Health-care resource use among patients who use illicit opioids in England, 2010-20: A descriptive matched cohort study.

BACKGROUND AND AIMS: People who use illicit opioids have higher mortality and morbidity than the general population. Limited quantitative research has investigated how this population engages with health-care, particularly regarding planned and primary care. We aimed to measure health-care use among patients with a history of illicit opioid use in England across five settings: general practice (GP), hospital outpatient care, emergency departments, emergency hospital admissions and elective hospital admissions. DESIGN: This was a matched cohort study using Clinical Practice Research Datalink a

Evaluation of acute substance abuse patients admitted to the ICU by different scoring system.

Drug abuse poisoning is a significant public health problem in many countries all over the world. The recent study aimed to assess the applicability of NEWS II and APACHE II scores as predictors of the ICU admission of acute intoxication by substance abuse. The study enrolled 98 patients admitted to the Alexandria Poison Center of the Main University Hospital (APC-AMUH) with a history of acute intoxication with substance abuse. Collected data included socio-demographic data, circumstances of exposure, and clinical and laboratory data. These data were employed to calculate NEWS II and APACHE II

Disseminated intravascular coagulation is associated with poor prognosis in patients with COVID-19.

This study aimed to investigate the incidence and significance of disseminated intravascular coagulation (DIC) in coronavirus disease 2019 (COVID-19). A multicenter cohort study was conducted using large-scale COVID-19 registry data. The patients were classified into DIC and non-DIC groups based on the diagnosis on admission (day 1) and on any of the days 1, 4, 8, and 15. In total, 23,054 patients were divided into DIC (n = 264) and non-DIC (n = 22,790) groups on admission. Thereafter, 1654 patients were divided into 181 patients with DIC and 1473 non-DIC patients based on the DIC diagnosis on

Community-acquired pneumonia requiring admission to an intensive care unit: a descriptive study.

Severe community-acquired pneumonia (CAP) requiring admission to an intensive care unit (ICU) has been inadequately studied. We compared characteristics and outcomes of patients with CAP who were admitted to the ICU with those of patients managed on the ward. Of the 3675 patients hospitalized with CAP, 374 (10%) were admitted to the ICU. The main reason for ICU admission was respiratory failure requiring intubation and ventilation (n = 303, 81%), although this indication decreased with increasing age (p < 0.05 for trend). Most patients (62%) required mechanical ventilation for 3 days or less.

Role of the intensive care unit in children undergoing bone marrow transplantation with life-threatening complications.

The role of support measures in the Intensive Care Unit for bone marrow transplant recipients has been controversial. Data from 176 pediatric bone marrow transplants were retrospectively analyzed to ascertain the probability, causes, risk factors and survival for life-threatening complications requiring intensive care. Ninety-two patients underwent allogeneic BMT and 84 autologous BMT between January 1991 and December 1995. Thirty-one ICU admissions were recorded. The most frequent causes were acute respiratory failure (n = 15, mostly interstitial pneumopathies), septic shock (n = 5) neurologi

Prognosis of Very Elderly Patients after Intensive Care.

Elderly patients (over age 85) are increasingly treated in Intensive Care Units (ICU), despite doctors' reluctance to accept these frail patients. There are only few studies describing the relevance of treatments for this group of patients in ICU. One of these studies defined an age of 85 or over as the essential admittance criterion. Exclusion criteriwere low autonomy before admittance or an inability to answer the phone. Epidemiological data, history, lifestyle, and autonomy (ADL score of six items) were recorded during admission to the ICU and by phone interviews six months later. Eight Fre

Associations between hospital admission frequency and household financial outcomes in Korea.

BACKGROUND: Health shocks within families affect not only individual patients but also their entire household, across various economic and psychological dimensions. Rising hospital admissions are strongly associated with increased medical expenditures that strain household finances, with larger relative financial strain among lower-income households. However, the broader socioeconomic consequences of these shocks have yet to be sufficiently investigated, leaving a notable research gap. This study examines the association between hospital admission frequency and household financial stability, f

Prognostic value of blood lactate, blood glucose, and hematocrit in canine babesiosis.

Canine babesiosis typically causes hemolytic anemia but also can result in multiple organ dysfunction. Human patients with severe disease often have persistent hyperlactatemia, and blood lactate concentration is correlated with survival rate. In dogs, blood lactate concentration has been shown to be of prognostic value in patients with gastric dilatation-volvulus and in dogs admitted to intensive care units. Serial blood lactate and glucose concentrations and hematocrit on admission were determined in 90 dogs with naturally occurring, severe or complicated canine babesiosis. Forty-five dogs (5

Cannabis and Polypharmacy Leading to Hospitalization for Acute Confusion and Inability to Ambulate.

Inability to ambulate is a common issue for which an Emergency Department (ED) may request inpatient admission to the hospital. Frequent contributing factors include advanced age, spinal stenosis, osteoarthritis, deconditioning, and polypharmacy. We present an interesting case of a patient admitted due to inability to ambulate who had a significant acute decline in function coincident with the introduction of gabapentin in the setting of heavy tetrahydrocannabinol (THC) use.

The CLIPS Protocol: Cast re-aLignment In Paediatric patients using Stationery.

Paediatric tibial shaft fractures are commonly treated nonoperatively and followed-up in the clinic setting. Loss of position has traditionally been managed with wedging of the cast or admission and manipulation in the operaing theatre. The authors present a technique to aid in the wedging of plasters for paediatric tibia and forearm fractures that reduces time requirement, hospital costs and emotional distress on the part of the patient and family.

Care of spinal cord injury in non-specialist settings.

Patient with spinal cord injuries have individualised care routines to help prevent complications. Disruption to these routines following admission to non-specialist settings can have long-term consequences. This article focuses on the key long-term problems of pressure ulcers, bladder and bowel dysfunction, and autonomic dysreflexia. Nurses working on general wards need to consider how to manage these problems when caring for patients with spinal cord injury.

The continuing antenatal management program (CAMP): Outpatient monitoring of high-risk pregnancies. Keeps patients safe, costs low and care nearby.

A program was developed for referred, stable, high-risk obstetrical patients allowing them to receive antenatal care close to a tertiary hospital without the costs of a hospital admission. There were 426 women managed from September 2007 through December of 2012 with diagnosis of preterm labor, fetal anomalies, hypertensive disorders, placental abruption and other conditions. This management saved the hospital almost $9,000,000 USD or $20,956 USD per patient.

An unusual cause of terminal hematuria in a child: Eosinophilic cystitis.

Eosinophilic cystitis is a rare inflammatory disease of the bladder; it rarely occurs in children. Patients typically show irritative urination symptoms frequently, with a possible need for urgency, alongside dysuria, gross haematuria, suprapubic pain and painful urination. Sometimes bladder mass accumulation with the possibility of malignancy is also observed. We present an 8-year-old male patient who gained admission for terminal hematuria and discuss the management of eosinophilic cystitis.

[Fulminant perimyocarditis following tonsillitis].

Fulminant perimyocarditis is a rare and potentially fatal condition that may mimick myocardial infarction. The long-term prognosis is favourable, once the patient has survived the index admission, and recurrence is extremely rare. This case describes a 35-year-old man, who experienced recurrence twice within six months. Due to the risk of a potentially fatal outcome, patients should be followed closely with clinical controls of infection parameters and echocardiography.

Incipient neuroleptic malignant syndrome with quetiapine/paroxetine combination treatment: atypical presentation and early, successful rechallenge with olanzapine.

A 26-year-old caucasian male developed incipient neuroleptic malignant syndrome (NMS) during the course of paranoid schizophrenia with catatonic signs at admission, while on quetiapine/paroxetine combination treatment. The clinical presentation was "atypical", but increased creatine kinase levels and positive myoglobinemia were highly suggestive of the diagnosis. Of note, the patient was successfully rechallenged with olanzapine. The implications of the drug regimen and of the catatonic presentation are discussed.
