The effect of radial head prosthesis diameter on posterolateral rotatory instability of the elbow.

BACKGROUND: The purpose of this study is to investigate how different diameters of radial head replacement affect posterolateral translation with a valgus and supination force. We hypothesized that there would be less posterolateral rotatory translation with larger implant diameter. METHODS: Eleven cadaveric arms were stressed at 30 and 60° of flexion with a consistent supination and valgus stress force under five conditions: native radial head, radial head excision, and with 3 sizes of radial head prosthesis. Displacement of the radial head posteriorly in relation to the capitellum on radiog

Bone Density, Microarchitecture, and Geometry Assessment in Patients with Pachydermoperiostosis Using Second-Generation High-Resolution Peripheral Quantitative Computed Tomography: A Case-Control Study.

Pachydermoperiostosis (PDP) is a rare genetic disorder manifesting with periostosis, clubbing, and thickened skin. The impact of PDP on bone density and microarchitecture is underexplored despite the potential derangement in bone health due to systemic inflammation. This cross-sectional case-control study was conducted in a tertiary care center in north India from July 2022 to July 2023. We compared treatment naïve PDP patients (n = 8) with age and BMI-matched apparently healthy controls. All participants underwent clinical examination and estimation of biochemical parameters including calcium

Teenagers and young adults with nephropathic cystinosis display significant bone disease and cortical impairment.

BACKGROUND: Bone impairment appears to be a novel complication of nephropathic cystinosis despite cysteamine therapy. Its exact underlying pathophysiology is nevertheless unclear. The objective of this study was to evaluate bone status among patients included in the French Crystobs study. METHODS: In addition to clinical data, bone status was evaluated using biomarkers (ALP, PTH, 25-D, 1-25D, FGF23), DXA (spine and total body), and high-resolution peripheral quantitative computed tomography (HR-pQCT) at the tibia and radius. Results were compared to age- and gender-matched healthy controls (1

Insulin Resistance Is Associated With Smaller Cortical Bone Size in Nondiabetic Men at the Age of Peak Bone Mass.

CONTEXT: In type 2 diabetes mellitus, fracture risk is increased despite preserved areal bone mineral density. Although this apparent paradox may in part be explained by insulin resistance affecting bone structure and/or material properties, few studies have investigated the association between insulin resistance and bone geometry. OBJECTIVE: We aimed to explore this association in a cohort of nondiabetic men at the age of peak bone mass. DESIGN, SETTING, AND PARTICIPANTS: Nine hundred ninety-six nondiabetic men aged 25 to 45 years were recruited in a cross-sectional, population-based siblin

Parathyroid hormone(1-84) treatment of postmenopausal women with low bone mass receiving hormone replacement therapy.

Treatment of postmenopausal osteoporosis (PMO) is based primarily on antiresorptive agents, including hormone replacement therapy (HT). To evaluate whether anabolic therapy together with HT provides additional benefits in the treatment of PMO, we evaluated the effects of parathyroid hormone (PTH) 1-84 in postmenopausal women with low bone mineral density (BMD) who were receiving chronic (> or =6 months) HT. Subjects were randomized to receive 100 microg PTH(1-84) or placebo injections daily for 24 months (n = 90/group). The primary efficacy outcome was change from baseline in lumbar spine BMD.

Retired elite female ballet dancers and nonathletic controls have similar bone mineral density at weightbearing sites.

Elite female ballet dancers exhibit several risk factors for osteoporosis during their performing years. To study the long-term effect of this lifestyle, we compared the bone mineral density (BMD) of 101 retired elite female ballet dancers (mean age 51 years, SD = 14 years) and 101 normal controls, derived from a twin study, matched hierarchically for age, height, weight, and menopausal status. The dancers, who had been retired for a mean of 25.6 years (range 1-53 years) reported a greater prevalence of previous menstrual disturbance, greater lifetime alcohol intake and smoking, and a lower di

Staged non-bridging circular external fixation with sequential osteoporosis therapy for MRSA-infected distal radius non-union: a case report.

BACKGROUND: Infected distal radius non-union in elderly patients with severe osteoporosis is particularly challenging because infection eradication, stable fixation, and preservation of wrist function must be achieved simultaneously. Conventional internal fixation in an infected osteoporotic field carries a high risk of recurrent infection and fixation failure, whereas bridging external fixation may result in prolonged immobilization and functional deterioration. We report a case of methicillin-resistant Staphylococcus aureus (MRSA)-infected distal radius non-union treated with staged non-brid

[Volar locking plating for distal radial fractures. Is osteoporosis associated with poorer functional results and higher complications rates?].

BACKGROUND: The purpose of this prospective study was to compare the functional and radiological results and complication rates after locking plate osteosynthesis of unstable distal radial fractures in elderly patients as a function of the bone mineral density (BMD). The hypothesis that reduced BMD is accompanied by poorer function and a higher rate of complications was investigated. PATIENTS AND METHODS: A total of 65 consecutive patients with unstable distal radial fractures, AO 23-A2 (n=1), -A3 (n=29), -C1 (n=2), -C2 (n=28) and -C3 (n=5) were included with a mean age of 69 years (range, 52

[Operative corrections of posttraumatic deformities of the elbow joint in children].

Posttraumatic deformities with need for surgical correction are rare and demanding in the pediatric population. The consequences of a bad outcome may last a life time. The best prevention of deformities around the elbow joint is proper initial treatment. The most common deformities are cubitus varus, cubitus valgus, chronic dislocation of the radial head and pseudarthrosis of the radial condyle. In contrast to the wide spread opinion to await the effect of the further growth, the deformity should be treated contemporarily to the underlying injury. This strategy will optimize the outcome. Treatment of type I capitellar fractures in adolescents.

BACKGROUND: Because fractures of the capitellum are rare in childhood and the young adolescent period, their treatment is still debatable and there appears to be no established treatment protocol. In the present study, we evaluated the results obtained in adolescents with type 1 capitellar fractures who were treated with open reduction and internal fixation with a 3.5 mm lag screw, directed from posterior to anterior. METHODS: Twelve type I capitellar fractures in adolescents were treated with open reduction and internal fixation with a single 3.5 mm cortical lag screw directed from the poste

Upper extremity stress fractures.

Although less common than lower-extremity stress fractures, upper-extremity stress fractures are becoming recognized much more frequently. A majority of these fractures are caused by overuse and fatigue of the surrounding musculature and, as a result, may be prevented by appropriate training and conditioning. Diagnosis is made by history and physical examination with the aid of plain radiographs, bone scans, and MRI. Most of these fractures heal with a period of relative rest followed by a structured rehabilitation program. A small percentage of these fractures, however, may require surgical f

The role of a multidisciplinary approach to opioid reduction for patients with operatively treated upper extremity fractures.

BACKGROUND: The aim of this study was to determine whether a multimodal program for opioid reduction in upper extremity fracture patients decreased (1) inpatient opioid consumption during post-operative days (PODs) 1-4 and (2) outpatient opioid prescription volume. METHODS: We conducted a retrospective comparative study of patients with isolated upper extremity fractures admitted to the inpatient orthopedic unit at an academic level 1 trauma center in 2018 and 2020 (pre- and post-program implementation). We calculated total inpatient opioid consumption (measured in morphine milligram equivale

Routine 6-weeks outpatient visit in patients treated surgically for upper extremity fractures: is it truly necessary?

BACKGROUND: Due to the increasing burden on the heatlhcare system the usefulness of the routinely planned 6-week outpatient visit and radiograph in patients treated surgically for the most common upper extremity fractures, including clavicle, proximal humerus, humeral shaft, olecranon, radial shaft, and distal radius, should be investigated. METHOD: This was a retrospective cohort study of all patients treated surgically for the most common upper extremity fractures between 2019 and 2022 in a level 1 trauma center. The first outcome of interest was the incidence of abnormalities found on the

Diagnostic accuracy of various modalities relative to open bone biopsy for detection of long bone posttraumatic osteomyelitis.

BACKGROUND: Long bone posttraumatic osteomyelitis (PTOM) is a relatively common complication following surgical fixation of open fractures. There is a lacking consensus on ideal strategies for diagnostic evaluation of long bone PTOM. While open bone biopsy and culture is considered the 'gold diagnostic standard,' its cost and invasiveness are often prohibitive and have prompted the search for alternate diagnostic methods. OBJECTIVE: To evaluate the sensitivity and specificity of various diagnostic modalities relative to open bone biopsy and culture for the detection of long bone PTOM. DESIGN
