An active microbiological surveillance project at an Italian teaching hospital: microbial isolates, recent epidemiological trends, major clinical concerns, and antimicrobial susceptibility rates during a four-year period.

A microbiological surveillance program is currently performed at our tertiary-care teaching hospital. The temporal trend of microbial isolates from patients admitted during four calendar years (2004 to 2007) was analyzed according to the main bacterial and fungal culture organisms. The same pathogens isolated more than once from the same patient within one month were considered only once. On the whole, the main pathogen group remained that of Enterobacteriaceae (6,608 isolations out of 19,666: 33.6%, with Escherichia coli found in 60-75% of cases), with no significant difference over time. Pulmonary Tuberculosis in HIV Infection : The Relationship of the Radiographic Appearance to CD4 T-Lymphocytes Count.

Pulmonary tuberculosis (TB) in the AIDS population has a variable chest radiographic presentation. The association between the chest radiographic presentation of pulmonary TB and CD4 T-lymphocyte count in the HIV-infected patient was investigated in order to provide an empirical approach for early diagnosis, treatment, and isolation of these patients. A retrospective analysis of chest radiographs, CD4 T-lymphocyte counts, and clinical history of 80 patients from Hospital Kota Bharu, was performed. All patients were HIV-seropositive and had culture and /or cytology-proven pulmonary tuberculosis

Group C Orthobunyavirus Infection in a Patient from the Amazon Putumayo Region of Colombia During an Oropouche Fever Epidemic.

Orthobunyaviruses, one of the largest and most diverse genera of viruses, include numerous reassortant viruses and present diagnostic challenges due to similar clinical presentations with other arboviral infections. The detection and isolation of a group C orthobunyavirus from a patient with acute febrile illness from Puerto Ospina, Putumayo Department, Colombia during the 2023-2024 Latin American Oropouche fever regional outbreaks are reported in the present study. Using a vertebrate virus-focused metagenomics sequencing approach with VirCapSeq-VERT, a virus related to the Caraparu complex wa

Whole-genome sequencing and comparative genomic analysis of Irpex lacteus isolated from the female reproductive tract.

Irpex lacteus is a fungus typically found on angiosperm branches and trunks. Here, we report the isolation of a novel strain of I. lacteus from the cervix uteri of a female patient diagnosed with endometriosis and COVID-19 at Beijing Obstetrics and Gynecology Hospital, Capital Medical University. Using whole-genome sequencing, we generated a high-quality draft genome for this human-derived strain. Comparative genomic analysis revealed significant variations amongst I. lacteus strains colonizing distinct environments. Functional annotations indicated that environmental adaptation has influenced

[Resilience in COVID-19 times: general considerations on the recovery of a 93-year-old patient on haemodialysis treatment].

We report the case of a 93-year-old woman on haemodialysis treatment for more than 30 months and with multiple comorbidities who recovered from a Covid-19 infection without any significant clinical problems. The patient has shown a delay in viral clearance with swab test negativization (confirmed) after 33 days; after testing positive again, she has resulted persistently negative, (confirmed after 49 days). After the first negative swab, IgG and IgM antibodies have been found; these have remained persistently positive after a month. As well as highlighting an unexpected resilience in an extrem

Resection of a Posterior Mediastinal Mass in a 4-Year-Old Child Complicated by Difficult Airway Management and Emergent Use of Extracorporeal Membrane Oxygenation.

Literature on posterior mediastinal masses is limited. Furthermore, they have traditionally been described to pose lower cardiopulmonary risks compared with anterior mediastinal masses. Studies on posterior mediastinal masses are even more limited in the pediatric population. We present a case of a large posterior mediastinal mass in a 4-year-old child who presented with extremely difficult airway management during endobronchial intubation due to severe external compression that led to use of an adapted airway management technique with a rigid airway exchanger for lung isolation. Due to the pa

Small Cartilage Defect Management.

Cartilage defects in the knee are common resulting in significant pain and morbidity over time. These defects can arise in isolation or concurrently with other associated injuries to the knee. The treatment of small (< 2-3 cm2) cartilage deficiencies has changed as our basic science knowledge of tissue healing has improved. Advancements have led to the development of new and more effective treatment modalities. It is important to address any associated knee injuries and limb malalignment. Surgical options are considered when nonoperative treatment fails. The specific procedure depends on indiv

[Fatal sepsis due to an emerging bacteria: Arcanobacterium hemolyticum].

Arcanobacterium hemolyticum (A.haemolyticum) is a gram-positive bacillus. Man is the primary environmental reservoir. It is essentially an opportunistic pathogen in immunocompromised patients and may be responsible for infections of the skin and pharynx in healthy subjects, especially in children and adolescents. It can cause superinfections of chronic ulcers, but occasionally it causes invasive infections. Its isolation from culture samples is always difficult because it simulates many bacteria to which it is often associated in pathological products. There are not recommendations concerning

Chicken pox outbreak in the Intensive Care Unit of a tertiary care hospital: Lessons learnt the hard way.

Varicella-zoster virus (VZV) causes 2 clinically and epidemiologically distinct forms of diseases. Chickenpox (varicella) is the disease that results from primary infection with the VZV. Herpes zoster (HZ) results from the reactivation of VZV latently infecting the dorsal root ganglia. We are reporting an outbreak of varicella infection among the health care workers (HCWs) in the Intensive Care Unit (ICU) of a tertiary care hospital. We found transmission of varicella among eight HCWs of pulmonary ICU. They had a history of contact with a patient having HZ infection. Investigation of the outbr

A case of miliary pulmonary cryptococcosis and review of literature.

We describe a case of cryptococcal fungemia in a 62-year-old male renal transplant patient. The diagnosis was established by isolation of Cryptococcus neoformans using the Isolator(®) blood culture lysis-centrifugation system. Testing for cryptococcal antigens was negative in the serum and cerebrospinal fluid. Transbronchial lung biopsies and bronchoalveolar lavage were negative. Antifungal therapy with fluconazole was started, resulting in fever remission, and a sustained clinical response was achieved. The literature on miliary pulmonary cryptococcosis is reviewed, and three similar cases we

The legacy of the Cartwright report: "lest it happen again".

The 1987 Cartwright Report into events at New Zealand's National Women's Hospital catalysed sweeping changes to promote and protect patients' rights. A generation on, it is comfortable to believe that such sustained and deliberate violations of patient rights "couldn't happen here" and "couldn't happen now." And yet, contemporary examples beg a different truth. Three of Cartwright's messages hold an enduring relevance for health practitioners and patients: the need for patients to be respected as people; to be supported to make informed choices; and to have their voices heard, even when they w

First identification of eggs of the Asian fish tapeworm Bothriocephalus acheilognathi (Cestoda: Bothriocephalidea) in human stool.

We report the first case of egg isolation of the Asian fish tapeworm Bothriocephalus acheilognathi (Bothriocephalidea) from human stool. A male patient from Saint Laurent du Maroni (French Guiana) presenting abdominal pain was examined in France for the diagnosis of intestinal parasites. Diphyllobothrium-like eggs were observed in his stool. However, molecular phylogenetic analyses based on sequences of rDNA and COI genes showed that the eggs observed belong to a bothriocephalidean cestode B. acheilognathi. The adult life stages of B. acheilognathi cestodes are known as invasive parasites of a

Truncus bicaroticus with aberrant right subclavian artery and origin of right vertebral from right common carotid artery.

We describe a rare constellation of variant anatomy of the aortic arch branches, seen on a magnetic resonance angiographic examination during the course of investigation for recent onset memory loss in a 52-year-old patient. There was a common origin of both the common carotid arteries (CCA), the common trunk being the first major branch of the aortic arch, the right vertebral artery arising from the right CCA and the right subclavian artery arising as the last branch of the arch. In isolation, the three components of this constellation have been reported with different frequencies, but as per

Nocardiosis: risk factors, clinical characteristics and outcome.

Nocardiosis has been reported increasingly in recent two decades, probably due to improvement in isolation of the organism and increased burden of immune compromised patients. Nocardia occasionally has been reported in healthy people. A case series of definitive Nocardiosis (2002 to 2010), clinical characteristics, underlying diseases, immune status and in-patient outcome were studied in a tertiary referral center. Twenty one patients with definite diagnosis of Nocardiosis were studied. 17 cases (81%) had an underlying disease (diabetes mellitus, corticosteroid therapy, and chronic granulomato

Childhood subdural hemorrhage, macrocephaly, and coagulopathy associated with Prader-Willi syndrome: case report and review of the literature.

A 16-month-old girl with a history of Prader-Willi syndrome and progressive macrocephaly manifested large, bilateral, subdural hemorrhages of differing ages on magnetic resonance imaging. Subsequent evaluation revealed a deficiency of von Willebrand factor, but after repeated evaluations, no conclusive evidence of nonaccidental trauma became apparent. Subdural hemorrhages of varying ages are frequently associated with nonaccidental trauma during early childhood. However, several uncommon conditions may present as subdural hemorrhages and thus mimic nonaccidental trauma. Our patient demonstrate

Recanalization of splenic artery aneurysm after transcatheter arterial embolization using N-butyl cyanoacrylate.

A 65-year-old woman who had been diagnosed as having microscopic polyangiitis developed sudden abdominal pain and entered a state of shock. Abdominal CT showed massive hemoperitoneum, and emergent angiography revealed a ruptured splenic artery aneurysm. After direct catheterization attempts failed due to tortuous vessels and angiospasm, transcatheter arterial embolization using an n-butyl cyanoacrylate (NBCA)-lipiodol mixture was successfully performed. Fifty days later, the patient developed sudden abdominal pain again. Repeated angiography demonstrated recanalization of the splenic artery an

Assessment and treatment of suicide risk in schizophrenia.

Schizophrenic patients at risk of suicide are more likely to be young, male, white, never married, with post-psychotic depression, and a history of substance abuse and suicide attempts. Hopelessness, social isolation, awareness of illness and hospitalization are also related to suicide risk. Deteriorating health with a high level of premorbid functioning, recent loss or rejection, limited external support, family stress or instability are other features that have been reported in schizophrenic patients who commit suicide. Atypical antipsychotics, especially clozapine, have emerged as important

Post-traumatic infection of the lower limb caused by rare Enterobacteriaceae and Mucorales in a young healthy male.

Enterobacter amnigenus and Leclercia adecarboxylata are gram-negative aerobic bacilli of the family Enterobacteriaceae that have been isolated from water and, rarely, from various clinical specimens. Absidia is a filamentous fungus of the class Zygomycetes that is ubiquitous in nature and can cause infection, primarily in immunocompromised hosts. Here, we describe an infection of the left lower limb caused by E. amnigenus and L. adecarboxylata with subsequent isolation of Absidia spp. in a patient with multiple traumatic injuries after a major motor vehicle accident. The severity of the clinic

Emergent stent graft isolation of a knife-related subclavian arterial venous fistula: lessons learned during long-term follow-up.

We present the unusual case of a 42-year-old gentleman who entered the trauma center with a hemothorax and intermittent bright red pulsatile blood from three stab wounds in the ipsilateral supraclavicular fossa. The patient was found on emergent angiography to have a large right subclavian artery to vein fistula that was treated successfully with a Dacron-covered stent. Angiography, 2 years following stent-graft isolation, confirmed a durable result with evidence of hemodynamically insignificant neointimal growth through the pores of the graft material. This case provides insight into the long

Abnormal origin of the right subclavian artery from the right pulmonary artery in a patient with D-transposition of the great vessels and left juxtaposition of the right atrial appendage: an unusual anatomical variant.

Isolation of the left subclavian artery (LSCA) or its anomalous origin from the pulmonary artery (PA) has been documented in several cases, especially in association with a right-sided aortic arch. Similar anomalies involving the right subclavian artery (RSCA) are less frequent. Anomalous origin of the RSCA from the PA in association with D-transposition of the great arteries (D-TGA) is exceedingly rare and only two cases have been reported so far. We present here, a case of aberrant origin of the RSCA from the right PA in a patient with D-TGA, in whom the diagnosis was rendered difficult due
