Systemic-to-Pulmonary Collateral Flow Correlates with Clinical Condition Late After the Fontan Procedure.

In the Fontan circulation, there is a substantial degree of systemic-to-pulmonary collateral flow (SPCF), which can be measured by cardiac magnetic resonance (CMR). However, the correlation between the degree of SPCF and long-term outcomes is not fully understood. We retrospectively studied 321 patients who underwent the Fontan procedure and CMR at a single center. Using CMR, we calculated SPCF as pulmonary blood flow - systemic blood flow. %SPCF was defined as SPCF ÷ pulmonary blood flow. The mean age of patients at CMR was 14.3 ± 7.5 years. The average %SPCF was 13.0% ± 11.0%. With a multiva

The Left Pneumonectomy Combined with Monocrotaline or Sugen as a Model of Pulmonary Hypertension in Rats.

In this protocol, we detail the correct procedural steps and necessary precautions to successfully perform a left pneumonectomy and induce PAH in rats with the additional administration of monocrotaline (MCT) or SU5416 (Sugen). We also compare these two models to other PAH models commonly used in research. In the last few years, the focus of animal PAH models has moved towards studying the mechanism of angioproliferation of plexiform lesions, in which the role of increased pulmonary blood flow is considered as an important trigger in the development of severe pulmonary vascular remodeling. Pulmonary arteriovenous fistula in a rare location: The importance of excluding patent foramen ovale.

UNLABELLED: A 46-year-old woman with a history of repeated thromboembolic stroke and anti-phospholipid antibody syndrome was referred to our hospital. Saline contrast transthoracic echocardiography showed that microbubbles appeared in the left atrium within 4 heartbeats. Thus, she was initially suspected as having a patent foramen ovale with associated paradoxical embolism. However, no evidence of patent foramen ovale or atrial septal defect could be found using transesophageal echocardiography. Saline contrast transesophageal echocardiography showed that microbubbles flowed into the left atri

Surgical repair of partial anomalous pulmonary venous return with intact atrial septum in a 65-year-old woman: a case report.

BACKGROUND: Partial anomalous pulmonary venous return is a rare congenital cardiac anomaly that usually involves the right pulmonary vein and an atrial septal defect. Isolated partial anomalous pulmonary venous return with an intact atrial septum is even rarer, and this condition is usually treated surgically in younger patients. We describe isolated partial anomalous pulmonary venous return in a 65-year-old woman who was treated by caval division with pericardial patch baffling through a surgically created atrial septal defect and reconstruction of the superior vena cava using a prosthetic gr

Augmentation of pulmonary blood flow by a right atrial balloon pump after the Fontan operation.

The Fontan principle of right ventricular exclusion is frequently applied to complex congenital lesions. Results are adversely affected by increases in pulmonary vascular resistance and/or ventricular end-diastolic pressure. Survival depends on means to increase pulmonary blood flow. To address this problem, we have developed a triple-balloon catheter that is passed via the femoral vein to the right atrium. Inflation is sequenced so that small balloons located at the junction of each vena cava with the atrium inflate first. Then the larger central balloon inflates, ejecting blood from the righ

Effect of acute hypoxia on airway and vascular exchangeable lung water spaces in dogs.

To determine if alveolar hypoxia causes subclinical noncardiogenic pulmonary edema, we measured in 8 dogs the rebreathing pulmonary tissue volume (Vt) and the pulmonary extravascular water volume using the single-pass double-indicator dilution method. After baseline measurements, the dogs were ventilated with the lowest concentration of oxygen that would not cause left ventricular failure (9 to 13% O2). One to 6 h of hypoxia had no effect on Vt, but caused a reversible 38% fall in pulmonary extravascular lung water volume by the indicator method (p less than 0.01). The ratio of the extravascul

The IL-5 receptor on human bronchus selectively primes for hyperresponsiveness.

BACKGROUND: The role of IL-5-induced eosinophilia in airway hyperresponsiveness has been questioned. In addition, eosinophil-independent IL-5-induced airway hyperresponsiveness has been demonstrated in animals. OBJECTIVE: In this study, IL-5 was investigated for direct effects on human bronchial responsiveness. METHODS: Human muscle preparations were isolated from organ donor and surgical tissue. Bronchus, jejunum, and saphenous vein were incubated for 24 hours in vitro with recombinant human (rh) IL-5. Contractility to acetylcholine (bronchus, jejunum) and phenylephrine (saphenous vein) was

Aspirin for Venous Thromboembolism Prophylaxis in Hip Fracture, Total Knee, and Hip Arthroplasty.

Venous thromboembolism is a complication of major orthopedic surgeries such as hip fracture repair, total knee arthroplasty, and total hip arthroplasty. Prophylaxis against venous thromboembolism reduces the risk of pulmonary embolism and deep vein thrombosis associated with these procedures. Aspirin is widely prescribed for venous thromboembolism prophylaxis in major orthopedic surgery. We review the epidemiology of hip fracture, total knee arthroplasty, and total hip arthroplasty, and describe aspirin's mechanisms of action, efficacy, and safety.

Vascular Ultrasound.

Vascular point-of-care ultrasound is a useful tool for emergency department (ED) clinicians to evaluate lower extremity pain and swelling. It can quickly and safely detect deep vein thrombosis, a serious condition that can lead to pulmonary embolism and post-thrombotic syndrome as well as diagnose pseudoaneurysm, hematoma, and acute arterial occlusion, a vascular emergency that requires urgent surgical intervention. Point-of-Care Ultrasound can be a useful tool for the evaluation of ED patients with vascular emergencies.

Troubleshooting OptEase inferior vena cava filter retrieval.

For treatment of deep vein thrombosis and prevention of pulmonary thromboembolism, a retrievable inferior vena cava filter is commonly utilized as an effective bridge to anticoagulation. However, we have experienced difficulties in retrieving inferior vena cava filters. Endovascular retrieval assisted by disposable biopsy forceps is an appropriate approach because it provides a less-invasive low-cost way to remove a migrated filter. We suggest this troubleshooting technique to deal with filter hook migration into the caval wall.

Transcaval invasion of right atrium by thymoma: resection via transient cava-pulmonary shunt.

Transcaval extension of the thymoma to the right atrium has very rarely been reported, and cardiopulmonary bypass is recommended for successful resection. An invasive thymoma with intravascular invasion of the superior vena cava, and the left innominate vein extending into the right atrium was presented. Intra-atrial extension was resected through a transient external shunt from the inferior vena cava to the main pulmonary artery. We discussed the feasibility of this surgical technique and possible advantages of cardiopulmonary bypass avoidance.

Cerebral air embolism as a complication of peptic ulcer in the gastric tube: case report.

BACKGROUND: The reported incidence of ulcer formation in the gastric tube in esophageal replacement is rare. CASE PRESENTATION: This is the first report of a case of cerebral air embolism as a result of spontaneous perforation of an ulcer in the constructed gastric tube into the pulmonary vein during post-operative follow-up in a patient with esophageal cancer. CONCLUSIONS: Cerebral air embolism is a rare complication of penetrating gastric ulcer, but should be considered in patients with a history of esophagectomy with gastric conduit that present with acute neurologic findings.

Is physical training contraindicated in patients with deep vein thrombosis during cardiac rehabilitation?

Deep vein thrombosis is a potential complication in patients admitted to cardiac rehabilitation programs after acute coronary syndromes, episodes of acute congestive heart failure, and cardiac revascularization. A common clinical problem in these patients is to decide whether to start or continue physical training or not, given the risk of pulmonary embolism. Until definite evidence becomes available, careful patient selection and inpatient supervision may avoid the a priori withdrawal of such an important core component of cardiac rehabilitation programs.

Upper extremity deep vein thrombosis.

Upper extremity deep vein thrombosis (UEDVT) is a rare thrombotic disorder (1-4% or all DVT), but it has a potential for considerable morbidity in the form of pulmonary embolism, persistent upper extremity pain and swelling, superior vena cava syndrome and loss of vascular access. Newer non-invasive methods such as duplex ultrasound and magnetic resonance angiography facilitate early diagnosis. Early catheter directed thrombolysis followed by anticoagulation prevent long-term sequalae.

A case of superoinferior ventricular heart with situs ambiguus, dextroventricular loop, and levo transposition of the arteries: prenatal and postnatal echocardiographic diagnosis.

A unique case of superoinferior ventricles, left atrial isomerism, concordant atrioventricular connection, and discordant ventriculoarterial connection is described. The associated anomalies were azygous vein continuation, large subpulmonary and apical ventricular septal defects, and left ventricular outflow tract obstruction (pulmonary stenosis). The diagnosis was mostly made by fetal echocardiography and the superoinferior location of the ventricles was confirmed by postnatal echocardiography. Fetal and postnatal echocardiography facilitated the precise morphologic diagnosis and segmental an

Is Baker's cyst a risk factor for pulmonary embolism?

We encountered a 73-year-old man with acute pulmonary embolism (PE) and Baker's cyst. Venography revealed that the right popliteal vein was compressed by Baker's cyst and deep venous thrombosis (DVT) had developed. DVT associated with Baker's cyst is rather common and these two conditions are thought to be causally related. Baker's cyst is the most frequent mass lesion in the popliteal region. We suggest that Baker's cyst is a risk factor for PE as well as surgery and trauma.
