Simulation modeling of neighborhood-level differences in life course health outcomes: a proof of concept.

BACKGROUND AND OBJECTIVES: Adults in low-resource neighborhoods face greater burden of preventable diseases than the general population. Methodologies to measure the impact of population-level efforts to reduce neighborhood-level disparities in health outcomes are lacking. We introduce a simulation modeling approach for incident disease and mortality risk over the life course and apply the model to assess the impact of a hypothetical population health initiative to reduce neighborhood-level disparities. RESEARCH DESIGN AND METHODS: Using electronic health records, we constructed a probabilist

Political Intrusions into the International Health Regulations Treaty and Its Impact on Management of Rapidly Emerging Zoonotic Pandemics: What History Tells Us.

For a large number of health care providers world-wide, the coronavirus disease 2019 (COVID-19) pandemic is their first experience in population-based care. In past decades, lower population densities, infectious disease outbreaks, epidemics, and pandemics were rare and driven almost exclusively by natural disasters, predatory animals, and war. In the early 1900s, Sir William Osler first advanced the knowledge of zoonotic diseases that are spread from reservoir animals to human animals. Once rare, they now make up 71% or more of new diseases. Globally, zoonotic spread occurs for many reasons.

Use of Hypofractionation Among Underserved Populations Undergoing Radiation Therapy for Cancer: Implications for Policy.

PURPOSE: To examine the use of hypofractionation among underserved populations, with a focus on implications for policy. METHODS AND MATERIALS: Using a 20% Medicare claims sample, we evaluated the receipt of hypofractionation, a short-course radiation therapy approach, among underserved versus nonunderserved populations. Hypofractionation was defined based on cancer/site-specific guidelines and expert review. Underserved groups included racially/ethnically minoritized, rural, and patients with low socioeconomic status. We included 4 cancer types that represent the majority of new cancer diagn

Acceptance of the COVID-19 Booster Vaccine Amid the Omicron JN.1 Surge in Southern Bangladesh: A Comparative Analysis of Rural-Urban Disparities.

Rapid spread of SARS-CoV-2 variants, including Omicron JN.1, has revived significant challenges to health systems and regenerates microbial risk to public health security. Although COVID-19 booster vaccines have ensured enhanced protection against new variants, there remain persistent disparities in vaccine acceptance and uptake between rural and urban populations. This study assesses COVID-19 booster vaccine acceptance and compares antecedents of acceptance across rural and urban communities in Southern Bangladesh. A cross-sectional observational study was conducted between December 15, 2023,

Pre-exposure and postexposure prophylaxis access in rural versus urban pharmacies in Georgia and Pennsylvania.

BACKGROUND: Pre-exposure prophylaxis (PrEP) and postexposure prophylaxis (PEP) prevent HIV among individuals at high risk for acquisition. Pre-existing structural barriers to PrEP/PEP access among rural patients may be exacerbated further if pharmacies do not keep PrEP/PEP in stock, constituting a significant barrier to mitigating the HIV epidemic. OBJECTIVES: To compare PrEP/PEP availability for same-day pickup in rural vs urban Georgia and Pennsylvania pharmacies. METHODS: We conducted a cross-sectional simulated patient caller study, calling pharmacists in Georgia and Pennsylvania to see

Associations of Serum Folate and Holotranscobalamin with Cardiometabolic Risk Factors in Rural and Urban Cameroon.

A low intake of fruit and vegetables and a high intake of meat are associated with higher cardiometabolic disease risk; however much prior research has relied on subjective methods for dietary assessment and focused on Western populations. We aimed to investigate the association of blood folate as an objective marker of fruit and vegetable intake and holotranscobalamin (holoTC) as a marker of animal-sourced food intake with cardiometabolic risk factors. We conducted a population-based cross-sectional study on 578 adults (mean ± SD age = 38.2 ± 8.6 years; 64% women). The primary outcome was a c

The aging population: demographics and the biology of aging.

Epidemiologic studies show that 11% of the world's population is over 60 years of age; this is projected to increase, by 2050, to 22% of the population. Oral aging is a current focus of several organizations including the Federation Dentaire Internationale, the World Health Organization and the American and Japanese Dental Associations. In their Tokyo Declaration, the Japanese Association identified the elderly population as one of its main target groups. One of the WHO goals is for each person to retain more than 20 teeth by age 80, despite the fact that the prevalence of periodontal disease

The psychosocial characteristics and needs of patients presenting with orofacial injury.

Individuals with orofacial injury presenting to urban trauma centers in the United States tend to be disproportionately socioeconomically disadvantaged, young, adult, ethnic minority men. Most injuries are assaultive in origin, suggesting poor impulse control and maladaptive social behaviors. Compared with matched control populations, patients with orofacial injuries are more likely to report higher levels of substance use behaviors and to manifest greater levels of hostility, anxiety, and depression. Although they have significantly greater current and lifetime need for mental health service
