[Mediastinal actinomycosis with formation of an esophagotracheal fistula. A case report].

Before antibiotics were available, actinomycosis was the most commonly diagnosed "fungal disease" of the lung because of its morphological similarity to true fungi. At that time actinomycosis presented a fairly typical clinical picture of empyema thoracis and sinus tracts in the chest wall. Nowadays it has become a rare infectious disease that is usually caused by the bacterium Actinomyces israelii and is amenable to treatment by most antibiotics available today. The following report describes the case of a 59-year-old man with an uncommon mediastinal actinomycosis that caused an oesophagotrac

Nutrition and current concepts of obesity.

Obesity is and has been one of the foremost health problems in the U.S. and in many other countries. It is so interrelated with different factors that it is very difficult to single out the effect of one particular aspect. However, an effort is made in this review to focus on obesity only from a nutritional point of view. Childhood nutrition and its impact on obesity is particularly emphasised since weight gain during early infancy has been associated with obesity in later stages of life. There is considerable evidence that links obesity with certain diseases of the cardiovascular system and d

Hypoinsulinemic hypoglycemia with hemihypertrophy in a Filipino infant: management in a resource-limited setting.

Persistent infantile hypoglycemia is a medical emergency associated with significant risk of neurological injury. Although most commonly attributed to congenital hyperinsulinism, disorders of impaired insulin signaling with hypoketotic hypoglycemia despite low or undetectable insulin occur in fewer than 1 in 1 000 000 infants worldwide, leading to misdiagnosis and ineffective therapy. We report a Filipino infant with recurrent fasting hypoglycemic seizures and hemihypertrophy. Biochemical evaluation showed absent ketones with low to mildly detectable insulin, suggesting dysregulated insulin si

Upregulation of ADAM10 reinforces CD8+ T cells toward exhaustion in the TME.

Using cytotoxic CD8+ T cell-mediated cellular immunity has shown considerable efficacy in cancer treatment. However, the effectiveness of these cells against solid tumors is limited by the presence of an immunosuppressive tumor microenvironment (TME), which constitutes complex networks of regulatory pathways and resistance mechanisms. Given its critical role in TME deterioration through shedding of immunosuppressive molecules from tumor and immune cell surfaces, the upregulation of A Disintegrin and Metalloproteinase 10 (ADAM10) expression can reinforce CD8+ T cells into a state of exhaustion.

Contributors to the heterogeneity of response to CFTR modulators.

Cystic fibrosis transmembrane conductance regulator (CFTR) modulators have transformed the management of cystic fibrosis; however, substantial interindividual variability in clinical response persists, even among people with identical CFTR genotypes. This variability challenges clinicians, regulators, and researchers in defining treatment success and optimising long-term outcomes. We synthesise evidence from clinical trials, real-world cohorts, and mechanistic studies to examine the contributors to heterogeneity in response to CFTR modulators, with a focus on triple therapy (elexacaftor-tezaca

Physical activity delays ovarian aging in part through adiponectin-related signaling pathways.

Ovarian aging leads to infertility, endocrine dysregulation and increased chronic disease risk, yet there are currently no approved therapies to delay ovarian aging. Physical activity (PA) is a crucial lifestyle factor for human health and antiaging, but its effects and mechanisms on ovarian aging remain unclear. Here we show, through a cross-sectional analysis of 152,435 participants in the UK Biobank, that lower PA levels were observed among postmenopausal persons when compared to premenopausal persons, and this finding was supported by another cross-sectional study of 12,418 participants in

Case Report: A rare but critical complication in patients with lumbar infection combined with cauda equina syndrome-sacrococcygeal pressure sores.

This report presents a complex case of a 65-year-old male patient with a lumbar spine infection (T12-L1) caused by Staphylococcus aureus, leading to cauda equina syndrome, and subsequently developing a massive infectious pressure sore in the sacrococcygeal region. Back pain due to spinal infections, analgesic use, neurological deficits, and systemic infections masked the early signs of the pressure sore. Prolonged immobilization and neurogenic bladder and bowel incontinence led to a 5 cm × 5 cm deep pressure sore in the sacrococcygeal area. Involving a single-stage surgery for debriding the sp

A Case of Asymptomatic Lead Poisoning in a Two-Year-Old With a Background of Pica and Iron Deficiency Anaemia.

Lead poisoning remains an important yet under-recognised cause of morbidity in children. We report the case of a two-year-old child presenting with asymptomatic lead poisoning identified following investigation of pica, including ingestion of wall paint. This was with concomitant iron deficiency anaemia secondary to excessive cow's milk intake. Given these significant risk factors, blood lead level screening was done despite her asymptomatic presentation, revealing a markedly elevated lead level of 3.34 µmol/L (reference range ≤ 0.10 µmol/L). Following these results, she was admitted for treat

Psoas metastasis of urothelial carcinoma: A rare case.

INTRODUCTION: Urothelial carcinoma (UC) is a common malignancy with a high propensity for metastasis, typically to the lungs, liver, bones, and lymph nodes. Metastasis to the psoas muscle is exceedingly rare and poorly documented. CASE PRESENTATION: We report a 46-year-old chronic smoker with a history of muscle-invasive bladder UC treated with cystectomy and urinary diversion. After two years of lost follow-up, he presented with hematuria, lumbar pain, and a large left psoas tumor confirmed as recurrent UC. Surgical resection of the psoas and excretory tract tumor was performed, coupled with

Comment on "Pancreatic Enzyme Replacement Therapy in Pancreatic Exocrine Insufficiency-Real-World's Dosing and Effectiveness: A Systematic Review".

Pancreatic enzyme replacement therapy (PERT) is vital for managing pancreatic exocrine insufficiency (PEI), yet real-world use often deviates from guideline-recommended dosing. In response to the recent review by Kadaj-Lipka et al., we highlight several overlooked but critical factors influencing PERT effectiveness. While the review identifies underdosing trends, it does not address patient adherence or the timing of enzyme intake, both essential for therapeutic success. Clinical applicability was further limited by the inconsistent measurement of important nutritional outcomes like weight gai

Efficacy of a Once-Daily Supplement in Managing Canine Chronic Kidney Disease.

BACKGROUND: Chronic kidney disease (CKD) in companion animals leads to progressive renal deterioration and metabolic complications such as hyperphosphatemia and metabolic acidosis, particularly in advanced stages. METHODS: This 90-day, double-blind, randomized controlled study evaluated the effects of a renal supplement (Renal Combi, Candioli srl, Beinasco, Turin, Italy) in 30 dogs diagnosed with CKD. Dogs were randomly assigned to a treatment group (TRT, n = 15), receiving a commercial renal diet plus the supplement (once daily), or a control group (CTR, n = 15), receiving the same diet and

Retrospective Study of Trigger Point Injections for Myofascial Pain Syndrome in Cancer Patients: Impact on Pain Relief and Opioid Tapering.

Although the pathogenesis and management of Myofascial Pain Syndrome (MPS) have been discussed in the literature, only a few studies have examined the effects of trigger point injection (TPI) on MPS or opioid dosage in patients with cancer. We evaluated changes in numeric rating scale (NRS) scores and opioid dosage, along with the duration of opioid use, nutritional status, and the types of cancer and metastases in patients with cancer and MPS who received TPI, to evaluate its clinical significance. Data were collected retrospectively from 20 inpatients with cancer and MPS who received TPI for

Systemic Consequences of Inflammatory Bowel Disease Beyond Immune-Mediated Manifestations.

Inflammatory bowel disease (IBD) management traditionally focuses on intestinal inflammation, yet extraintestinal manifestations can substantially impair patient quality of life. In this perspective, we emphasize the broad systemic impact of IBD-from highly prevalent conditions such as anemia, metabolic dysfunction-associated steatotic liver disease, or fatigue to rare but severe complications like interstitial lung disease and drug-induced glomerulonephritis. We review underlying mechanisms linking gut inflammation to distant organs, including immune dysregulation, microbial translocation, an

Trimethylamine-N-oxide formation, the bacterial taxa involved and intervention strategies to reduce its concentration in the human body.

INTRODUCTION: This study reviews the different steps involved in trimethylamine-N-oxide (TMAO) formation, a gut microbiota (GM)-derived compound that promotes cardiovascular and chronic kidney disease. The formation of TMAO is a metaorganismal process, where trimethylamine (TMA), produced from the dietary precursors betaine, L-carnitine and choline by various members of GM, is absorbed and subsequently oxidized by hepatic flavin-containing monooxygenases before entering the circulation. RESULTS AND DISCUSSION: We provide an updated database on members of GM exhibiting different biochemical pa

The Role of Mitochondrial Complexes in Liver Diseases.

Mitochondrial respiratory complexes (Complexes I-V) and their assembly into respiratory supercomplexes (SCs) are fundamental to liver bioenergetics, redox homeostasis, and metabolic adaptability. Disruption of these systems contributes to major liver diseases, including non-alcoholic fatty liver disease, alcoholic liver disease, drug-induced liver injury, viral hepatitis, and hepatocellular carcinoma, by impairing adenosine triphosphate synthesis, increasing oxidative stress, and altering metabolic pathways. Recent advances have clarified the structural-functional interdependence of individual

Role of diet, physical activity and new drugs in the primary management of cancer cachexia in gastrointestinal tumors - a comprehensive review.

Cancer cachexia is a multifactorial syndrome characterized by involuntary weight loss, muscle mass reduction, and systemic inflammatory response, negatively impacting the quality of life and survival of cancer patients. In gastrointestinal tumors, cachexia prevalence can reach up to 80%, presenting a complex clinical challenge. This review aims to explore the efficacy of a multimodal approach integrating nutritional support and physical activity in cancer cachexia management. Targeted dietary strategies, such as a high-protein diet enriched with omega-3 and branched-chain amino acids (BCAAs),

Comprehensive Insights into Sarcopenia in Dialysis Patients: Mechanisms, Assessment, and Therapeutic Approaches.

Sarcopenia, defined as the progressive loss of muscle mass, strength, and function, is largely prevalent but still clinically underrecognized among patients undergoing chronic dialysis therapy. The pathogenesis involves a complex interplay of chronic inflammation, oxidative stress, metabolic acidosis, hormonal imbalances, protein waste, malnutrition, and reduced physical activity. This multifactorial condition profoundly impairs quality of life and may lead to significant clinical consequences, including frailty, an increased risk of falls and hospitalization, and elevated mortality. Despite i
