Toward understanding how altered trunk-pelvis motions and loads during walking after lower limb loss influence lumbar spine mechanics: Insights from ex vivo testing.

Altered trunk-pelvis motions and loads during activities of daily living are biomechanical risk factors for low back pain after lower limb loss. This investigation evaluated the relative contributions of trunk-pelvis motions and loads to spinal laxity using a servohydraulic materials testing system to emulate walking biomechanics. Twelve porcine lumbar spines (age: 12-18 months, mass: 45-68 kg) were exposed to 6 different experimental conditions which comprised of all combinations of trunk-pelvis motion/load conditions (motion, load, and motion + load) and two cohorts (limb loss and uninjured)

Sagittal-oriented articular processes and intervertebral disc degeneration phenotypes can concurrently lead to an increased presence of annulus tears in the cranial adjacent motion segment : a clinical review and biomechanical simulation.

AIMS: Intervertebral disc degeneration (IDD) and sagittal-oriented articular processes can restrict motility and increase stiffness of the motion segment, potentially causing compensatory stress and higher motility in adjacent segments. It is unclear if these factors trigger IDD progression in adjacent segments. This study aimed to elucidate this using functional MRI, and identify biomechanical mechanisms with a validated numerical model. METHODS: Clinical data from 95 patients were retrospectively collected from January 2022 to April 2023. Disc collapse and fibrosis were assessed by disc hei

Thoracic-abdominopelvic contrast-enhanced dual-source computed tomography with a high-pitch acquisition protocol and free breathing for reducing motion artifacts and scan time in patients with cancer: a prospective study.

BACKGROUND: Contrast-enhanced computed tomography (CECT) is critical for cancer management. Although high-pitch dual-source computed tomography (DSCT) effectively reduces radiation and enhances image quality, its clinical benefits in free-breathing thoracic-abdominopelvic scans remain unclear. This study investigated DSCT-based high-pitch CECT in terms of image quality and scan efficiency in non-breath-holding patients with cancer. METHODS: A total of 169 patients with cancer who underwent thoracic-abdominopelvic contrast-enhanced DSCT were enrolled: 84 non-breath-holding patients underwent a

What Is the Critical Value of Glenoid Bone Loss at Which Soft Tissue Bankart Repair Does Not Restore Glenohumeral Translation, Restricts Range of Motion, and Leads to Abnormal Humeral Head Position?

BACKGROUND: A general consensus has been formed that glenoid bone loss greater than 20% to 25% is the critical amount at which bony augmentation procedures are needed; however, recent clinical results suggest that the critical levels must be reconsidered to lower values. PURPOSE: This study aimed to find the critical value of anterior glenoid bone loss when a soft tissue repair is not adequate to restore anterior-inferior glenohumeral translation, rotational range of motion, or humeral head position using a biomechanical anterior shoulder instability model. STUDY DESIGN: Controlled laborator

Correlation between range of motion and implant fracture: a 5 year follow-up of 72 joints in 18 patients in a randomized study comparing Swanson and Avanta/Sutter MCP silicone prosthesis.

Eighteen out of 18 rheumatoid patients (at one centre of a two-centre 30 patient study previously reported) with a mean age of 56 years, and 72/72 operated joints were randomized to Avanta/Sutter or Swanson MCP prostheses and followed for 5 years. Both ulnar deviation and extension lag were improved already at 6 weeks and remained improved at 5 years. The Avanta prosthesis had a better range of motion (ROM) than the Swanson. Six of nine patients with Avanta/Sutter implants had at least one implant fracture compared to 1/9 patients with the Swanson implant (P = 0.05) but fracture did not change

Individualizing glenoid lateralization to optimize range of motion after reverse shoulder arthroplasty with the Arthrex Univers reverse: a virtual assessment of the influence of scapular neck length using a statistical shape model.

BACKGROUND: Preoperative scapular neck length (SNL) varies widely due to anatomical factors and pathologic glenoid wear. A short SNL can lead to early impingement, decreased range of motion (ROM), and increased rates of scapular notching after reverse shoulder arthroplasty (rTSA). Glenoid-sided implant lateralization can avoid these issues, but it is unclear how much is necessary for varying SNLs. The objective was to use a statistical shape model, varying SNL, to evaluate impingement-free rotational ROM across different glenoid positions and lateralizations. METHODS: A total of 100 scans wer
