Intracoronary Thrombolysis During Primary PCI for STEMI: An Updated Meta-Analysis of Randomized-Controlled Trials.

Restoration of epicardial patency with primary percutaneous coronary intervention (PCI) in ST-segment elevation myocardial infarction (STEMI) does not necessarily translate into adequate tissue-level reperfusion. Adjunctive intracoronary thrombolysis has been proposed to improve microvascular perfusion, but randomized data remain heterogeneous. This study sought to evaluate the efficacy and safety of adjunctive intracoronary thrombolysis during primary PCI for STEMI. We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) comparing adjunctive intracoronary thr

Evaluation of Payer Policies to Reduce Low-Value Medical Device-Based Procedure Use.

IMPORTANCE: Low-value care occurs when the harms or costs of care exceed the benefits. Such care includes when medical device-based procedures are favored over medical management, such as invasive coronary angiography (ICA) and percutaneous coronary intervention (PCI) for stable coronary artery disease; endovascular intervention for lower extremity peripheral arterial disease with intermittent claudication; and nasal endoscopy with balloon ostial dilation and functional endoscopic sinus surgeries (collectively, sinus procedures) for chronic rhinosinusitis. OBJECTIVE: To evaluate the associati

Current and future applications of robotics in structural heart interventions.

Robotics entered the cardiovascular field in the late 1990s with a robot-assisted coronary artery bypass graft. Since then, the use of robots has become a common part of cardiovascular surgery in several types of interventions. The experience in transcatheter interventions has been slower, and the application of robotics to percutaneous coronary interventions has shown some encouraging results but also some technical limitations. Following the growth of structural heart interventions, attention has recently switched to the potential application of robotics in this field. So far, several cases

Considerations for Women with Congenital Heart Disease Undergoing Percutaneous Cardiovascular Procedures.

The catheterization of women with congenital heart disease has unique considerations that must be taken into account. Hemodynamic changes secondary to pregnancy, anticoagulation strategies of women in child bearing years, and protection of a fetus during pregnancy require interventions and evidence of current therapies in the treatment of women continues to have more questions than answers in our current era. This review highlights some challenges in the catheterization of women with congenital heart disease.

Insights on DAPT Abbreviation and De-escalation from ULTIMATE-DAPT and Related Trials: Are we Heading Toward an Aspirin-Free Strategy?

The results of the recently concluded ULTIMATE-DAPT and T-PASS trials strongly support the emerging concept of antiplatelet monotherapy in patients at high bleeding risk undergoing percutaneous coronary intervention. Monotherapy with more potent antiplatelets such as ticagrelor is both a safe and an equally effective strategy to circumvent major bleeding episodes in patients at high bleeding risk while guarding against ischemic events. Although these results were not replicated with low-dose prasugrel monotherapy in the STOP-DAPT-3 trial, the other major trials investigating ticagrelor monothe

Dual antiplatelet therapy duration and stent type in patients with high bleeding risk: A systematic review and network meta-analysis.

BACKGROUND: It is uncertain whether the efficacy and safety of dual antiplatelet therapy (DAPT) in patients with high bleeding risk (HBR) vary according to DAPT duration and stent type (eg, durable polymer drug-eluting stents (DP-DESs), biodegradable polymer DESs (BP-DESs), or polymer-free drug-coated stents (PF-DCSs)). We aimed to study the stent type and DAPT duration appropriate for patients with HBR. METHODS: PubMed and EMBASE were searched until October 2023. Randomized controlled trials (RCTs) involving patients with HBR that compared standard DAPT (6-12 months) with DP- or BP-DES versu

High platelet reactivity strongly predicts early stent thrombosis in patients with drug-eluting stent implantation.

Stent thrombosis (ST) is a fatal complication after percutaneous coronary intervention (PCI). The association between P2Y12 reaction unit (PRU) level and stent thrombosis occurrence remains unclear. Based on the multicenter, observational PTRG-DES (Platelet function and genoType-Related long-term proGnosis in DES-treated patients) registry of patients with drug-eluting stents (DES) implantation, a total of 11,714 patients with PRU values were analyzed. We sought to identify the predictors of early stent thrombosis (EST) and compared the primary outcome, a composite of cardiac death, myocardial

The Prognostic Value of Distinct Histological Growth Patterns of Colorectal Peritoneal Metastases: A Pilot Study.

BACKGROUND: Different histological growth patterns (HGP) describing the tumor-to-liver interface have been described in colorectal liver metastases and have been associated with a strong prognostic value. However, HGP of peritoneal metastases (PM) of colorectal cancer (CRC) have not yet been described. Our objective was to determine whether distinct HGP can be identified in PMCRC and to evaluate their potential prognostic value in these patients. METHODS: This retrospective study included 38 patients who underwent curative-intent surgery for PMCRC between July 2012 and March 2019, with PCI≤6,

Heart-Brain Team Approach of Acute Myocardial Infarction Complicating Acute Stroke: Characteristics of Guideline-Recommended Coronary Revascularization and Antithrombotic Therapy and Cardiovascular and Bleeding Outcomes.

Background Acute myocardial infarction (AMI) infrequently occurs after acute stroke. The Heart-brain team approach has a potential to appropriately manage this poststroke cardiovascular complication. However, clinical outcomes of AMI complicating acute stroke (AMI-CAS) with the heart-brain team approach have not been characterized. The current study investigated cardiovascular outcomes in patients with AMI-CAS managed by a heart-brain team. Methods and Results We retrospectively analyzed 2390 patients with AMI at our institute (January 1, 2007-September 30, 2020). AMI-CAS was defined as the oc

Unlocking Promising Therapies: Drug-Eluting Stents in Medically Refractory Angina Patients With Myocardial Bridging.

Myocardial bridging (MB) is a congenital variation in which a coronary artery segment tunnels through the myocardium instead of following its usual epicardial route. Although MB is usually diagnosed incidentally and has a good long-term prognosis, it can lead to complications such as angina, myocardial infarction, arrhythmias, and sudden death. This study aimed to evaluate the outcomes of drug-eluting stent (DES) implantation in patients with MB and medically refractory angina. The study included 12 patients with significant MB who did not respond to maximal medical therapy and underwent DES i
