Investigating the risk of maternal-fetal transmission of SARS-CoV-2 in early pregnancy.

INTRODUCTION: The possibility of vertical transmission of SARS-CoV-2 from the mother to the fetus is one of the most crucial issues regarding the COVID-19 effects on pregnancy. In this study, we aimed to explore the risk of maternal-fetal transmission before 24 weeks of gestation, through analysis of abortion materials collected from PCR-positive women with pregnancy loss. To the best of our knowledge, apart from case reports, this study is the first prospective work on the vertical transmission of SARS-CoV-2 in early pregnancy. METHODS: The patients who had attended our clinic with the diagn

Early pregnancy vitamin D and the risk of adverse maternal and infant outcomes: a retrospective cohort study.

BACKGROUND: Previous evidence has suggested that lower gestational vitamin D levels might increase the risks of adverse pregnancy and birth outcomes. The results remain inconsistent and require further exploration. METHODS: A total of 2814 Chinese mother-infant pairs were included in this retrospective cohort study. Serum concentrations of 25(OH)D were reviewed in early pregnancy (16.3 ± 2.3 weeks). Outcomes of maternal gestational diabetes mellitus (GDM), cesarean section, fetal distress, preterm birth, low birth weight (LBW), and macrosomia were extracted from the medical records. Cox regre

Systematic Review of Tularemia During Pregnancy.

BACKGROUND: Tularemia is caused by the gram-negative bacterium Francisella tularensis. Although rare, tularemia during pregnancy has been associated with pregnancy complications; data on efficacy of recommended antimicrobials for treatment are limited. We performed a systematic literature review to characterize clinical manifestations of tularemia during pregnancy and examine maternal, fetal, and neonatal outcomes with and without antimicrobial treatment. METHODS: We searched 9 databases, including Medline, Embase, Global Health, and PubMed Central, using terms related to tularemia and pregna

Factors associated with early obstetric ultrasound uptake at a tertiary hospital in Northern Uganda: a cross-sectional study.

BACKGROUND: Early obstetric ultrasound improves gestational age estimation, detection of fetal anomalies and management of pregnancy complications. The WHO recommends at least one ultrasound examination before 24 weeks of gestation. However, uptake of early obstetric ultrasound in many low-resource settings remains poorly documented, including in post-conflict Northern Uganda. This study assessed the prevalence and factors associated with uptake of obstetric ultrasound before 24 weeks of gestation among pregnant women attending antenatal care at a tertiary hospital in Northern Uganda. METHODS

Metabolites in Early-Mid Pregnancy Mediate the Association Between Prepregnancy Body Mass Index and Risk of Gestational Diabetes Mellitus.

AIMS: The study is aimed at identifying the shared metabolites in early-mid pregnancy associated with prepregnancy body mass index (pBMI) and subsequent gestational diabetes mellitus (GDM) risk and at exploring the mediating role of metabolites. METHODS: One hundred pregnant women with GDM and 100 matched controls were enrolled in the study. Serum samples were collected in 10-20 weeks' gestation and used for targeted metabolomic assay measurement. The associations among pBMI, metabolites, and GDM were investigated using linear regression and logistic regression models. Mediation analysis was

Predicting down syndrome: a comparative evaluation of nasal bone length in mid-trimester pregnancy.

BACKGROUND: This study aimed to evaluate the diagnostic performance of fetal nasal bone length (NBL), the biparietal diameter to nasal bone length ratio (BPD/NBL), and nasal bone length multiples of the median (NBL-MoM) values for Down syndrome (DS) screening between 17 and 25 weeks of gestation in a population-specific cohort. METHODS: A retrospective analysis was conducted on fetuses between 17 and 25 gestational weeks. Normative data for NBL and NBL-MoM were generated using gestational age–specific reference medians, and regression equations were established. The NBL, BPD/NBL ratio, and NBL

Second Trimester Abortion: A Dilation and Evacuation Simulation for Gynecologic Surgery and Obstetrics Residents.

INTRODUCTION: The future of training in second trimester surgical abortions with dilation and evacuation (D&E) procedures faces ongoing legal and political scrutiny; thus, adjuncts to standard clinical experiences are exceedingly important. We sought to build medical trainees' surgical familiarity with D&Es using a realistic simulation model. METHODS: The simulation began with an instructional video reviewing accessible and affordable materials used to build the fetal model (vaginal swabs, styrofoam ball, and putty) and the uterine model (collapsible water bottle). Required personnel roles in

Fetal hemodynamic changes and mitochondrial dysfunction in myocardium and brain tissues in response to anemia: a lesson from hemoglobin Bart's disease.

OBJECTIVE: Whether or not the effects of anemia in the early phase, while the fetuses attempts to increase cardiac output to meet oxygen requirement in peripheral organs, is detrimental to the fetal developing vital organs is little-known. The objective of this is to compare prenatal cardiovascular changes and post-abortal cellular damages in the myocardium as a pumping organ and the brain as a perfused organ between anemic fetuses (using fetal Hb Bart's disease as a study model) in pre-hydropic phase and non-anemic fetuses. METHODS: Fetuses affected by Hb Bart's disease and non-anemic fetuse

Prenatal diagnosis of a de novo tetrasomy 15q24.3-25.3: Case report and literature review.

BACKGROUND: Terminal duplication on chromosome 15q is a rare chromosomal variation. Affected individuals show similar features such as growth dysplasia or the development of frontal bossing, body deformities, facial abnormalities, and genitourinary or cardiovascular disorders. However, it is not yet clear whether such 15q repeats lead to identifiable patterns of clinical abnormalities. Therefore, the purpose of this study was to analyze the prenatal diagnostic results and clinical manifestations of a fetus with 15q duplication and to summarize the literature. METHODS: The case was a fetus at

Apert syndrome: prenatal diagnosis challenge.

Apert syndrome is a rare genetic disorder that manifests as craniosynostosis, craniofacial and limb dysmorphic features. Mutations in fibroblast growth factor receptor 2 (FGFR2) gene account for almost all cases. Given the impact it can have throughout life, prenatal management becomes a challenge. A healthy 33-year-old woman, gravida 4, para 0, was referred to routine ultrasound at 22 weeks of gestation. Atypical cranial morphology with prominent forehead, ocular proptosis, hypertelorism and mitten hands were detected. Genetic investigation revealed an FGFR2 gene mutation (c.755C>G(p.Ser252Tr
