Eleven lactose malabsorbers were studied to compare the effectiveness of commercially available products recommended for dietary treatment of lactose malabsorption. One product, a commercial lactase preparation, is added to milk for lactose hydrolysis before consumption. The other is a commercial milk product containing lactose-hydrolyzing, nonpathogenic bacteria, Lactobacillus acidophilus. Both of these products are presently recommended for management of lactose malabsorption, although such recommendations have not been validated by controlled studies. Intolerance to lactose and cow's milk protein was prospectively evaluated in 56 infants with apparent colic. A complete blood cell count, sedimentation rate, test for quantitative IgE level, radioallergosorbent test (performed in 15 patients), and at least three stool examinations for pH, presence of reducing substances, and occult blood were performed in 56 infants (46, appropriate size for gestational age; ten, small for gestational age) and results were within normal limits for age. Intestinal lactase assays before and after a Billroth II procedure for chronic duodenal ulcer did not reveal a significant difference. Postoperative lactose intolerance is due to the loss of pyloric function, causing entrance of lactose into the jejunum at a rate exceeding the hydrolytic capacity of the mucosa if the lactase level is low.

Clinical symptoms and lactose malabsorption may occur in persons intolerant to lactose. To predict malabsorption from symptoms and age, the authors used simple regression, multiple regression, and discrimant analysis on data from 436 Chileans. All individual r2 values from simple regression were less than 0.05. In 1969--1970, a simple random sample of 129 Finnish school-aged children was examined to study selective adult-type lactose malabsorption (SLM) in this age category. SLM was found in 8 children. All subjects were reexamined 5 years later. SLM was reconfirmed in these 8 persons and found in 3 additional subjects who had normal lactose absorption in the first examination. Of 40 patients with a partial gastrectomy (Billroth-II) 10 developed a milk intolerance and 11 had a lactase deficiency, the latter in 4 cases appearing together with a decrease in other disaccharidases. Only 2 of the 11 lactase-deficient patients complained of milk intolerance. The Ethanol lactose tolerance test (ELTT) was performed in 21 patients and was found to be abnormal in 6. Q fever was first described in Australia and has since been reported from many countries. Rickettsia burneti infects numerous species of mammals and has been isolated from ticks. Infected cattle, sheep and goats remain healthy but excrete the organism, which is present in high concentration in products of conception and survives in dust for long periods. The ways in which humans may become infected are considered. Many of these infections do not cause symptoms, but some are fatal and the prevention of this disease remains a challenge.
