Establishing the diagnosis of lung cancer: Diagnosis and management of lung cancer, 3rd ed: American College of Chest Physicians evidence-based clinical practice guidelines.

BACKGROUND: Lung cancer is usually suspected in individuals who have an abnormal chest radiograph or have symptoms caused by either local or systemic effects of the tumor. The method of diagnosis of lung cancer depends on the type of lung cancer (small cell lung cancer or non-small cell lung cancer [NSCLC]), the size and location of the primary tumor, the presence of metastasis, and the overall clinical status of the patient. The objective of this study was to determine the test performance characteristics of various modalities for the diagnosis of suspected lung cancer. METHODS: To update pr

Patient-centered priorities in endometriosis and chronic pelvic pain: A mixed-methods and thematic analysis of intake narratives.

INTRODUCTION: Understanding what patients prioritize when seeking care for endometriosis and chronic pelvic pain (CPP) is crucial for delivering personalized, responsive care. However, patient-defined goals remain poorly characterized across various life stages and disease phenotypes. This study aimed to explore and rank patient priorities at the time of tertiary referral to inform patient-centered care models. MATERIAL AND METHODS: We conducted a convergent mixed-methods study of 649 new patients referred to a tertiary endometriosis center between 2021 and 2024. Participants completed a stan

Unprecedented journey to 650 transpedicular screws using freehand technique and intraoperative C-arm imaging with technical nuances.

INTRODUCTION: Pedicle screw placement plays a crucial role in treating various cases such as fractures, scoliosis, degenerative spine issues, and kyphosis, reinforcing all three spinal columns simultaneously. While three-dimensional navigation-assisted pedicle screw placement is considered superior, the freehand technique relies on anatomical landmarks and tactile feedback, with observed low complication rates. MATERIALS AND METHODS: This was a prospective single-center study conducted over a period of 3 years. It included all patients of dorsal, lumbar, and sacral spinal instability of myria

Take 2: The maintenance of a colorectal cancer screening program in intervention clinics and roll out in usual care clinics during the second year of a pragmatic trial.

BACKGROUND: In the United States, patients in rural settings screen for colorectal cancer (CRC) at a lower rate than urban patients. SMARTER CRC, a pragmatic trial to increase CRC screening in rural clinics in Oregon, implemented a screening program that included 1) mailing fecal immunochemical tests (FIT tests) in partnership with Medicaid health plans and 2) training clinics in navigating patients to complete colonoscopy follow-up to abnormal FIT testing. We describe the second year of the SMARTER CRC project, which includes the second year of implementation for the intervention clinics, and

Combining the power of flexible and navigable suction access sheath (FANS) and direct in scope suction (DISS): introduction to the flexible integrated retrograde suction-ureteroscopy technique (FIRST) procedure-an audit study by the endourology section of the European Association of Urology.

OBJECTIVE: Flexible ureteroscopy (F-URS) with suction technology has become a minimally invasive surgery for the management of renal stones. While the Flexible and Navigable Suction Ureteral Access Sheath (FANS) and Direct In-Scope Suction (DISS) ureteroscopes individually have proven benefits and restrictions, there is no dedicated research if integrating both these technologies can complement the limitations of each to improve suction F-URS outcomes. This audit introduces the Flexible Integrated Retrograde Suction-Ureteroscopy Technique (FIRST) procedure, which integrates FANS with a DISS ur

Optimizing instrument selection during core decompression for osteonecrosis of the femoral head: a 3D analysis.

BACKGROUND: Core decompression is the leading surgical treatment for pre-collapse osteonecrosis of the femoral head (ONFH). There are many different instruments that are used during core decompression, ranging from drill bits to expandable reamers, yet there is limited guidance regarding how to select the appropriate instrument for an individual patient's unique anatomy. We sought to examine how lesion characteristics affect instrument performance. To quantitatively compare instruments' efficacy at removing necrotic bone while preserving healthy bone, we developed an in-silico metric called th

Comparison of the Efficacy and Safety of Myasthenia Gravis Treatments: A Bayesian Network Meta-Analysis.

BACKGROUND AND OBJECTIVES: In the past decade, several novel therapies have shown promise in treating generalized myasthenia gravis (MG); however, no head-to-head prospective studies have compared their efficacy and safety. To assist clinicians in navigating this complex treatment landscape, we sought to compare treatments while accounting for differences in trial populations. METHODS: We conducted a Bayesian network meta-analysis (PROSPERO ID: CRD42023428733) of MG treatments. We searched Scopus, Embase, Web of Science, CENTRAL, and MEDLINE up to May 2, 2025, for randomized trials in MG. Two

Planning transcatheter pulmonary valve implantation in the dysfunctional native RVOT: A semi-automated pipeline for dynamic analysis based on 4D-CT imaging.

BACKGROUND AND OBJECTIVE: Dysfunction of the right ventricular outflow tract (RVOT) is a common long-term complication following surgical repair in patients with congenital heart disease. Transcatheter pulmonary valve implantation (TPVI) offers a viable alternative to surgical pulmonary valve replacement (SPVR) for treating pulmonary regurgitation but not all RVOT anatomies are suitable for TPVI. To identify a suitable landing zone (LZ) for TPVI, three-dimensional multiphase (4D) computed tomography (CT) is used to evaluate the size, shape, and dynamic behavior of the RVOT throughout the cardi

User and Usability Testing of a Web-Based Genetics Education Tool for Parkinson Disease: Mixed Methods Study.

BACKGROUND: Genetic testing is essential to identify research participants for clinical trials enrolling people with Parkinson disease (PD) carrying a variant in the glucocerebrosidase (GBA) or leucine-rich repeat kinase 2 (LRRK2) genes. The limited availability of professionals trained in neurogenetics or genetic counseling is a major barrier to increased testing. Telehealth solutions to increase access to genetics education can help address issues around counselor availability and offer options to patients and family members. OBJECTIVE: As an alternative to pretest genetic counseling, we de

Moving Person-Centered Care Planning From Theory Into Practice: Patient Perspectives.

INTRODUCTION: Persons with multiple chronic conditions often receive care from a complicated network of multiple providers across multiple health systems unique to their conditions, needs, available resources, and insurance. Patients must navigate this healthcare system, which is a difficult and complicated process with just one provider or system, let alone multiple. Developing new solutions to overcome the systemic challenges within healthcare systems to provide high-quality, comprehensive, person-centered care for persons living with, or at risk for, multiple chronic conditions is critical.

Carbon fiber-reinforced PEEK implants in oncologic spine surgery: a multicenter experience on implications for postoperative patient management.

OBJECTIVE: Carbon fiber-reinforced polyetheretherketone (CFRP) implants have been used safely in treating spinal oncological disease and promise improved imaging follow-up and radiotherapy planning. However, data on the implant's routine use in the clinical setting with implications for postoperative management are scarce. The aim of this observational study was to provide real-world insight into the current use of CFRP instrumentations in spinal oncological disease and to define their actual relevance for postoperative imaging follow-up and adjuvant treatment planning. METHODS: Datasets of p

Surgery of Motor Eloquent Glioblastoma Guided by TMS-Informed Tractography: Driving Resection Completeness Towards Prolonged Survival.

BACKGROUND: Surgical treatment of patients with glioblastoma affecting motor eloquent brain regions remains critically discussed given the risk-benefit dilemma of prolonging survival at the cost of motor-functional damage. Tractography informed by navigated transcranial magnetic stimulation (nTMS-informed tractography, TIT) provides a rather robust estimate of the individual location of the corticospinal tract (CST), a highly vulnerable structure with poor functional reorganisation potential. We hypothesised that by a more comprehensive, individualised surgical decision-making using TIT, tumou

Minimal invasive biopsy of intraconal expansion by PET/CT/MRI image-guided navigation: a new method.

UNLABELLED: Intraorbital tumours are often undetected for a long period and may lead to compression of the optic nerve and loss of vision. Although CT, MRI's and ultrasound can help in determining the probable diagnosis, most orbital tumours are only diagnosed by surgical biopsy. In intraconal lesions this may prove especially difficult as the expansions are situated next to sensitive anatomical structures (eye bulb, optic nerve). In search of a minimally invasive access to the intraconal region, we describe a method of a three-dimensional, image-guided biopsy of orbital tumours using a combin
