Biallelic Variants in UBA5 Reveal that Disruption of the UFM1 Cascade Can Result in Early-Onset Encephalopathy.

Via whole-exome sequencing, we identified rare autosomal-recessive variants in UBA5 in five children from four unrelated families affected with a similar pattern of severe intellectual deficiency, microcephaly, movement disorders, and/or early-onset intractable epilepsy. UBA5 encodes the E1-activating enzyme of ubiquitin-fold modifier 1 (UFM1), a recently identified ubiquitin-like protein. Biochemical studies of mutant UBA5 proteins and studies in fibroblasts from affected individuals revealed that UBA5 mutations impair the process of ufmylation, resulting in an abnormal endoplasmic reticulum

Periodic eye opening and upward eye deviation accompanied by burst-suppression, as an isolated clinical manifestation of acute post-hypoxic myoclonus.

Various movement disorders have been described following hypoxic-ischaemic brain injury. Here, we present a 72-year-old female patient who developed periodic opening and upward deviation of the eyes as an isolated clinical finding, within 24 hours after cardio-pulmonary arrest. These movements were accompanied by burst-suppression on EEG, and both clinical and electrophysiological findings were suppressed 18 hours after intravenous levetiracetam infusion. The strictly periodic nature of both EEG discharges and eye opening with vertical deviation suggest a cause due to either activation of a su

White Matter Microstructure in Idiopathic Craniocervical Dystonia.

BACKGROUND: Dystonias are hyperkinetic movement disorders characterized by involuntary muscle contractions resulting in abnormal torsional movements and postures. Recent neuroimaging studies in idiopathic craniocervical dystonia (CCD) have uncovered the involvement of multiple areas, including cortical ones. Our goal was to evaluate white matter (WM) microstructure in subjects with CCD using diffusion tensor imaging (DTI) analysis. METHODS: We compared 40 patients with 40 healthy controls. Patients were then divided into subgroups: cervical dystonia, blepharospasm, blepharospasm + oromandibul

Idiopathic bilateral carpal tunnel syndrome in a 9-month-old infant presenting as a pseudo-dystonia.

BACKGROUND: Carpal tunnel syndrome is the most common focal peripheral neuropathy seen in most electrophysiological laboratories. Although the incidence of carpal tunnel syndrome in adults is 50 to 150 cases per 100,000 people, it is rare in children. There are less than 200 case reports of carpal tunnel syndrome in children, with mucopolysaccharides and mucolipidosis being the most frequent cause. Idiopathic carpal tunnel syndrome with childhood onset occurs in less than 0.2% of cases. PATIENT: We describe a 9-month-old infant who presented with intermittent abnormal posturing movement of bo

Error monitoring is altered in musician's dystonia: evidence from ERP-based studies.

Musician's dystonia (MD) is a task-specific movement disorder characterized by a loss of voluntary motor control in highly trained movements like piano playing. Its underlying pathophysiology is defined by deficient functioning of neural pathways at different levels of the central nervous system. However, a few studies have examined the brain responses associated with executive functions such as error monitoring in MD. We recorded the electroencephalogram (EEG) in professional pianists during the performance of memorized music sequences at fast tempi. Event-related potentials (ERPs) locked to

Motor variability--an important issue in occupational life.

Several recent reviews have reported that 'repetitive movements' is a risk factor for occupational musculoskeletal disorders (MSD) in the neck, shoulder and arm regions. More variation in biomechanical exposure is often suggested as an effective intervention in such settings. While increasing variation using extrinsic methods like job rotation may not always be possible in an industrial context, the intrinsic variability of the motor system may offer an alternative opportunity to increase variation. Motor variability (MV) refers to the natural variation in postures, movements and muscle activi

Propriospinal myoclonus: report of a case.

Propriospinal myoclonus is an unusual movement disorder. Here we reported a patient with electrophysiological data. The 32-year-old man had spontaneous, intermittent and brief jerks over the trunk, hip and lower limbs muscles for 4 years. The jerks were not sensitive to external stimuli and usually exacerbated whenever the patient felt relaxed, particularly preceding the onset of sleep. Electromyographic (EMG) recordings showed a long duration in each EMG burst (over 200 milliseconds) with a slow propagating pattern initiating from the rectus abdominis muscles. There was no time-locked cortica

Surgical treatment of blepharoptosis caused by chronic progressive external ophthalmoplegia.

UNLABELLED: Chronic progressive external ophthalmoplegia (CPEO) is a neuromyopathic disorder characterized by progressive weakness of the extraocular and levator muscles, which causes blepharoptosis and impairment of ocular motility. Because of the risk of worsening of lagophthalmos and exposure keratitis due to an associated poor Bell phenomenon or weak orbicularis function, surgical treatment of the blepharoptosis caused by CPEO is problematic. We present our experience with a case of blepharoptosis in CPEO. CASE: A 61-year-old woman presented with slowly progressive bilateral blepharoptosi

A neural network model of Parkinson's disease bradykinesia.

Parkinson's disease (PD) is caused by dopamine (DA) depletion consequent to cell degeneration in the substantia nigra pars compacta (SNc) and the ventral tegmental area (VTA). Although computational analyses of PD have focused on DA depletion in DA-recipient parts of the basal ganglia, there is also extensive DAergic innervation of the frontal and parietal cortex as well as the spinal cord. To understand PD bradykinesia, a comprehensive network model is needed to study how patterns of DA depletion at key cellular sites in the basal ganglia, cortex and spinal cord contribute to disordered neuro

Dyslexia: the articulatory hypothesis revisited.

Articulatory disorders have been associated with developmental phonological dyslexia in the literature. However, very few information is available about the articulatory movements involved in speech production in dyslexic children. This study uses aerodynamic/acoustic data to explore how dyslexic children produce bilabial stops in French (/b, p/) within a sentence where they occurred in two positions and in three vowel environments. Average durations of articulatory closure and release were calculated in 10 phonological dyslexic children and two groups of age-matched and reading age-matched co

Musculoskeletal problems among VDU workers in a Hong Kong bank.

A survey of musculoskeletal problems among visual display unit (VDU) users was carried out in a bank using a self-administered questionnaire. The prevalence of complaints in various body parts were: neck--31.4%, back 30.6%, shoulder--16.5%, hand and wrist--14.9% and arm--6.6%. Frequent users of VDU had significantly more musculoskeletal problems in the neck and shoulder regions than infrequent users. Individual musculoskeltal complaints were associated with various risk factors including personal attributes, working posture, repetitive movements and work station design. Back, neck and shoulder

The effects of education on hand use with industrial workers in repetitive jobs.

Eighteen subjects participated in a preliminary study to determine the effects of two types of educational programs on the hand-use patterns of industrial workers at risk for developing cumulative trauma disorder (CTD). The subjects were divided into three groups: Two groups received different educational programs and the third group served as the control. One program used a handout as the only educational tool; the other used the handout as well as a hands-on demonstration of the concepts in the handout. Pretests and posttests of the frequency of movements identified with CTD of the hand and

Value of otoneurological tests in the staging of multiple sclerosis.

We looked for correlations between otoneurological findings and neurological signs in multiple sclerosis (MS). 41 patients were tested for: spontaneous and caloric nystagmus, horizontal pursuit eye tracking, full-field optokinetic nystagmus and eye movements. The findings were compared with the course of the disease (progressive or remitting), age at onset of MS and disability, in order to test the predictive value of this type of test. The correlation between otoneurological abnormalities and lesion site is very high for brainstem lesions. Subclinical brainstem lesions were detected in 66% of

A primate genesis model of focal dystonia and repetitive strain injury: I. Learning-induced dedifferentiation of the representation of the hand in the primary somatosensory cortex in adult monkeys.

In this study we tested a neuroplasticity/learning origins hypothesis for repetitive strain injuries (RSIs), including occupationally induced focal dystonia. Repetitive movements produced in a specific form and in an appropriate behavioral context cause a degradation of the sensory feedback information controlling fine motor movements, resulting in the "learned" genesis of RSIs. Two adult New World owl monkeys were trained at a behavioral task that required them to maintain an attended grasp on a hand grip that repetitively and rapidly (20 msec) opened and closed over short distances. The monk

Nonprimary dystonias.

Dystonias can be classified as primary or secondary, as dystonia-plus syndromes, and as heredodegenerative dystonias. Their prevalence is difficult to determine. In our experience 80-90% of all dystonias are primary. About 20-30% of those have a genetic background; 10-20% are secondary, with tardive dystonia and dystonia in cerebral palsy being the most common forms. If dystonia in spastic conditions is accepted as secondary dystonia, this is the most common form of all dystonia. In primary dystonias, the dystonic movements are the only symptoms. In secondary dystonias, dystonic movements resu

A model of the basal ganglia in voluntary movement and postural reactions.

A basal ganglia central pattern generator (CPG) is developed and its role in voluntary movements on the ground and postural reactions on a disturbed platform are studied and analysed by simulation. Biped dynamics and platform kinematics are utilised. The effects of agonist-antagonist muscular co-activation and joint stiffness are formulated. The implementation of the necessary counter-manoeuvres for maintaining balance and postural stability is studied. A control strategy, applicable to large systems, is formulated. The biped manoeuvres and transitions terminate in pre-specified intervals of t

Word length and word frequency affect eye movements in dyslexic children reading in a regular (German) orthography.

We combined independently the word length and word frequency to examine if the difficulty of reading material affects eye movements in readers of German, which has high orthographic regularity, comparing the outcome with previous findings available in other languages. Sixteen carefully selected German-speaking dyslexic children (mean age, 9.5 years) and 16 age-matched controls read aloud four lists, each comprising ten unrelated words. The lists varied orthogonally in word length and word frequency: high-frequency, short; high-frequency, long; low-frequency, short; low-frequency, long. Eye mov

A computational method for recording and analysis of mandibular movements.

This study proposed the development of a new clinical tool capable of quantifying the movements of opening-closing, protrusion and laterotrusion of the mandible. These movements are important for the clinical evaluation of the temporomandibular function and muscles involved in mastication. Unlike current commercial systems, the proposed system employs a low-cost video camera and a computer program that is used for reconstructing the trajectory of a reflective marker that is fixed on the mandible. In order to illustrate the clinical application of this tool, a clinical experiment consisting on
