Smoking and causes of death among U.S. veterans: 16 years of observation.

In a 16-year mortality followup of some 293,000 insured U.S. veterans, specific causes of death were studied in relation to smoking status. The main results confirmed earlier findings.Mortality ratios for cigarette smokers as compared with nonsmokers were 1.73 for all causes of death, 1.58 for all cardiovascular diseases, 2.12 for all cancers, and 4.31 for all respiratory diseases. The highest ratios (those greater than 5.0) were observed for cor pulmonale, aortic aneurysm, emphysema and bronchitis, cancer of the pharynx, cancer of the esophagus, cancer of the larynx, and cancer of the lung an

Effects of Anesthetic Depth Using the Bispectral Index During Sleep Endoscopy With PAP Titration.

OBJECTIVES: Airway collapsibility varies with the level of anesthetic depth during drug-induced sleep endoscopy (DISE). Bispectral index (BIS) monitoring uses EEG signals to quantify anesthetic depth. We conducted a prospective study to examine changes in VOTE scores and pharyngeal opening pressure (PhOP) during DISE with positive airway pressure (DISE-PAP), at light and deep sedation. METHODS: Anesthetic depth was monitored using BIS during DISE-PAP for 17 patients. We classified BIS ≥ 55 as light sedation and < 55 as deep sedation. The DISE exam was performed and subsequently PAP applied to

Effects of Herbst and Pendex appliances treatment on the upper airway: A cone-beam computed tomography analysis.

INTRODUCTION: Class II functional appliances are widely used to treat skeletal Class II malocclusion in growing patients, whereas molar distalization is often employed for dental Class II malocclusion. However, the impact of these treatment modalities on upper airway dimensions remains controversial. This study examines the effects of Herbst appliance treatments on the upper airway changes compared with the Pendex appliance in adolescent patients with Class II malocclusion. METHODS: Cone-beam computed tomography (CBCT) scans of 50 patients with Class II malocclusion were obtained from a priva

Concordance between upper and lower airway microbiota in children with cystic fibrosis.

Introduction. Sputum is the most used sample type to monitor the lower respiratory tract microbiota in cystic fibrosis (CF), but young patients often cannot expectorate.Hypothesis. We hypothesized that throat swabs could reflect lower airway colonization and assessed the concordance of bacterial community composition between paired sputum and throat swab samples from children with CF.Aim. We aimed to compare bacterial community diversity and composition between sputum and throat swabs in the full cohort and in patients with paired samples from the same visit.Methodology. The prospective longit

Automated Computed Tomography Segmentation of the Pharyngeal Airway and Palate to Accelerate Tübingen Palatal Plate Fabrication in Pierre Robin Sequence.

Infants with Pierre Robin sequence frequently develop upper airway obstruction due to micrognathia and glossoptosis. The Tübingen palatal plate repositions the tongue base anteriorly to improve airway patency; however, conventional fabrication requires serial intraoral impressions and repeated nasoendoscopy, which prolongs airway compromise. Computed tomography (CT) enables single-session virtual spur design, yet manual pharyngeal airway and hard palate segmentation is labor-intensive, delaying treatment. The authors evaluated convolutional neural network-based automated segmentation to accele

Randomized controlled trial to examine the tolerability of bronchoscopy with and without pharyngeal laryngeal anesthesia (TACOYAKI study).

BACKGROUND: The SARS-CoV-2 pandemic, pharyngeal anesthesia such as nebulizer or lidocaine pump spray is the risk of droplet transmission to health care workers from coughing due to spraying anesthesia. Absence of pharyngeal anesthesia may induce coughing and reduce patient and operator satisfaction, but the efficacy of pharyngeal anesthesia under sedation is still limited. Therefore we evaluated a prospective, randomized, single-blind trial to evaluate efficacy of pharyngeal anesthesia in patients receiving sedation. MATERIALS AND METHODS: We conducted a randomized comparison of pharyngeal an

Evaluation of Oropharyngeal Structure and DISE Findings for Successful Soft Palatal Webbing Flap Pharyngoplasty.

PURPOSE: Lateral pharyngeal wall collapse, linked to weak pharyngeal wall stability, is characteristic of severe obstructive sleep apnea (OSA) patients. Soft palate webbing flap palatopharyngoplasty (SPWFPP) has been introduced to enhance the stability of the lateral pharyngeal wall with minimal postoperative complications by eliminating the need for tonsillectomy. This study analyzed the anatomic findings of oropharynx for determination of outcome in OSA patients requiring SPWFPP. METHODS: Twenty-eight OSA patients who underwent SPWFPP combined with septoturbinoplasty were included in this s

Study of the upper airway anatomy using magnetic resonance imaging in Indian obese patients with obstructive sleep apnea - A pilot study.

BACKGROUND: Obstructive sleep apnea (OSA) is very common in obese patients. However, why some obese patients have severe OSA while others do not is unclear. Research is limited regarding which structures contribute to upper airway narrowing, especially in Asian patients where bony restrictions is thought to be important. METHODS: Nineteen consecutive patients with BMI ≥35 kg/m2, and newly diagnosed with OSA based on overnight polysomnography were studied using non-contrast magnetic resonance imaging (MRI) of the upper airway during wakefulness. RESULTS: Patients were divided into two groups,

Impact of low pharyngeal/esophageal pressure associated with sarcopenia on postendoscopic submucosal dissection pneumonia in patients with superficial esophageal cancer.

OBJECTIVES: The aging population, including patients with superficial esophageal cancer, encounters critical dysphagia- and postoperative pneumonia-related issues. Although endoscopic submucosal dissection (ESD) provides advantages over other modalities, older patients are at higher risk of postoperative pneumonia. Furthermore, the etiologies of pneumonia are complex and include patient- (such as sarcopenia) and treatment- (including ESD) related factors. Therefore, this study evaluated swallowing function in patients with superficial esophageal cancer and identified post-ESD pneumonia-associa
