Different patterns of nicotinic acetylcholine receptor subunit transcription in human thymus.

Clinical observations suggest that the thymus is strongly implicated in the pathogenesis of myasthenia gravis (MG), but questions such as the level and location of nicotinic acetylcholine receptor (AChR) subunit expression that are fundamental to postulate any pathogenic mechanism, remain controversial. We have re-examined this question by combining calibrated RT-PCR and real-time PCR to study nicotinic AChR subunit mRNA expression in a panel of normal and myasthenic thymi. The results suggest that the expression of the different AChR subunits follows three distinct patterns: constitutive for,…

Autoantibodies to ion channels at the neuromuscular junction.

Autoantibodies directed against voltage- or ligand-gated ion channels and their associated proteins at the neuromuscular junction give rise to a family of neurological autoimmune diseases. Antibodies to acetylcholine receptors or muscle-specific kinase present on the postsynaptic muscle membrane are associated with different forms of myasthenia gravis (MG). Antibodies to the presynaptic voltage-gated potassium and calcium channels are responsible for acquired neuromyotonia and Lambert-Eaton myasthenic syndrome (LEMS), respectively. The patients respond to immunotherapies and their plasma can t…

Immunology of paraneoplastic syndromes: overview.

Cancers that cause disturbances of organs or tissues remote from the site of the tumor or its metastases are called paraneoplastic syndromes. The nervous system can be affected at virtually any site, including the neuromuscular junction (e.g., Lambert-Eaton myasthenic syndrome, myasthenia gravis). Paraneoplastic syndromes affecting the central nervous system are characterized by (1) high titers of antibodies that react with both the cancer and the affected portion of the nervous system, (2) specifically reacting T cells in the blood and cerebrospinal fluid, and (3) autopsy evidence of neuronal…

Reactive cutaneous cytophagocytosis in nocardiosis.

Cutaneous nocardiosis, which usually manifests in the form of pustules, abscesses, or subcutaneous nodules, is occasionally found in immunocompromised patients. A 59-yr-old Korean man with myasthenia gravis and thymoma developed nodular skin lesions on his trunk. Histopathologically, abscess formation with a dense infiltrate of neutrophils and many cytophagic histiocytes were observed. Numerous filamentous organisms, which turned out to be Nocardia asteroides by culture, were also found. After sulfamethoxazole-trimethoprim therapy, all of the skin lesions rapidly decreased in size, with a mark…

[Analysis of the influence factors on the resection and prognosis of thymic tumors].

One hundred eleven thymic tumors were intervened surgically, including 92 thymoma, 8 thymic carcinoid, and 10 thymic carcinoma. In this series 50.9% of the cases were complicated with various syndrome, 44.5% with myasthenia gravis (MG), more frequently in the epithelial cell type. Pure red cell aplasia often associated with the spindle-cell thymoma. Of the thymic carcinoid 50% had ectopic ACTH syndrome. Resection rate correlated with the size and invasion of the tumor. There were significant difference in resection rate among thymoma, thymic carcinoid, and thymic carcinoma. The degree of invas…

[Effect of heredity and environment in immune diseases. Presentation of twin data].

BACKGROUND: The study of twins is an important and informative tool in the investigation of the influence of genetic and environmental factors on the pathogenesis of familial traits. MATERIAL: Seven diseases were analysed by carrying out an intensive study of the literature to search for concordance of monozygotic and dizygotic twins. Apart from single case studies, large unselected series have been reported, some of which show considerable differences in concordance rates. Data from twins were collected for myasthenia gravis, systemic lupus erythematosus, rheumatoid arthritis and type 1 diab…

A case of multiple thymoma: the possibility of intra-thymic metastasis.

A case of multiple thymoma is reported. The patient was a 28-year-old woman who came to us with myasthenia gravis. There were two separate thymoma in the anterior mediastinum: one measured 70 x 30 x 30 mm, while the other was 1.5 mm in diameter and situated in the surrounding thymic tissue. A histological study revealed that both thymoma consisted of epithelial cells with abundant cytoplasm, round-to-oval nuclei and inconspicuous nucleoli. Morphometric analysis showed the nuclear size of the epithelial cells to be similar in both tumors. The likelihood of positive Leu7 and keratin immunohistoc…

Multiple interferons in the circulation of patients with systemic lupus erythematosus and vasculitis.

Recently, we found interferon in the sera of patients with systemic lupus erythematosus, rheumatoid arthritis, scleroderma, and Sjögren's syndrome. In this study, we surveyed a variety of other immunologically mediated diseases. We did not find interferon in the sera of patients with Wegener's granulomatosis, sarcoidosis, infectious mononucleosis, minimal change nephritis, kidney transplants, myasthenia gravis, or uveitis, but we did find this protein in the sera of patients with active systemic and cutaneous vasculitis. Attempts to characterize the interferon in the sera of patients with syst…

[Pemphigus and thymoma: review and etiopathogenesis (author's transl)].

The association of pemphigus and thymoma has been investigated in a recent personally observed case and data from nineteen further cases published in the literature have been reviewed. From these twenty cases several prominent features emerge. Clinically, pemphigus of Senear-Usher type (pemphigus erythematosus) is most commonly found (11/20 cases). There is a male preponderance (at least 13/20 cases) although associated myasthenia gravis which occurred in thirteen cases was more common in females. Thymoma probably preceded pemphigus in all cases, although this was certain in only nine. Evidenc…

Clinical pharmacokinetics of cholinesterase inhibitors.

This review deals mainly with the pharmacokinetics of the reversible quaternary cholinesterase inhibitors neostigmine, pyridostigmine and edrophonium, which are mainly used to antagonise non-depolarising neuromuscular blockade in general anaesthesia and in the symptomatic treatment of myasthenia gravis. Only in the last few years, since the introduction of highly sensitive and selective analytical procedures based on gas and liquid chromatography, have proper pharmacokinetic studies of these drugs become possible. Rapid cooling and addition of internal standard to samples before freezing are i…

Mitochondrial Myopathy in the Oculoplastic Practice.

Ptosis is a common ocular complaint in the elderly population. While most cases are often due to involutional changes in the eyelid, neurogenic, mechanical or myogenic diseases should remain as important differential diagnoses. We report a case of a 75-year-old Chinese man who presented with chronic progressive ptosis and was initially managed as presumed seronegative ocular myasthenia gravis (MG). Despite positive bedside findings suggestive of MG, serological and electrophysiological investigations were inconclusive, and there was no clinical response to pyridostigmine. The diagnosis was ach…

A Clinical Case of Thymoma Presenting With Pure Erythroid Aplasia.

Thymic tumors are a rare condition that affects both genders equally, typically presenting between the ages of 40 and 70. Although often asymptomatic, they can give rise to paraneoplastic syndromes, with myasthenia gravis being the most common. The presentation of pure red cell aplasia, however, remains an uncommon manifestation. Thymectomy is the primary treatment, but it may not always be sufficient for managing associated syndromes, and immunosuppressive therapy may be necessary. This case involves a 50-year-old, otherwise healthy man, who presented to the emergency department with progress…

Selective Inhibition of Rat α7 Nicotinic Acetylcholine Receptors by LvID, a Newly Characterized α4/7-Conotoxin from Conus lividus.

The α7 nicotinic acetylcholine receptors (nAChRs), identified in peripheral and central nervous systems, are crucial for cognitive function, memory, inflammation, and are linked to disorders like Alzheimer's disease (AD), lung cancer, myasthenia gravis, and atherosclerosis. Here we report that a novel α4/7-conotoxin (CTx) LvID, from Conus lividus, potently inhibits rat α7 nAChRs expressed in Xenopus oocytes with an IC50 of 13.8 nM, showing little activity against other rat nAChR subtypes. The structure of LvID was elucidated using nuclear magnetic resonance (NMR) spectroscopy and comprises a s…

Sepsis complicated by haemophagocytic lymphohistiocytosis triggered by methicillin-resistant Staphylococcus aureus and human herpesvirus 8 in an immunocompromised elderly patient: A case report.

Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. A rare but severe complication is haemophagocytic lymphohistiocytosis (HLH), an aggressive syndrome of excessive immune activation. Managing both conditions is particularly challenging in patients with multiple comorbidities. A 78-year-old male with a complex history, including myasthenia gravis and chronic kidney disease, was admitted with sepsis. Investigations confirmed infection with methicillin-resistant Staphylococcus aureus and human herpesvirus 8. He was subsequently diagnosed with HLH b…
