Real-World External Validation of Artificial Intelligence-Based Full-Vessel Segmentation for Intracoronary Optical Coherence Tomography.

BACKGROUND: Artificial intelligence (AI) allows automated evaluation of intracoronary optical coherence tomography (OCT) images. However, algorithms are mostly developed and validated on well-curated data sets, which may not represent real-world data. We sought to externally validate a previously developed algorithm performing full-vessel segmentation for OCT in an unselected consecutive real-world data set. METHODS: This was a retrospective, single-center, external validation study comprising 100 consecutive patients undergoing clinically indicated OCT. A previously developed AI algorithm (O

Air travel and cystic fibrosis: An algorithm to assess the risk of In-Flight Hypoxemia.

BACKGROUND: Air travel may cause significant hypoxemia in patients with cystic fibrosis (CF). A pre-flight algorithm has previously been validated for patients with chronic obstructive pulmonary disease (COPD). No such tools are available for CF patients. The aim of this study was to evaluate if the pre-flight algorithm for COPD patients can be used by CF patients. METHODS: In this prospective cross-sectional study, oxygen saturation at sea level (SpO2 SL) and during exercise (SpO2 6MWT) were used to evaluate whether CF patients a) are fit for flight without further assessment, b) require in-

Estimation of Ganciclovir Exposure in Adults Transplant Patients by Machine Learning.

INTRODUCTION: Valganciclovir, a prodrug of ganciclovir (GCV), is used to prevent cytomegalovirus infection after transplantation, with doses adjusted based on creatinine clearance (CrCL) to target GCV AUC0-24 h of 40-60 mg*h/L. This sometimes leads to overexposure or underexposure. This study aimed to train, test and validate machine learning (ML) algorithms for accurate GCV AUC0-24 h estimation in solid organ transplantation. METHODS: We simulated patients for different dosing regimen (900 mg/24 h, 450 mg/24 h, 450 mg/48 h, 450 mg/72 h) using two literature population pharmacokinetic models,

Next-generation chemotherapy treatments based on black hole algorithms: From cancer remission to chronic disease management.

PROBLEM: Therapeutic planning strategies have been developed to enhance the effectiveness of cancer drugs. Nevertheless, their performance is highly limited by the inefficient biological representativeness of predictive tumor growth models, which hinders their translation to clinical practice. OBJECTIVE: This study proposes a disruptive approach to oncology based on nature-inspired control using realistic Black Hole physical laws, in which tumor masses are trapped to experience attraction dynamics on their path to complete remission or to become a chronic disease. This control method is desig

Early Diagnosing and Transformation Prediction of Alzheimer's Disease Using Multi-Scaled Self-Attention Network on Structural MRI Images with Occlusion Sensitivity Analysis.

BACKGROUND: Structural magnetic resonance imaging (sMRI) is vital for early Alzheimer's disease (AD) diagnosis, though confirming specific biomarkers remains challenging. Our proposed Multi-Scale Self-Attention Network (MUSAN) enhances classification of cognitively normal (CN) and AD individuals, distinguishing stable (sMCI) from progressive mild cognitive impairment (pMCI). OBJECTIVE: This study leverages AD structural atrophy properties to achieve precise AD classification, combining different scales of brain region features. The ultimate goal is an interpretable algorithm for this method.

Performance and clinical utility of an artificial intelligence-enabled tool for pulmonary embolism detection.

PURPOSE: Diagnosing pulmonary embolism (PE) is still challenging due to other conditions that can mimic its appearance, leading to incomplete or delayed management and several inter-observer variabilities. This study evaluated the performance and clinical utility of an artificial intelligence (AI)-based application designed to assist clinicians in the detection of PE on CT pulmonary angiography (CTPA). PATIENTS AND METHODS: CTPAs from 230 US cities acquired on 57 scanner models from 6 different vendors were retrospectively collected. Three US board certified expert radiologists defined the gr

Artificial intelligence for detection of ventricular oversensing: Machine learning approaches for noise detection within nonsustained ventricular tachycardia episodes remotely transmitted by pacemakers and implantable cardioverter-defibrillators.

BACKGROUND: Pacemakers (PMs) and implantable cardioverter-defibrillators (ICDs) increasingly automatically record and remotely transmit nonsustained ventricular tachycardia (NSVT) episodes, which may reveal ventricular oversensing. OBJECTIVES: We aimed to develop and validate a machine learning algorithm that accurately classifies NSVT episodes transmitted by PMs and ICDs in order to lighten health care workload burden and improve patient safety. METHODS: PMs or ICDs (Boston Scientific, St Paul, MN) from 4 French hospitals with ≥1 transmitted NSVT episode were split into 3 subgroups: trainin

Nondestructive testing and visualization of compound heavy metals in lettuce leaves using fluorescence hyperspectral imaging.

This study evaluated the feasibility of nondestructive testing and visualization of compound heavy metals (cadmium and lead) in lettuce leaves using fluorescence hyperspectral imaging. In addition, a method involving wavelet transform and stepwise regression (WT-SR) was proposed to perform dimensionality reduction of fluorescence spectral data. Fluorescent hyperspectral image acquisition and mathematical analysis were carried out on lettuce leaf samples processed with different compound heavy metal concentrations. The entire lettuce leaf sample was selected as a region of interest (ROI). Savit

Revisiting a Prognosticating Algorithm from Cardiopulmonary Exercise Testing in Chronic Heart Failure (from the MECKI Score Population).

Cardiopulmonary exercise testing is a prognostic tool in heart failure with reduced left ventricular ejection fraction (HFrEF). Prognosticating algorithms have been proposed, but none has been validated. In 2017, a predictive algorithm, based on peak oxygen consumption (VO2), ventilatory response to exercise (ventilation [VE] carbon dioxide production [VCO2], the VE/VCO2 slope), exertional oscillatory ventilation (EOV), and peak respiratory exchange ratio, was recommended, according treatment with β blockers: patients with HFrEF registered in the metabolic exercise test data combined with card

Development of a neural network based control algorithm for powered ankle prosthesis.

Lower limb amputation is partial or complete removal of the limb due to disease, accident or trauma. Surface electromyograms (sEMG) of a large number of muscles and force sensors have been used to develop control algorithms for lower limb powered prostheses, but there are no commercial sEMG controlled prostheses available to date. Unlike ankle disarticulation, transtibial amputation yields less intact lower leg muscle mass. Therefore, minimizing the use of sEMG muscle sources utilized will make powered prosthesis controller economic, and limiting the use of specifically the lower leg muscles w

Maternal metabolic profiling to assess fetal gestational age and predict preterm delivery: a two-centre retrospective cohort study in the US.

OBJECTIVES: The aim of this study was to develop a single blood test that could determine gestational age and estimate the risk of preterm birth by measuring serum metabolites. We hypothesised that serial metabolic modelling of serum analytes throughout pregnancy could be used to describe fetal gestational age and project preterm birth with a high degree of precision. STUDY DESIGN: A retrospective cohort study. SETTING: Two medical centres from the USA. PARTICIPANTS: Thirty-six patients (20 full-term, 16 preterm) enrolled at Stanford University were used to develop gestational age and prete

Moderate sensitivity and high specificity of emergency department administrative data for transient ischemic attacks.

BACKGROUND: Validation of administrative data case definitions is key for accurate passive surveillance of disease. Transient ischemic attack (TIA) is a condition primarily managed in the emergency department. However, prior validation studies have focused on data after inpatient hospitalization. We aimed to determine the validity of the Canadian 10th International Classification of Diseases (ICD-10-CA) codes for TIA in the national ambulatory administrative database. METHODS: We performed a diagnostic accuracy study of four ICD-10-CA case definition algorithms for TIA in the emergency depart

Medicaid patients at high risk for frequent hospital admission: real-time identification and remediable risks.

Patients with frequent hospitalizations generate a disproportionate share of hospital visits and costs. Accurate determination of patients who might benefit from interventions is challenging: most patients with frequent admissions in 1 year would not continue to have them in the next. Our objective was to employ a validated regression algorithm to case-find Medicaid patients at high-risk for hospitalization in the next 12 months and identify intervention-amenable characteristics to reduce hospitalization risk. We obtained encounter data for 36,457 Medicaid patients with any visit to an urban p

Efficient reconstruction of haplotype structure via perfect phylogeny.

Each person's genome contains two copies of each chromosome, one inherited from the father and the other from the mother. A person's genotype specifies the pair of bases at each site, but does not specify which base occurs on which chromosome. The sequence of each chromosome separately is called a haplotype. The determination of the haplotypes within a population is essential for understanding genetic variation and the inheritance of complex diseases. The haplotype mapping project, a successor to the human genome project, seeks to determine the common haplotypes in the human population. Since
