Retropleural Thoracic Approach.

The lateral retropleural thoracic approach offers minimally invasive access for the treatment of thoracic spine pathology, specifically thoracic herniated discs. Alternatives to the retropleural approach traditionally included posterolateral or anterior approaches, which carry increased morbidity. The retropleural approach affords lateral access to the thoracic spine that allows for addressing pathology such as herniated discs, corpectomy, tumor, or trauma. This article outlines preoperative workup and planning, intraoperative steps, tips, and postoperative care.

[Gastroschisis repair under caudal anesthesia: a series of three cases].

Gastroschisis is a congenital anomaly characterized by a defect in the anterior abdominal wall with protrusion of abdominal viscera. Perioperative mortality is very high in these patients. Traditionally gastroschisis repair has been performed under general anesthesia with endotracheal intubation, requiring postoperative intensive care admission and mechanical ventilation. Caudal block is an attractive alternative to general anesthesia. We present a series of three neonates with gastroschisis, repaired solely under caudal anesthesia.

Brachycephalic Syndrome.

Animals presenting with brachycephalic syndrome suffer from multilevel obstruction of the airways as well as secondary structural collapse. Stenotic nares, aberrant turbinates, nasopharyngeal collapse, soft palate elongation and hyperplasia, laryngeal collapse, and left bronchus collapse are being described as the most common associated anomalies. Rhinoplasty and palatoplasty as well as newer surgical techniques and postoperative care strategies have resulted in significant improvement of the prognosis even in middle-aged dogs.

[Community-acquired bacterial septic arthritis in adults: diagnosis and treatment].

The diagnosis of acute native joint bacterial infection can be difficult, because of its non- specific clinical and biological manifestation. Its management is often an emergency. Following a joint puncture, early joint lavage is performed, either by surgical drainage or by repeated arthrocentesis; and accompanied by systemic antibiotics, of which the ideal duration and route of administration remains unknown. The postoperative care is characterized by joint mobilization to avoid joint stiffening.

[Common fractures in a general practice: indication for operations and modern operative technique].

Distal radius, clavicle, malleolar and proximal humerus fractures belong to the most frequent fracture types in every day practice. The initial diagnose with conservative therapy or the postoperative care is often carried out by the general practitioner. In recent years, therapy modality of these fractures changed. With the development of angular stable implants and minimal invasive operations technique, operative treatment is favoured mainly in osteoporotic bone. The mentioned fractures are highlighted.

Nurse-controlled analgesia using a patient-controlled analgesia device: an alternative strategy in the management of severe cancer pain in children.

Nurse-controlled analgesia (NCA) using a patient-controlled analgesia (PCA) device has been described for intensive care and postoperative use. This report describes the effective use of this technique for severe episodic and procedural pain in four children with advanced malignancy and high opioid requirements where conventional parenteral analgesia had been inadequate. Both morphine and fentanyl were used. Average duration of NCA was 6.75 days (range 4-12).

Meningiomas of the cerebellopontine angle and foramen magnum.

The most commonly occurring meningiomas in the posterior cranial fossa are located along the posterior surface of the temporal bone in the region of the cerebellopontine angle. Topics discussed in this article include clinical features, radiologic findings, surgical techniques, postoperative care, alternative treatments, and meningiomas of the foramen magnum. The author underscores the need for early diagnosis of these lesions so that surgical intervention can result in optimal chances for recovery.

A war injury to the thoracic trachea.

The authors present a case of war injury to the thoracic trachea with the defect involving more than half of its circumference. After minor difficulties in establishing the right diagnosis, we passed to the surgical procedure and managed the lesion by simple suture. The patient's postoperative course at the Intensive Care Unit was uneventful. Both early and late follow-up roentgenograms, computerized tomography of the trachea and bronchoscopy showed normal findings.

Sickle cell anemia and mitral valve replacement. Case report.

An 8-year-old black boy with sickle cell disease and severe hemolytic anemia crisis (95% hemoglobin S) also had mitral incompetence due to rheumatic valve disease. A 27 mm monostrut Björk-Shiley valve prosthesis was implanted after partial exchange transfusions had reduced the hemoglobin S to less than 40%. High-flow normothermic perfusion was used during extracorporeal circulation, with care taken to avoid hypoxia and acidosis. Postoperative recovery was uneventful.

The transoral surgical approach to craniovertebral pathology.

The transoral approach is a surgical technique that can be used effectively in the treatment of anterior craniovertebral junction (CVJ) pathology. In this review, we outline the signs, symptoms, and diagnostic studies that identify the pathology and help to determine subsequent therapy. The transoral approach, preoperative and postoperative management problems, and potential complications of the procedure are reviewed. The nursing care of a patient with CVJ pathology is discussed.

Epidermolysis bullosa and associated problems in oral surgical treatment.

The problems encountered in the anesthetic and oral surgical management of patients with epidermolysis bullosa are many and varied and are always challenging. Two patients with the disease, from the same family, underwent complete odontectomies. Exacerbation of the disease was prevented by hospitalization with meticulous preparation and exacting postoperative care. This is the first report in the dental literature of complete odontectomies performed on two afficted patients from one family.

Postoperative myocardial ischemia in patients undergoing coronary artery bypass graft surgery. S.P.I. Research Group.

Cardiac morbidity and mortality after coronary artery bypass graft (CABG) surgery continue to be significant problems. To determine the prevalence, characteristics, and prognostic importance of postoperative myocardial ischemia after CABG surgery, the authors monitored 50 patients continuously for 10 perioperative days with the use of two-lead electrocardiography (ECG). ECG changes consistent with ischemia were defined as a reversible ST depression of 1 mm or greater or an elevation of 2 mm or greater from baseline, lasting at least 1 min. Baseline was adjusted for positional changes and tempo

Pain management after open colorectal surgery: An update of the systematic review and procedure-specific postoperative pain management (PROSPECT) recommendations.

BACKGROUND: Open colectomy is still performed around the world and associated with significant postoperative pain. OBJECTIVES: Unpublished recommendations based on a systematic review were proposed by the PROcedure SPECific postoperative pain managemenT (PROSPECT) group in 2016. We aimed to update these recommendations by evaluating the available literature and develop recommendations for optimal pain management after open colectomy according to the PROSPECT methodology. DESIGN AND DATA SOURCES: A systematic review using the PROSPECT methodology was undertaken. Randomised controlled trials a

Heparin-Induced Thrombocytopenia in Patients Undergoing Valvular and Aortic Surgery: A Modern Assessment of Risk.

OBJECTIVE: The incidence and outcomes of patients with heparin-induced thrombocytopenia (HIT) are well defined for general cardiac surgical populations. The purpose of this study was to define the outcomes of patients with HIT in a population excluding patients who underwent coronary artery bypass grafting (CABG). METHODS: The local Society of Thoracic Surgeons cardiac surgical database was queried between January 2008 and May 2017 for patients who underwent either open valvular surgery or aortic surgery. Patients who underwent either isolated or combined CABG procedures were excluded. Cohort

Yield of early postoperative computed tomography after frontal ventriculoperitoneal shunt placement.

Despite being widely used, ventriculoperitoneal (VP) shunt placement is a procedure often associated with complications and revision surgeries. Many neurosurgical centers routinely perform early postoperative cranial computer tomography (CT) to detect postoperative complications (e.g., catheter malposition, postoperative bleed, over-drainage). Because guidelines are lacking, our study aimed to evaluate the yield of early routine postoperative CT after shunt placement for adult hydrocephalus. We retrospectively reviewed 173 patients who underwent frontal VP shunting for various neurosurgical co

Early postoperative hemorrhage after first rib resection for vascular thoracic outlet syndrome.

BACKGROUND: Thrombosis and embolization are the most frequent complications associated with the vascular presentation of thoracic outlet syndrome (VTOS). Therefore, surgery for these conditions requires careful balancing of anticoagulation and hemostasis. Our goal is to identify the optimal postoperative anticoagulation management of these patients. METHODS: A prospective database of consecutive patients who have presented to our institution with the diagnosis of thoracic outlet syndrome was reviewed from 1996 through 2010 for instances of postoperative hemorrhage. All venous cases were manag

Postoperative chest radiograph: II. Alterations after major intrathoracic surgery.

Major radiographic changes almost always occur after surgery for pulmonary resection and adult acquired heart disease. With careful attention to the timing and clinical setting of these alterations, their significance can be evaluated and their management planned. This paper reviews the expected postoperative changes as well as those indicating a potential complication of surgery.

Improvements in Social Quality of Life for Recovering Addicts Receiving Dental Implant Rehabilitation.

Substance use disorders (SUDs) are associated with severe oral deterioration, chronic systemic inflammation, and elevated depression risk, yet definitive dental rehabilitation is rarely integrated into long-term recovery care. This IRB-approved retrospective cohort study evaluated the effect of full-arch implant rehabilitation on depressive symptoms in 101 consecutively treated adults with verified long-term recovery from SUDs. Patients completed a modified PHQ-9 at baseline and again at final prosthesis delivery. Mean scores decreased from 24.16 preoperatively to 0.75 postoperatively, an 86.7
