Lofexidine versus diazepam for the treatment of opioid withdrawal syndrome: A double-blind randomized clinical trial in Singapore.

BACKGROUND: Many individuals leave costly inpatient detoxification programs prematurely because of the severity of withdrawal symptoms experienced. In the absence of opioid-assisted detoxification in Singapore, diazepam is used to manage withdrawal. However since diazepam is addictive, there is a need to explore the effectiveness of alternative medications. DESIGN AND PROCEDURES: The study aimed to examine the safety and efficacy of lofexidine, a non-opiate, non-addictive, alpha 2-adrenergic agonist in assisting opioid detoxification in Singapore, using a randomized, double-blind, investigato

Alpha₂-adrenergic agonists for the management of opioid withdrawal.

BACKGROUND: Withdrawal is a necessary step prior to drug-free treatment or as the endpoint of long-term substitution treatment. OBJECTIVES: To assess the effectiveness of interventions involving the use of alpha2-adrenergic agonists compared with placebo, reducing doses of methadone, symptomatic medications, or an alpha2-adrenergic agonist regimen different to the experimental intervention, for the management of the acute phase of opioid withdrawal. Outcomes included the withdrawal syndrome experienced, duration of treatment, occurrence of adverse effects, and completion of treatment. SEARCH

Itch Management: Topical Agents.

Chronic pruritus is a common problem in patients with inflammatory skin diseases as well as in subjects with dry or sensitive skin. Regardless of the underlying cause of the pruritus, a topical therapy is not only useful but most often necessary to achieve symptom control. A good topical therapy should fulfill different functions. An optimal basic therapy based on the condition of the skin is important to repair epithelial barrier defects and to rehydrate the skin. An adequate disease-specific topical therapy is crucial for inflamed skin, e.g. anti-inflammatory topical therapy is an important

Impact of prospectively determined A118G polymorphism on treatment response to injectable naltrexone among methamphetamine-dependent patients: an open-label, pilot study.

OBJECTIVES: Methamphetamine (MA) addiction has no known effective pharmacotherapy. Small trials showed beneficial effects for oral naltrexone in amphetamine users. Trials in alcohol-dependent subjects showed better response in persons with the A118G single nucleotide polymorphism of the μ-opioid receptor. We conducted a pharmacogenetic trial of sustained release intramuscular naltrexone to examine the role of the A118G single nucleotide polymorphism in MA dependence. METHOD: All eligible A118G subjects screened were enrolled; an equal number of wild type (A118A) subjects were selected using m

Genetic and early environmental contributions to alcohol's aversive and physiological effects.

Genetic and early environmental factors interact to influence ethanol's motivational effects. To explore these issues, a reciprocal cross-fostering paradigm was applied to Fischer and Lewis rats. The adult female offspring received vehicle or the kappa opioid antagonist nor-BNI (1 mg/kg) followed by assessments of conditioned taste aversion (CTA), blood alcohol concentrations (BACs) and hypothermia induced by 1.25 g/kg intraperitoneal ethanol. CTA acquisition in the in-fostered Fischer and Lewis animals did not differ; however, the Fischer maternal environment produced stronger acquisition in

Pharmacologic therapies for complex regional pain syndrome.

Complex regional pain syndrome (CRPS) remains a challenging condition to diagnose and treat. There are few large-scale, randomized trials of pharmacologic agents, and most published studies are small, uncontrolled, or presented only in abstract form at meetings. The most commonly used agents, such as anticonvulsants, antidepressants, and opiates, have been found to be useful for other neuropathic pain conditions in large-scale trials but have not been adequately studied in CRPS. Systemic steroids delivered by multiple routes continue to be used, with some good evidence for short-term administr

Novel insights into M5 muscarinic acetylcholine receptor function by the use of gene targeting technology.

Until recently, little was known about the possible physiological functions of the M(5) muscarinic acetylcholine receptor subtype, the last member of the muscarinic receptor family (M(1)-M(5)) to be cloned. To learn more about the potential physiological roles of this receptor subtype, we generated and analyzed M(5) receptor-deficient mice (M5 -/- mice). Strikingly, acetylcholine, a potent dilator of most vascular beds, virtually lost the ability to dilate cerebral arteries and arterioles in M5 -/- mice, suggesting that endothelial M(5) receptors mediate this activity in wild-type mice. This e

Physiologic and neuroendocrine responses to intravenous naloxone in subjects with Alzheimer's disease and age-matched controls.

BACKGROUND: Prior work showed that administration of naloxone HCl had different behavioral effects in patients with Alzheimer's disease (AD) than controls. The aim of the present study was to contrast the physiologic and neuroendocrine responses to administration of a wide range of doses of intravenous naloxone of patients with probable Alzheimer's disease to aged-matched controls. METHODS: This was a double-blind, placebo-controlled, study of 12 patients with probable Alzheimer's disease and 8 age-matched normal controls who each received intravenous infusions of naloxone HCl on 3 different

Quantification of human opiate receptor concentration and affinity using high and low specific activity [11C]diprenorphine and positron emission tomography.

[11C]Diprenorphine, a weak partial opiate agonist, and positron emission tomography were used to obtain noninvasive regional estimates of opiate receptor concentration (Bmax) and affinity (Kd) in human brain. Different compartmental models and fitting strategies were compared statistically to establish the most reliable method of parameter estimation. Paired studies were performed in six normal subjects using high (769-5,920 Ci/mmol) and low (27-80 Ci/mmol) specific activity (SA) [11C]diprenorphine. Two subjects were studied a third time using high SA [11C]diprenorphine after a pretreatment wi

Peripartum Esketamine: A Narrative Review of Analgesia, Postpartum Depressive Symptoms, Sleep-Related Outcomes, and Safety.

Pain, depressive symptoms, and sleep disruption frequently overlap during the peripartum period, although their relationships are associative rather than established as causal. Esketamine, an N-methyl-D-aspartate receptor antagonist, has attracted interest as a peripartum intervention with potential effects on pain, mood, and sleep. This narrative review evaluates the clinical and mechanistic evidence for peripartum esketamine in analgesia, postpartum depressive symptoms, sleep-related outcomes, and maternal-infant safety, with particular attention to the directness and interpretability of the

Mislabeling and Status of Semisynthetic Kratom-Derived Products in the US Market.

Kratom (Mitragyna speciosa), a plant native to Southeast Asia, has traditionally been used for stimulant and analgesic effects, yet its safety profile remains poorly understood given its complex polypharmacology and rapidly evolving commercial formulations. It contains a wide variety of indole alkaloids, with mitragynine (MTG) as the major constituent and most abundant psychoactive alkaloid. There has been a rapid growth of the kratom market, accompanied by a marked increase in kratom-related exposures, hospitalizations, and deaths in the United States, coinciding with the introduction of krat

Neuroscientific Basis of Ketamine as an Analgesic and Emerging Role in the Management of Depression.

PURPOSE OF REVIEW: Ketamine was originally developed as a dissociative anesthetic and has remained a valuable agent in anesthesia related to its ability to preserve airway reflexes and relative hemodynamic stability. Over time, its clinical applications have expanded beyond procedural sedation to include roles in pain management and psychiatry. In pain medicine, the therapeutic rationale is primarily grounded in its noncompetitive antagonism of the N-methyl-D-aspartate (NMDA) receptor and modulation of central sensitization, a key mechanism underlying refractory neuropathic and centralized pai

Suzetrigine for Acute Pain in the Emergency Department: A First-in-Class NaV1.8 Blocker.

Suzetrigine is the first selective, orally bioavailable inhibitor of the peripheral voltage-gated sodium channel NaV1.8. In January 2025, it became the first nonopioid analgesic approved by the US Food and Drug Administration in more than 2 decades. We review preclinical data, regulatory filings, and all published phase 2 to 3 randomized, double-blind trials conducted to describe its potential role in emergency department (ED) pain management. The target receptor, NaV1.8, is critical for nociceptor firing yet absent from the central nervous system and cardiac tissue. This means suzetrigine pro

Spinal NLRP3 blockade reverses morphine-prolonged neuropathic pain and proinflammatory immune actions in prenatal alcohol-exposed male mice.

Prenatal alcohol exposure (PAE) exerts long-term alterations in the adult neuroimmune system. Previous work showed that PAE exacerbates morphine-mediated immune responses, prolonging nerve injury-induced allodynia and promoting aberrant NLRP3-driven inflammation in both the periphery and spinal cord. Whether the spinal actions of NLRP3, arising from aberrant TLR4 activation, are critical in driving allodynia remains unknown. With a 25+ day-long time course study, using another previously established PAE model in mice, we reproduced these adverse effects of PAE and further examined whether spin
