive treatment for selected chronic wounds with microcirculatory impairment, but treatment response is typically assessed clinically after several weeks. This case series evaluated the feasibility and potential utility of laser speckle contrast imaging (LSCI) for monitoring cutaneous perfusion during HBOT. Six patients with chronic wounds refractory to conventional management underwent HBOT. Cutaneous perfusion was assessed using LSCI before and after HBOT sessions at three predefined regions of interest: referent healthy skin, wound margin and woun

Postocclusive muscle reoxygenation kinetics and nitric oxide bioavailability following 10-day bed rest.

Bed rest (BR) studies have demonstrated the detrimental effects of microgravity and physical inactivity on cardiovascular function, including the negative consequences on microvascular function and O2 extraction. However, whether a short period of BR affects microvascular responsiveness and its molecular mediator, nitric oxide (NO), remain unclear. Nine male volunteers (23 ± 5 yr) completed 10-day horizontal BR. Microvascular postocclusive reactive hyperemia in the vastus lateralis (VL) and rectus femoris (RF) muscles were assessed by near-infrared spectroscopy during a vascular occlusion test

Epicardial Fat Tissue: A Potential Marker for Coronary Microvascular Dysfunction.

BACKGROUND: Coronary microvascular dysfunction (CMD), which mimics symptoms of obstructive coronary artery disease, has significant prognostic implications. While epicardial adipose tissue normally has a protective role, increased epicardial adipose tissue is associated with inflammation and may contribute to CMD. However, a direct correlation remains unclear. We aimed to investigate this association. METHODS AND RESULTS: The CMDR (Coronary Microvascular Disease Registry) is a prospective, 2-center registry that is enrolling patients with angina and nonobstructive coronary artery disease who

Correlation Between Index of Microcirculatory Resistance and Angiography-Derived Microcirculatory Resistance in Takotsubo Syndrome.

Takotsubo syndrome (TTS) is an acute stress-induced cardiomyopathy characterized by transient left ventricular dysfunction, often mimicking acute coronary syndrome (ACS). Coronary microvascular dysfunction has been implicated in its pathophysiology. The index of microcirculatory resistance (IMR) is the invasive reference standard for microvascular assessment, whereas angiography-derived microcirculatory resistance (AMR) offers a noninvasive alternative. The correlation between IMR and AMR in TTS has not yet been established. A prospective monocentric study was conducted at Clinique Saint-Hilai

Angiography-Derived Microcirculatory Resistance in Detecting Microvascular Obstruction and Predicting Heart Failure After STEMI.

BACKGROUND: Microvascular obstruction (MVO) is associated with heart failure (HF) following ST-segment-elevation myocardial infarction. Angiography-derived microcirculatory resistance (AMR), a wire- and adenosine-free measure, may facilitate early assessment of microvascular function post-primary percutaneous coronary intervention. This study aimed to evaluate the ability of AMR to detect MVO and its prognostic value for predicting HF in patients with ST-segment-elevation myocardial infarction post-primary percutaneous coronary intervention. METHODS: Patients with consecutive ST-segment-eleva

Coronary microvascular dysfunction and inflammation: Insights from the Coronary Microvascular Disease Registry.

BACKGROUND: Coronary microvascular dysfunction (CMD) is associated with various inflammatory conditions that worsen endothelial dysfunction. This study aimed to investigate the relationship between CMD and inflammation using common inflammatory markers derived from complete blood count (CBC) analysis. METHODS: Information was gathered from the Coronary Microvascular Disease Registry to examine the neutrophil-to-lymphocyte ratio (NLR), eosinophil-to-monocyte ratio (EMR), and monocyte-to-high-density lipoprotein ratio (MHR) in a cohort of patients with angina who showed non-obstructive coronary

Coronary Microvascular Disease Registry (CMDR): Study design and rationale.

BACKGROUND: Coronary microvascular dysfunction (CMD) is a prevalent condition among patients with cardiovascular risk factors, leading to a reduced quality of life and an increased risk of major adverse cardiovascular events. Novel invasive techniques have emerged to more accurately diagnose CMD. However, CMD's natural history remains poorly understood due to limited data. To address this knowledge gap, the Coronary Microvascular Disease Registry (CMDR) was established with the primary aim of standardizing comprehensive coronary functional testing and understanding of CMD. DESIGN: CMDR is a p

Angiography-Derived Index of Microcirculatory Resistance to Define the Risk of Early Discharge in STEMI.

BACKGROUND: Patients with ST-segment-elevation myocardial infarction but no coronary microvascular injury are at low risk of early cardiovascular complications (ECC). We aim to assess whether nonhyperemic angiography-derived index of microcirculatory resistance (NH-IMRangio) could be a user-friendly tool to identify patients at low risk of ECC, potentially candidates for expedited care pathway and early hospital discharge. METHODS: Retrospective analysis of 2 independent, international, prospective, observational cohorts included 568 patients with ST-segment-elevation myocardial infarction. N

Measurements of cerebral microvascular blood flow, oxygenation, and morphology in a mouse model of whole-brain irradiation-induced cognitive impairment by two-photon microscopy and optical coherence tomography: evidence for microvascular injury in the cerebral white matter.

Whole-brain irradiation (WBI, also known as whole-brain radiation therapy) is a mainstay treatment modality for patients with multiple brain metastases. It is also used as a prophylactic treatment for microscopic tumors that cannot be detected by magnetic resonance imaging. WBI induces a progressive cognitive decline in ~ 50% of the patients surviving over 6 months, significantly compromising the quality of life. There is increasing preclinical evidence that radiation-induced injury to the cerebral microvasculature and accelerated neurovascular senescence plays a central role in this side effe

Microvascular rarefaction in patients with cerebrovascular events.

Capillary density rarefaction and endothelial dysfunction contribute to chronic hypoperfusion and cerebral small vessel disease. Previous animal experiments revealed spatiotemporal microvascular remodeling directing post-stroke brain reorganization. We hypothesized that microcirculatory changes during acute cerebrovascular events could be reflected systemically and visualized sublingually. In a prospective observational trial in vivo sublingual sidestream darkfield videomicroscopy was performed in twenty-one patients with either acute stroke (n = 13 ischemic, n = 1 ischemic with hemorrhagic tr
