Isokinetic foot dorsiflexion strength and muscle biopsy data from eight patients with overuse of tibialis anterior muscle (TA) fibres due to weakness after prior polio or L5 root lesion were compared to data from age and sex matched, healthy subjects. Concentric peak torque at an angular velocity of 30 degrees/s was 6-24 Nm in the patients and 15-34 Nm in the controls (p < 0.01). Muscle biopsies from TA showed a significantly higher proportion of type 1 fibres in the patients as compared to the controls (p < 0.005). The type 1 fibres in the patients had a significantly larger cross-sectional a

UNLABELLED: Polymyositis with peripheral nerve lesion are rare. The study of eight cases of polymyositis with some peripheral lesion, pathologic investigation of muscle biopsy of all patients and biopsy of sural nerve in six cases were reported. Age: 19-52 (average 34) yrs.
COURSE: 4mo-10 (average 4) yrs. They showed symmetric weakness and tenderness of the proximal muscles, peripheral hypoesthesia and hypo even areflexia. Electrophysiological parameters showed myogenic lesion in 8 cases and neurogenic in 3 cases. Pathological (light and electron-microscopic) findings coincide with acute and

The muscles of four infants with cerebro-hepato-renal (Zellweger) syndrome were studied during life and/or at necropsy. A mitochondrial myopathy was demonstrated, similar to mitochondrial alterations demonstrated in liver and brain in this disease. Muscle fibers with red-staining subsarcolemmal aggregates were identified with Gomori trichrome stain in two cases. Subsarcolemmal and intermyofibrillar zones of increased concentrations of NADH-TR, SDH, and cytochrome-c-oxidase activity were demonstrated histochemically in all four cases. Degenerative and cytoarchitectural changes in muscle fibers

Glycogen storage disease due to amylo-1,6-glucosidase deficiency was diagnosed in a 21-year-old patient. The enzyme defect was demonstrated by biochemical analysis of muscle tissue, the glycogen content of which was typically increased. Investigation of the patient's kindred showed that his 25-year-old sister was also affected. This report sets out to show that in adolescence and in adult life myopathy may be the leading symptom of the disease. Besides the clinical symptoms of muscle weakness and stiffness, an increase in serum creatine kinase usually is found. While an increase in the glycoge

INTRODUCTION: Childhood onset myasthenia gravis associated with anti-muscle-specific tyrosine kinase antibodies is very rare and atypical in presentation.
CASE REPORT: As a baby, the pre- sented patient was choking and sleeping with open eyes. She had weak cry and breathing difficulties. In childhood, there were fre- quent falls and fluctuating swallowing difficulties. At the age of 19 she was misdiagnosed with Miller Fisher syndrome due to the presence of diplopia, ataxia and hyporeflexia with spontaneous recovery. Repetitive nerve stimulation test was normal. Four years later, after several

Cricopharyngeal achalasia is a rare cause of dysphagia in the dog. However it must be differentiated from other causes of dysphagia as it is treatable with surgery. It is a disruption of the cricopharyngeal phase of the oropharyngeal phase of deglutition. There appears to be an incoordination in the swallowing process between the relaxation of the rostral, middle pharyngeal muscles and the caudal pharyngeal muscles. It is seen as a primary condition in young animals presenting soon after weaning onto solid food. The dogs appear clinically healthy unless there is secondary aspiration pneumonia

Rhabdomyolysis refers to a number of clinical and biochemical symptoms, which result from the destruction of skeletal muscles. The following triad of symptoms is considered typical: myalgia, muscle weakness, and dark urine. The most common reasons for rhabdomyolysis in children are infections. It has also been reported that rhabdomyolysis may be caused by chemotherapy drugs. The most difficult complication of rhabdomyolysis is renal failure. The authors present a 17-year-old boy diagnosed with Ewing sarcoma and a 16-year-old boy suffering from acute leukemia, both with rhabdomyolysis developed

The incidence of obstetric brachial palsy is high and their sequelaes are frequent. Physiotherapy, microsurgical nerve reconstruction and secondary corrections are used together to improve the shoulder function. The most common posture is shoulder in internal rotation and adduction, because of the antagonist weakness. The muscle forces imbalance over the osteoarticular system, will result in a progressive glenohumeral joint deformity which can be recognized with a magnetic resonance image. Tendon transfers of the internal rotators towards the external abductor/rotator muscles, has good results

Older people are at risk of constipation because of age-related changes that may weaken intestinal muscles, decrease peristalsis and reduce physical activity. Primary causes of constipation include decrease in fluid intake, decrease in ingestion of high fibre foods which may be associated with reduced ability to chew food, and weak abdominal muscles reducing effective evacuation. Secondary causes include disease processes such as dementia, Parkinson's, hypothryroidism, diverticular disease and prescribed medications, including narcotics, calcium channel blockers, anticholinergics and antidepre

Lumbosacral plexitis (LSP) is an uncommon idiopathic disorder characterized by the acute onset of severe lower extremity pain followed by wasting and weakness of leg muscles with variable sensory loss. Muscles innervated by multiple roots and multiple nerves are affected, and other causes of lumbosacral plexopathy such as mass lesions and trauma must be excluded. Postulated causes of LSP include vasculitis with subsequent axonal injury and autoimmune-related demyelination, although evidence is accumulating that the former pathophysiology is more likely. Recovery is variable but is often quite

Hemifacial spasm (HFS) is characterized by tonic and clonic contractions of the muscles innervated by the ipsilateral facial nerve. It is important to distinguish this from other causes of facial spasms, such as psychogenic facial spasm, facial tic, facial myokymia, blepharospasm, and tardive dyskinesia. Magnetic resonance imaging and angiography studies frequently demonstrate vascular compression of the root exit zone of the facial nerve. Importantly, an underlying space-occupying lesion needs to be excluded in patients with associated atypical features such as facial numbness and weakness. B

Antibody to tubulin in man has been studied using a specific radioimmunoassay, affinity chromatography radioimmunoassay but markedly increased levels were noted in patients with infectious mononucleosis where the antibody was predominantly IgM in type. This finding was confirmed on fluorescence microscopy. Affinity chromatography purified antibody produced characteristic microtubular staining of fixed 3T3 cells, but in addition, produced weak staining of cryostat sections of rat tissue, similar in distribution to that of smooth muscle antibody. Our studies indicate that the IgM smooth muscle a

2 cases of back curvature considered to be due to myopathy, but without associated limb weakness, are described. 1 case presented as infantile kyphosis with subsequent progression to scoliosis. There were some associated myopathic clinical features, including recurrent herniae and hip and knee contractures. The second case presented as an otherwise classical instance of Scheurmann's disease. Paraspinal muscle in both cases showed nonspecific abnormalities consistent with varieties of minimal change myopathy. EMG examination of limb muscles revealed myopathic abnormalities. These cases support

Six patients with a prolonged history of severe muscular asthenopia caused by persistent convergence insufficiency underwent resection of both medial rectus muscles. A temporary consecutive esotrapia occurred in each instance and was controlled with prisms. Five weeks after surgery the consecutive esotropia had disappeared in all but one patient who still required prismatic correction for distance fixation. All patients experienced relief from their symptoms. Whereas orthoptic training is useful in most cases of convergence insufficiency a resection of both medial rectus muscles should be cons

Surgical procedures for exposure of the upper third of the fibula have been known to cause weakness of the long extensor of the big toe post-operatively. The authors present three representative cases of surgically induced dropped big toe. From cadaveric dissection, an anatomic basis was found for this phenomenon. The tibialis anterior and extensor digitorum longus muscles have their origin at the proximal end of the leg and receive their first motor innervation from a branch that arises from the common peroneal or deep peroneal nerve at about the level of the neck of the fibula. However, the

BACKGROUND: Neurovascular free muscle transfer is currently the mainstay for smile reconstruction. However, problems such as excessive muscle bulk and dislocation of the transferred muscle attachment have been described. Furthermore, dynamic movements of the transferred muscle are sometimes too strong or too weak, resulting in facial asymmetry. In these cases, secondary revisional operations for the transferred muscle are required after neurovascular free muscle transfer. This report describes revisional operative procedures in detail and examines the extent of improvement of the smile by comp

Thirty self-ambulatory children with mid-lumbar to low-sacral myelomeningocele who walked without aids and 21 control children were evaluated by three-dimensional gait analysis. Characteristic kinematic patterns and parameters in the trunk, pelvis, hip, knee and ankle were analyzed with respect to groups with successive weakness of the ankle plantarflexor, ankle dorsiflexor, hip abductor, hip extensor and knee flexor muscles. Extensive weakness of the plantarflexors resulted in kinematic alterations in the trunk, pelvis, hip and knee and in all three planes seen as knee flexion, anterior pelvi

Myasthenia gravis is an autoimmune disease that affects the neuromuscular junction. It causes generalized muscle weakness that may include the respiratory muscles, potentially leading to a medical emergency known as a myasthenic crisis. This is a case report of a rare incident of a myasthenia gravis exacerbation after administration of the oral antipsychotic risperidone to a schizophrenic patient. Few similar cases have been reported. Although rare, such incidents are dangerous as physicians could easily confuse myasthenia gravis symptoms with extrapyramidal side effects of antipsychotics. Thi

The chest is a dynamic structure. For normal movement it relies on a coordinated movement of the multiple bones, joints and muscles of the respiratory system. While muscle weakness can have clear impact on respiration by decreasing respiratory motion, so can conditions that cause chest wall hypoplasia and produce an immobile chest wall. These conditions, such as Jarcho-Levin and Jeune syndrome, present significantly different challenges than those faced with early onset scoliosis in which chest wall mechanics and thoracic volume may be much closer to normal. Because of this difference more agg

Patients with hepatocellular carcinoma may manifest paraneoplastic syndromes such as hypercholesterolaemia, hypoglycaemia, hypercalcaemia and erythrocytosis. An exceptional case of hepatocellular carcinoma associated with polymyositis, a rheumatic paraneoplastic syndrome, is reported. A 72-year-old male complained for a recent muscular weakness mainly in the proximal limb muscles. The clinical course, a raised level of serum creatine kinase and electromyographic findings suggested polymyositis, and the pathological findings on muscle biopsy were compatible with this diagnosis. Computed tomogra
