[Nutritional and surgical management of short bowel syndrome. Our last 6 patients' experience].

BACKGROUND: Children cause of intestinal failure is short-bowel syndrome. It provokes an altered absorption of nutrients and makes patients to be dependent on parenteral nutrition (PN) while they wait or not for an intestinal transplantation, with its side effects. It is crucial to achieve the maximum efficiency of remaining intestine. Many surgical techniques have been led to reduce stasis of dilated small intestine and improve the mucosal surface area for absorption. METHODS: Six patients have presented intestinal failure because of a surgical resection during newborn period. 2 gastroschisi

Nutrition and oral health in Africa.

Poverty, low birth weight, low life expectation at birth, widespread malnutrition, numerous endemic infections, little or no access to safe water, poor oral hygiene, deplorable environmental sanitation and political instability among other problems, characterise the lives of many Africans, particularly in sub-Saharan Africa. In African countries undergoing rapid urbanisation, health problems associated with undernutrition and overnutrition coexist, and these result from lifestyle changes which promote physical inactivity, increased consumption of fats and refined carbohydrates, as well as abus

Iodine supplementation of pregnant women in Europe: a review and recommendations.

OBJECTIVE: Nearly two-thirds of the population of Western and Central Europe live in countries that are iodine deficient. Damage to reproductive function and to the development of the fetus and newborn is the most important consequence of iodine deficiency. The objective of this review was to examine the iodine status of pregnant women in Europe and the potential need for iodine supplementation. DESIGN: A MEDLINE/PubMed search and compilation of all published studies since 1990 of iodine nutrition and iodine supplementation of pregnant women in Europe, as well as an Internet-based search and

Selenium in global food systems.

Food systems need to produce enough of the essential trace element Se to provide regular adult intakes of at least 40 microg/d to support the maximal expression of the Se enzymes, and perhaps as much as 300 microg/d to reduce risks of cancer. Deprivation of Se is associated with impairments in antioxidant protection, redox regulation and energy production as consequences of suboptimal expression of one or more of the Se-containing enzymes. These impairments may not cause deficiency signs in the classical sense, but instead contribute to health problems caused by physiological and environmental

Efficacy and safety of radiologically placed gastrostomy tubes in paediatric haematology/oncology patients.

BACKGROUND: The treatment of malnutrition, which is of great concern in paediatric haematology/oncology patients, is fraught with problems. The goals of our study were to document the complications and assess the weight gain with gastrostomy tubes (G-tubes) in this population. PROCEDURE: Patient data were acquired by retrospective review of all haematology, oncology, and bone marrow transplant (BMT) patients (n = 44) who received radiologically placed G-tubes at our institution over a 4-year period. RESULTS: Forty-four G-tubes were placed (59% peri-BMT). At tube placement, 55% of patients we

Vitamin A status of wild mallards (Anas platyrhynchos) wintering in Saskatchewan.

Vitamin A status of wild male mallards (Anas platyrhynchos) overwintering in Saskatchewan, Canada was determined. Vitamin A levels < 0.2 micrograms hepatic retinyl palmitate/g liver, occurred in 6% and 25% of male mallards sampled in 1991 to 1992 and 1992 to 1993, respectively. There was no temporal trend in vitamin A levels over either winter. Squamous metaplastic lesions, commonly associated with vitamin A deficiency in domestic animals, were not observed in any bird; hence, they were not a good indicator of vitamin A status in wild mallards. Serum retinol was not a good indicator of vitamin

Ascariasis and childhood malnutrition.

The present review will examine epidemiological perspectives and be confined mainly to the results of those field studies published since 1975 in order to provide concrete scientific evidence of the effect of ascariasis on childhood malnutrition, particularly on growth. The field studies were done in many developing countries from Africa, Asia and Latin America, using cross-sectional and intervention studies in which anthelmintics were employed, with different dosing frequency and follow-up periods ranging from 33 days to 2 years. In general, a better nutritional status in terms of growth, lac

Prevalence of protein calorie malnutrition in general surgical patients.

The prevalence of protein calorie malnutrition (PCM) based on ten nutritional parameters was studied in 307 patients undergoing major elective surgical operations. These parameters included anthropometric measurements (weight/height, triceps skin fold thickness, arm muscle circumference) and biochemical (serum total proteins, albumin, transferrin, prealbumin, retinol binding protein) and immunological tests (total lymphocyte count and delayed hypersensitivity test). Using these criteria, the prevalence of PCM was high. Eighty-six percent of patients had at least one abnormal parameter. The pre

Case-control study of Cryptosporidium parvum infection in Peruvian children hospitalized for diarrhea: possible association with malnutrition and nosocomial infection.

A retrospective, hospital-based case-control study was used to investigate whether there were any clinical characteristics that could distinguish Cryptosporidium parvum-infected children with diarrhea from other non-C. parvum-infected children with diarrhea. Ten percent (24 of 248) of children admitted to a rehydration ward at Cayetano Heredia University Hospital, Lima, Peru, were infected with C. parvum. The 24 patients infected with C. parvum (cases) were matched to an equal number of noninfected patients (controls). C. parvum-infected patients were more likely to be malnourished than were c

Use of a liquid diet as the sole source of nutrition in six dysphagic horses and as a dietary supplement in seven hypophagic horses.

Six horses with dysphagia (attributable to botulism, glossitis, or guttural pouch mycosis) were given a commercially available liquid diet as the sole source of nutrition. Seven horses with hypophagia caused by severe bacterial pleuropneumonia or peritonitis were given the liquid diet to supplement food consumed voluntarily. The liquid diet was administered through a nasogastric tube 2 or 3 times daily. Body weight did not change significantly, and pertinent laboratory values remained at satisfactory concentrations throughout the feeding period. Serious complications were not encountered. Thre

[Comparison between oral nutrition and enteral nutrition using a percutaneous endoscopically guided gastrostomy (PEG) in patients undergoing radiotherapy for head and neck tumors].

The nutritional state of 99 patients with tumors in the head and neck area was determined prior to, during and after radiotherapy by anthropometric, laboratory-chemical and immunological parameters. In 73 orally nourished patients, the nutritional parameters deteriorated very quickly during radiotherapy and recovered slowly and incompletely after radiotherapy. In 26 patients treated prospectively by percutaneous endoscopically guided gastrostomy (PEG), however, an improved nutritional state was achieved already during the irradiation in spite of a less favorable starting point. The subjective

Value of nutritional support in adult cancer patients.

Both the presence of cancer and oncologic therapy cause metabolic alterations that may decrease the ability of the host to maintain anabolism. Nutritional support, properly administered, will replenish lean body mass, visceral proteins, and immunocompetence in human beings and experimental animals with small to modest tumor burdens. To date, stimulation of tumor growth by intravenous hyperalimentation in malnourished patients has not been documented scientifically. The exact role of intravenous hyperalimentation, in combination with chemotherapy or radiation therapy, is controversial, whereas

Maternal Exercise and Nutrition Function as Interconnected Regulators of Embryonic Organogenesis and Fetal Development.

The intrauterine environment plays a critical role in shaping lifelong health, forming the foundation of the Developmental Origins of Health and Disease (DOHaD) paradigm. Increasing evidence suggests that maternal lifestyle factors, particularly physical activity and nutrition, can actively influence embryonic development; however, how exercise and dietary signals interact to regulate developmental programming remains insufficiently understood. In this review, we summarize emerging evidence that maternal exercise and nutrition function as interconnected regulators of embryonic organogenesis th

Position of the American Dietetic Association: nutrition intervention in the treatment of eating disorders.

It is the position of the American Dietetic Association that nutrition intervention, including nutritional counseling by a registered dietitian (RD), is an essential component of team treatment of patients with anorexia nervosa, bulimia nervosa, and other eating disorders (EDs) during assessment and treatment across the continuum of care. Diagnostic criteria for EDs provide important guidelines for identification and treatment. In addition, individuals may experience disordered eating that extends along a range from food restriction to partial conditions to diagnosed EDs. Understanding the rol

Nutrition support for lung transplant patients.

Lung transplantation has emerged as an established and accepted therapy for patients with endstage pulmonary disease. Very little information has been published about the nutrition management of these patients during the period after transplantation. We conducted a retrospective review of the nutrition support records of 52 adult patients who had undergone lung transplantation at our institution. In addition to patient demographics, data were collected on baseline nutritional status, energy and protein goals, type of specialized nutrition support therapy, length of therapy, and incidence of me

Nutritional management of pregnancy in patients on home parenteral nutrition.

Three pregnancies are reported in two patients on home parenteral nutrition for intestinal failure. The nutritional regimen, 2300 kcal (9.63 MJ) (18% as lipid emulsion), 14 g amino acid nitrogen and electrolytes supplied daily in a 'big bag', was based on the measured resting energy expenditure and urinary nitrogen excretion during the third trimester in a 30-year-old woman on home parenteral nutrition. Commercial preparations of trace elements were added to the infusion as dictated by serum levels. Iron, iodine and fluoride were given orally, and vitamin B12 by monthly intramuscular injection
