Hemomediastinum resulting from subclavian artery laceration during internal jugular catheterization.

UNLABELLED: The complications associated with internal jugular vein catheterization are inadvertent arterial puncture, pleural and mediastinal injuries, pneumothorax, hemothorax, and hemomediastinum. Complications caused by laceration of a subclavian artery are rare. We present a case of hemomediastinum resulting from laceration of the subclavian artery during central venous catheterization. After right internal jugular vein catheterization, the left lateral decubitus position was maintained for 6 h during surgery. The severe hypotension was first noted in the supine position after transfer to

[Idiopathic mediastinal and subcutaneous emphysema in a patient with acute lymphocytic leukemia after allogeneic bone marrow transplantation].

A 45-year-old man was diagnosed as having Ph1+ acute lymphocytic leukemia (ALL) in February 1997. Complete remission was achieved by chemotherapy. Allogeneic BMT from his HLA-identical sister was performed on June 11, 1997. Engraftment was relatively quick, but acute GVHD (grade I) developed. The patient was discharged on day 113. Seven months after BMT, in January 1998, exertional dyspnea developed gradually. Chest X-ray examination showed diffuse interstitial pneumonia, for which corticosteroid was started immediately. The symptoms and signs gradually improved. However, on the 20th hospital

[A case of mediastinal myelolipoma].

Myelolipoma is a benign tumor composed of mature adipose tissue and hematopoietic tissue. It was mainly found in the adrenal grand, but there have been reports of extra-adrenal locations. Only 7 cases of mediastinal myelolipoma have been reported ever. We have experienced a case of mediastinal myelolipoma surgically resected from a 55-year-old-man. He has visited our department because of chest pain. Chest X-ray showed typical pneumothorax, but abnormal mediastinal shadow was remarked. Chest CT showed a well circumscribed mass with fat and soft-tissue attenuation in posterior mediastinum. Lung

Concurrent Pneumocystis carinii and cytomegalovirus pneumonia after autologous peripheral blood stem cell transplantation.

A 46-year-old woman developed concurrent CMV and Pneumocystis carinii pneumonia (PCP) 140 days after autologous peripheral blood stem cell transplantation (APBSCT) for AML. She was seropositive for CMV before undergoing APBSCT and had required prednisone for immune thrombocytopenia and allergic dermatitis for 9 weeks prior to the onset of pneumonia. She had also been receiving PCP prophylaxis with pentamidine aerosol every month for 3 months before developing symptoms. The pneumonia was complicated by severe hypoxia, requiring ventilator support and pneumothorax requiring chest tube thoracosto

Complications of subclavian catheterization in hemodialysis patients.

Subclavian vein catheterization, the most commonly used temporary vascular access for hemodialysis, is associated with a variety of complications. This retrospective study was done on 96 patients who had 150 percutaneous subclavian vein catheterizations over a one year period using Quinton double lumen polyurethane catheters. A total of 56 complications were seen of which 12 were immediate and 44 delayed. Immediate complications seen were six cases of failure of insertion of catheter and three cases each of subclavian artery puncture and inadvertent entry of the catheter into the jugular vein.

Anaesthesia for transthoracic endoscopic sympathectomy in the treatment of upper limb hyperhidrosis.

Renewed interest has been shown in transthoracic endoscopic sympathectomy (TES) for the treatment of upper limb hyperhidrosis. We review our experience and discuss the anaesthetic technique and perioperative problems encountered in 58 patients undergoing TES for hyperhidrosis. Patients were monitored for arterial pressure, heart rate, ECG, pulse oximetry (SpO2), end-tidal carbon dioxide concentration, peak inspired airway pressure and skin temperature. General anaesthesia, with a double-lumen endobronchial tube, enabled the lungs to be collapsed alternately, thereby ensuring easy and clear acc

Delayed presentation of congenital diaphragmatic hernia.

Eight patients aged 1 month up to nine years with congenital diaphragmatic hernias (seven left sided postero-lateral, one right-sided antero-medial), who presented outside of the neonatal period, are reported, four are described in detail. Radiographic presentation was obvious in four patients, simulated inflammatory lung disease in one and pneumothorax in two. In one patient a Morgagni hernia was primarily missed. Two had previous normal chest X-rays. All cases with herniated bowel showed "connecting" bowel segments passing through the diaphragmatic defect. Primarily the lack of awareness of

[One year's experience with mechanical ventilation in newborn infants (author's transl)].

During the first year of newborn intensive care at the Landeskinderkrankenhaus Linz/Donau there were 80 cases of artificial ventilation, 62 of these were Low Birth Weight Infants (LBWI). 12% of all LBWI required mechanical ventilation. 53 ventilated infants suffered from severe Hyalin Membrane Syndrome (HMS). 17 survived, 18 died of intraventricular hemorrhage and 18 died of HMS. Bronchopulmonary Dysplasia was found in all patients ventilated for more than 7 days. 5 cases of pneumothorax occurred during ventilation. Good results were obtained in cases of respiratory insufficiency other than HM

[Expert consensus on the diagnosis and management of Birt-Hogg-Dubé syndrome].

Birt-Hogg-Dubé syndrome (BHD) is a rare autosomal dominant disorder characterized by diffuse pulmonary cysts often leading to recurrent spontaneous pneumothorax, cutaneous fibrofolliculomas or trichodiscomas, and a variety of renal cell cancers. It is caused by pathogenic variants in the FLCN gene located on chromosome 17p11.2. Although an increasing number of patients with BHD syndrome are being recognized in China, the missed diagnosis and delayed diagnosis are still common. In addition, appropriate management is difficult for most of them. Pulmonary cysts and pneumothorax are the main prese

An Uncommon Cause of Massive Hemothorax: Pulmonary Arteriovenous Malformation in the Right Middle Lobe.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies that can lead to severe complications when ruptured. We report the case of a 65-year-old active smoker who presented to the emergency department with acute pleuritic chest pain, severe dyspnea, and hypoxemia. Chest CT revealed a large PAVM in the right middle lobe, associated with massive hemothorax and a small pneumothorax. The patient had no history of hereditary hemorrhagic telangiectasia. Initial management included pleural drainage and endovascular embolization with a 16 mm Amplatzer™ device. Due to persistent respi

Pneumomediastinum and Subcutaneous Emphysema as Initial Presentations of Pneumocystis Jirovecii Pneumonia in a Newly Diagnosed HIV-Infected Patient: A Case Report.

Pneumocystis jirovecii pneumonia (PJP) is a life-threatening opportunistic infection that primarily affects immunocompromised individuals, particularly those with acquired immunodeficiency syndrome caused by human immunodeficiency virus (HIV). The clinical presentation of PJP can be atypical, with symptoms such as spontaneous pneumothorax, pneumomediastinum, and subcutaneous emphysema, complicating diagnosis. In this report, we present a case of a 31-year-old male pharmacist with newly diagnosed HIV and PJP, who presented with persistent cough, dyspnea, intermittent fever, and subcutaneous emp

Inadvertent intrapleural placement of a thoracic epidural catheter identified during robotic-assisted video-assisted thoracoscopic surgery (VATS).

BACKGROUND: Thoracic epidural analgesia is widely considered the gold standard for perioperative pain control in thoracic surgery. Despite its benefits, placement is technically challenging and can result in rare complications such as catheter misplacement into the intrapleural space. CASE PRESENTATION: This case report will discuss a female patient in her late 70s with scoliosis and obesity who underwent robotic-assisted video-assisted thoracoscopic surgery for a pulmonary mass. The thoracic epidural catheter placed preoperatively was later directly visualized in the intrapleural space durin

Traumatic Diaphragmatic Injuries.

Diaphragmatic injuries are uncommon but are associated with high mortality rates in acute and chronic presentations. Mortality increases with complications of diaphragmatic hernia, which include strangulated bowel, perforation, and peritonitis. Traumatic diaphragmatic injury (TDI) is notoriously underdiagnosed and delayed presentations are common. Firearm injuries and stab wounds account for the majority of TDI. Blunt TDI is frequently associated with severe injuries including pneumothorax and hemorrhaging shock. Contrast enhanced computed tomography imaging is the gold standard for diagnosis

Severe Respiratory Syncytial Virus Bronchiolitis Complicated by Bilateral Massive Pleural Effusions in a Previously Healthy Infant: A Case Report.

Respiratory syncytial virus (RSV) bronchiolitis is one of the leading causes of hospitalization in infants, typically presenting with mild-to-moderate respiratory distress. Severe complications such as apnea, respiratory failure, and pneumothorax are known, but massive pleural effusions remain an exceptionally rare manifestation. We report a case of an eight-week-old previously healthy Saudi female infant presenting with severe RSV bronchiolitis. The patient developed progressive respiratory failure requiring mechanical ventilation, re-intubation, and circulatory support. Notably, she develope

Robotic repair of an esophago-left main bronchial fistula using a pedicled autologous pericardial patch and latissimus dorsi muscle flap: a novel technique after chemoradiotherapy for cT4b esophageal cancer.

Once an esophago-airway fistula develops during chemoradiotherapy for cT4b esophageal cancer, further curative treatment is often precluded. We describe a robotic technique for closure of a large esophago-left main bronchial fistula using a pedicled autologous pericardial patch reinforced with a latissimus dorsi muscle flap. A 66-year-old woman with squamous cell carcinoma of the middle to upper thoracic esophagus invading the left main bronchus developed a fistula after definitive chemoradiotherapy. After temporary airway stenting, salvage surgery was performed in the semi-prone position unde

Case Series of Blowhole Creation with or without Negative Pressure Wound Therapy for Severe Subcutaneous Emphysema.

Severe subcutaneous emphysema that is refractory to chest tube drainage can result in significant patient discomfort, airway compromise, and hemodynamic instability. Various interventional approaches, including subcutaneous drain insertion and the blowhole technique, with or without negative pressure wound therapy (NPWT), have been proposed to manage this condition. In this case series, we describe 10 patients who developed severe subcutaneous emphysema following surgery or pneumothorax and were treated using the blowhole technique, with or without NPWT. A Wound Protector/Retractor XXS or LapP

Case Report: Empyema secondary to percutaneous transthoracic needle biopsy in three cases.

Percutaneous transthoracic needle biopsy (PTNB) is a widely utilized diagnostic procedure for pulmonary lesions, with the current literature predominantly documenting pneumothorax and hemorrhage as primary complications. While empyema represents a rare complication, its clinical implications warrant special attention. In this study, we report the cases of three patients with unidentified pulmonary masses who developed empyema after PTNB. All cases exhibited fever (24-48 h post-procedure) and radiographic evidence of pleural effusion progression shortly after the procedure, which was successful

Pulmonary hemorrhage and hemopneumothorax following the administration of lenvatinib for lung metastasis from hepatocellular carcinoma.

A 71-year-old man was started on 12 mg/day of lenvatinib for lung metastasis from hepatocellular carcinoma (HCC). The day after starting treatment, he developed hemoptysis, and a CT scan revealed a rupture of the left upper lobe metastasis, accompanied by pulmonary hemorrhage. Angiography was performed, and the responsible blood vessels, the bronchial artery and the left pulmonary artery (A1+2ab), were embolized. One week later, the patient developed a left tension pneumothorax. Despite thoracic drainage, air leakage and bloody drainage persisted. We decided to perform a left upper lobe resect

A Rare Case of Cervical Subcutaneous Emphysema Following Swimming.

Subcutaneous cervical emphysema refers to the presence of air or gas trapped beneath the skin in the neck or cervical region. It typically occurs when air is introduced into the tissues due to trauma, infection, or medical procedures that disrupt the respiratory tract. Diagnosis is based on clinical presentation in addition to chest and neck radiographic examinations. Subcutaneous cervical emphysema resulting from swimming is extremely rare, and patients may be asymptomatic or present with crepitus, neck swelling, and dysphonia. In severe cases, life-threatening complications can occur, such a
