Comparative outcomes of open and laparoscopic Kasai portoenterostomy for biliary Atresia patients progressing to liver transplantation: A Real-World cohort study.

PURPOSE: We compared the long-term outcomes of open (OKPE) versus laparoscopic Kasai portoenterostomy (LKPE) for children with biliary atresia (BA) who ultimately progressed to liver transplantation (LT). METHODS: The subjects of this retrospective multicenter analysis were 276 BA patients (OKPE:225, LKPE:51) undergoing living donor liver transplantation (LDLT) between 2021 and 2023. Propensity score matching (PSM, 51 pairs) and inverse probability treatment weighting (IPTW) balanced the confounders. The time from KPE to LDLT was assessed using Kaplan-Meier curves and Cox regression analysis.

Risk of rhabdomyolysis in patients with mental disorders.

OBJECTIVES: Rhabdomyolysis is caused by rapid muscle breakdown. Although patients with mental disorders are more susceptible to rhabdomyolysis, its incidence in this group is poorly established. Therefore, we conducted a population-based cohort study to investigate its incidence and risk in this group. METHODS: We conducted a retrospective cohort study of 120,094 patients with mental disorders and 120,094 control patients from the Taiwan National Health Insurance Research Database. We excluded patients with rhabdomyolysis diagnosed before the index date, which was defined as the date of the f

Comparative outcomes of COVID-19 in patients with primary Sjögren's syndrome treated with hydroxychloroquine versus methotrexate: a retrospective cohort study.

BACKGROUND: People with rheumatic diseases are considered at risk for severe coronavirus disease 2019 (COVID-19) because of their underlying abnormal immune responses and frequent disease-modifying antirheumatic drug (DMARD) use. We aimed to explore COVID-19 outcomes in patients with primary Sjögren's syndrome (pSS) receiving different conventional synthetic DMARDs. METHODS: The study was a retrospective analysis of real data from the TriNetX platform between 1 January 2020 and 30 June 2023. We compared COVID-19 outcomes between cohorts of pSS patients treated with hydroxychloroquine (HCQ) an

Effect of preoperative cancer cachexia on postoperative sarcopenia in patients with non-small cell lung cancer.

OBJECTIVE: Many patients with lung cancer have cancer cachexia, which may result in complications and affect prognosis; however, its preoperative prevalence is unknown. Skeletal muscle mass loss after lung cancer surgery also affects prognosis. Although cachexia and sarcopenia share some similarities, whether preoperative cachexia affects postoperative sarcopenia is still unknown. Thus, we designed a cohort study of patients with non-small cell lung cancer (NSCLC) to investigate this. RESEARCH METHODS AND PROCEDURES: Patients with NSCLC who underwent surgery and perioperative rehabilitation w

The prognostic impact of net ultrafiltration intensity in critically ill patients receiving continuous renal replacement therapy: a multivariable and propensity-matched analysis.

OBJECTIVES: Net ultrafiltration (UFnet) is widely used for fluid management during continuous renal replacement therapy (CRRT) for critically ill patients over extended periods. Despite widespread use, the relationship between UFnet intensity and clinical outcomes, particularly mortality, remains controversial. METHODS: This retrospective observational study examined critically ill patients undergoing CRRT for more than 72 h from January 2021 to September 2023. Patients were stratified by their UFnet intensity during the initial 72 h of CRRT into low (<1.01 mL/kg/h), moderate (1.01-1.75 mL/kg

Glucagon-Like Peptide-1 Receptor Agonists Reduce Surgeries and Hospitalizations in Hidradenitis Suppurativa: A Multicenter TriNetX Cohort Study.

BACKGROUND: Hidradenitis suppurativa is a chronic inflammatory skin condition frequently associated with type 2 diabetes mellitus and obesity. Glucagon-like peptide-1 receptor agonists have emerged as promising agents for managing type 2 diabetes mellitus due to their potent effects on glycemic control and weight reduction. Recent evidence suggests that glucagon-like peptide-1 receptor agonists may also possess anti-inflammatory properties, potentially making them effective in treating inflammatory conditions, such as hidradenitis suppurativa. OBJECTIVE: To determine if glucagon-like peptide-

Optimal glycaemic control and the reduced risk of colorectal adenoma and cancer in patients with diabetes: a population-based cohort study.

OBJECTIVE: Whether varying degrees of glycaemic control impact colonic neoplasm risk in patients with diabetes mellitus (DM) remains uncertain. DESIGN: Patients with newly diagnosed DM were retrieved from 2005 to 2013. Optimal glycaemic control at baseline was defined as mean haemoglobin A1c (HbA1c)<7%. Outcomes of interest included colorectal cancer (CRC) and colonic adenoma development. We used propensity score (PS) matching with competing risk models to estimate subdistribution HRs (SHRs). We further analysed the combined effect of baseline and postbaseline glycaemic control based on time-
