Surgical Offloading in the Diabetic Foot.

Diabetic foot ulcers are severe complications of diabetes resulting in high health care costs and reduced quality of life. Persistent mechanical pressure, neuropathy, foot deformity, and patient comorbidities contribute to the ulcer formation and poor healing. While nonsurgical offloading techniques are common, they are associated with noncompliance, skin irritation, and limited wound assessment capability. Surgical offloading offers a direct approach in reducing and redistributing pressures to promote ulcer healing. Its success depends on proper patient selection, glycemic control, and postop

A Rare Case of Bronchopulmonary Infection by Lophomonas Blattarum : A Case Report.

Lophomonas blattarum is a flagellated protozoan parasite found in cockroaches' and termites' hindgut. It can rarely cause bronchopulmonary infection in humans, especially in people with other comorbidities or immunocompromised, but the prevalence and characterization of the disease remains poorly understood. In this case report, we present a case of a 51-year-old male patient with underlying Hepatitis B presenting with a persistent cough for more than two weeks. During evaluation, microscopic examination of the wet mount of Bronchoalveolar lavage revealed actively motile trophozoite of Lophomo

Targeting HBV with RNA interference: Paths to cure.

Chronic hepatitis B virus (HBV) infection affects millions worldwide despite the availability of effective vaccines. The stability of HBV's genomic minichromosome (cccDNA) within hepatocytes, the persistence of integrated viral sequences capable of producing viral antigens, and the ability of the virus to evade immune control all contribute to the difficulty in achieving a functional cure. Existing antiviral treatments have minimal impact on HBV transcription, allowing persistent viral antigen production and immune dysfunction. Emerging RNA interference (RNAi) therapies targeting HBV RNAs redu

How I do it: continuous intraventricular interferon alpha infusion in pediatric patients with subacute sclerosing panencephalitis.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a chronic disease affecting the central nervous system (CNS) because of persistent measles virus (MeV) infection. Among the various treatment available the intraventricular interferon alpha administration demonstrated greater effectiveness. METHOD: In this article is described the step-by-step surgical technique of the positioning of an intraventricular catheter connected to a rechargeable subcutaneous pump. The main surgical steps and the pump settings are illustrated in a supplementary video. CONCLUSION: This surgical management gua

Post-COVID pulmonary injury in K18-hACE2 mice shows persistent neutrophils and neutrophil extracellular trap formation.

The involvement of neutrophils in the lungs during the recovery phase of coronavirus disease 2019 (COVID-19) is not well defined mainly due to the limited accessibility of lung tissues from COVID-19 survivors. The lack of an appropriate small animal model has affected the development of effective therapeutic strategies. We here developed a long COVID mouse model to study changes in neutrophil phenotype and association with lung injury. Our data shows persistent neutrophil recruitment and neutrophil extracellular trap formation in the lungs for up to 30 days post-infection which correlates with

Spinal Cord Stimulation in Refractory Postherpetic Neuralgia in Portugal: A Case Report.

Postherpetic neuralgia is one of the most severe complications after herpes zoster infection. Patients who experience persistent pain despite conservative treatment may benefit from interventional therapies, such as spinal cord stimulation. We present the case of a patient with severe refractory postherpetic neuralgia in the right T8 to L1 distribution who responded effectively to spinal cord stimulation. After its implantation, the patient had improvements in pain intensity, pain-related interference, quality of life, and satisfaction, with a simultaneous reduction of previous medications. Th

Severe postherpetic neuralgia and facial paralysis in the oral and periauricular regions managed with acupuncture and electroacupuncture: a case report.

Postherpetic neuralgia (PHN) is a severe and persistent pain condition following herpes zoster infection. This case report details the analgesic effects of acupuncture combined with electroacupuncture in a 66-year-old male patient presenting with PHN and peripheral facial paralysis, who showed limited response to conventional treatment with corticosteroids and antiviral medications. Following a comprehensive treatment protocol, including pricking-cupping bloodletting, and targeted acupuncture, the patient experienced significant pain relief and improved facial nerve function. This report highl

Complete heart block caused by sinus of valsalva pseudoaneurysm-A rare case.

Acquired pseudoaneurysms of the aortic root involving the sinus of Valsalva (SOV) are rare and serious complications arising from trauma, infection, or following cardiac surgery or intervention. Complete heart block (CHB) is an atypical presentation of SOV pseudoaneurysm due to either direct compression effects or involvement of the main conducting system by blood and inflammatory cell infiltration. Herein, we describe a rare case of a patient who presented with CHB caused by an SOV pseudoaneurysm following polytrauma and was treated with surgical closure of pseudoaneurysm followed by implanta

Mitigating Infectious Complications of Injection Pharyngoplasty with Calcium Hydroxyapatite for Velopharyngeal Insufficiency: Retropharyngeal Abscess and Osteomyelitis.

Velopharyngeal insufficiency is a complex condition with various treatment options. In this case, a patient with a cleft palate who was treated for velopharyngeal insufficiency with calcium hydroxyapatite injection pharyngoplasty developed persistent cervical pain on postoperative day 6. CT imaging showed a hypodense structure in the right retropharyngeal tissue, and cultures tested positive for Streptococcus intermedius and Staphylococcus aureus. An MRI later revealed skull base osteomyelitis. The infection was controlled via surgical intervention and antibiotics. This case highlights potenti

Congenital perineal grove: an unusual Anorectal anomaly in females-report of two cases and literature review.

Congenital perineal grove (CPG) is a rare anorectal anomaly; only 65 cases have been reported in literature. Two cases who were referred for evaluation of a lesion in the perineum are reported here. The patients were diagnosed clinically as CPG in neonatal period and were initially managed conservatively. Surgery was required in one case as the lesion was persistent and symptomatic. A high index of suspicion is required for diagnosis of CPG to avoid parental anxiety and unnecessary diagnostic work-up and surgery. Surgery is required only in cases where the lesion persists or there is infection

A Rare Case of Mucosa-Associated Lymphoid Tissue Lymphoma in the Ileum.

The body contains mucosa-associated lymphoid tissue (MALT), with the greatest amount located in the gastrointestinal (Gl) tract. Lymphoma may form when the cell growth in this tissue is aberrant. The small intestine is a common extranodular site of lymphoma, a systemic illness. Additionally, it has been proposed that MALT lymphomas (MALTomas) arise as a result of chronic and persistent immunological activation, either of an autoimmune or infectious type. The MALToma that develops in the duodenum is typically thought to be unrelated to Helicobacter pylori infection. However, some examples show

An Agitated Patient With COVID-19 Infection and Early-onset Alzheimer Disease.

Encephalopathy, delirium, and agitation are documented symptoms of coronavirus disease (COVID-19) infection, but research into the management of agitation in the setting of COVID-19 and pre-existing neuropsychiatric disease is ongoing. We present a 55-year-old male patient with early-onset Alzheimer disease and deteriorating mental and functional status who presented to our institution with agitation and persistent COVID-19 positivity on polymerase chain reaction testing. His agitation was improved through pharmacologic optimization including the avoidance of benzodiazepines and initiation of

Urology case report: Multifactorial bladder dysfunction in the setting of down syndrome.

A 16-year-old male patient with Down syndrome diagnosed with AKI and urinary tract infection was treated with meropenem for ESBL-positive E. coli in urine culture. Persistently elevated creatinine and persistent post-void residual (PVR) of >300 mL led to further testing, which revealed urethral stricture and a lower sacral Tarlov cyst. Due to no complete improvement with urethral dilatation, he underwent laminectomy and Tarlov cyst fenestration. Creatinine normalized, with increased urine output and robust flow. Due to a PVR of >100 mL, he received behavioral therapy, including sitting and tim

Burkholderia cepacia Complex Lumbar Spondylodiscitis: A Rare Nosocomial Infection.

Pyogenic spondylodiscitis is rarely caused by Burkholderia cepacia complex. B. cepacia is widespread in the environment and recognized as an opportunistic pathogen for patients with cystic fibrosis and immune disorders. A female in her mid-30s with underlying hyperthyroidism, but otherwise immunocompetent, was admitted to the hospital with persistent lower back pain after elective bariatric surgery in Mexico. Lumbar MRI showed L2/L3 osteomyelitis and discitis. Culture of disk aspiration grew Burkholderia cepacia complex sensitive to cefepime, ceftazidime, ciprofloxacin, gentamicin, imipenem, l

Lower Airway Infection Delaying the Diagnosis of Aortic Dissection in an Elderly Woman.

Type A aortic dissection is a surgical emergency occurring when an intimal tear in the aorta creates a false lumen in the ascending aorta. The authors report the case of an older woman with a medical history of arterial hypertension, atrial fibrillation, dyslipidemia, heart failure, and osteoarticular spinal pathology, presenting with sudden and persistent retrosternal pain, who was initially misdiagnosed with a lower airway infection and was discovered to have an acute type A aortic dissection. The authors intend to draw attention to medical errors and emphasize the importance of early diagno

A Coxoperitoneal Shunt as Salvage for a Recurrent Giant Seroma of the Hip After Periprosthetic Joint Infection: A Case Report.

CASE: A giant seroma developed in the hip of a 44-year-old man after resection arthroplasty performed for recurrent periprosthetic joint infection (PJI). The seroma persisted despite joint reconstruction but was ultimately treated successfully by internal drainage through a coxoperitoneal shunt. CONCLUSION: Resection arthroplasty of the hip is considered a salvage procedure for failed implant retention in the situation of persistent PJI. Nevertheless, functional results are poor and further soft-tissue complications may occur. A giant seroma of the hip may be drained into the peritoneal cavit

Dilated Cardiomyopathy Risk in Patients with Coronavirus Disease 2019: How to Identify and Characterise it Early?

Multiple lines of evidence have shown that elevated blood troponin is strongly associated with poor prognosis in patients with the novel coronavirus disease 2019 (COVID-19). Possible mechanisms of myocardial injury in COVID-19 include ischaemia due to circulatory and respiratory failure, epicardial or intramyocardial small coronary artery thrombotic obstruction due to increased coagulability, and myocarditis caused by systemic inflammation or direct binding of the virus to its receptor, angiotensin-converting enzyme-2 (ACE2), which is abundantly expressed in the heart. It is postulated that pe

Management of Monteggia Injuries in the Adult.

Monteggia fracture-dislocation of the elbow is a fracture of the proximal ulna with associated dislocation of the radial head or radial neck fracture. In adults, this injury is managed with open reduction and internal fixation of the ulna fracture. Care should be taken to ensure anatomic reduction of the proximal ulna. If radial head dislocation or subluxation persists, reduction of the ulna should be reassessed. Rarely, interposed soft tissue may block radial head reduction, and requires removal. Complications include hardware prominence, stiffness, infection, heterotopic ossification, nerve

Herpes Zoster-Mediated Radiculitis After Lumbar Spine Surgery: A Report on 2 Cases with Differing Outcomes.

CASES: Two cases of herpetic neuralgia after lumbar fusion are reported. Although in one case the diagnosis was evident and treatment promptly initiated, the second exhibited a less typical presentation of neuralgia predating the cutaneous rash leading to a misdiagnosis of persistent nerve root stenosis, with additional surgeries and a longer hospital stay. CONCLUSION: The cases presented highlight why postoperative neuralgia should be approached carefully and systematically investigated. Although rare, postoperative neuralgia may occur as a consequence of postoperative herpes zoster infectio

V348I mutation in UL23 gene of human herpesvirus 1 in a case of herpetic hepatitis and haemophagocytic lymphohistiocytosis.

We herein report the case of a young immunocompetent adult patient with a rapidly fatal haemophagocytic lymphohistiocytosis syndrome related to human herpesvirus 1 (HHV-1) infection, with herpetic hepatitis and persistent high-level viraemia despite treatment with acyclovir. Haemophagocytic lymphohistiocytosis was confirmed in the patient's spleen and bone marrow. HHV-1 DNA was extracted from whole blood and liver biopsy and the UL23 gene was sequenced. A V348I natural polymorphism of the TK protein was found in blood and liver specimens. Further studies are needed to investigate the role of t
