Effect of low protein diet on manganese neurotoxicity: I. Developmental and biochemical changes.

The first in a series of studies on the effect of concurrent low protein diet (10% casein) and manganese exposure (Mn2+, 3 mg/ml drinking water) in rats is reported here. The effect on growing (Fo-90 days) rehabilitated (Fo low leads to normal protein diet-28 days) and the F1 generation pups (weaned) were studied. Mn2+ exposure had no significant effect on growth pattern, brain weight or brain and plasma protein contents in either dietary groups. The diet regimen had no significant effect on the accumulation of Mn2+ in brain in any of the groups studied but the levels were higher in the F1 pup

Hyperventilation-induced asthma: evidence for two mechanisms.

The mechanism by which airway cooling induces airflow obstruction in asthmatic subjects has not yet been established. Using a pair of isocapnic hyperventilation challenges, with a 40-minute interval, we looked for the presence of a refractory period in 19 asthmatic patients (aged 9-18 years). The subjects fell into two groups. The eight in the "non-refractory" group showed less than a 25% reduction in response to the second challenge, but the 11 in the "refractory" group showed at least a 35% reduction. Twelve subjects also performed a hyperventilation challenge after cholinergic blockade with

Effects of phentolamine, dihydroergocristine and isoxsuprine on the blood pressure and heart rate in normotensive, hypotensive and hypertensive rats.

Phentolamine, dihydroergocristine and isoxsuprine were compared for their effects on the blood pressure in anaesthetized normotensive rats, in rats made hypotensive by ganglionic blockade or by pithing and in rats with noradrenaline-induced hypertension. Their ability to inhibit pressor responses elicited by electrical stimulation of the posterior hypothalamus and of the sympathetic outflow from the spinal cord was also investigated. All three drugs appeared very potent in inhibiting noradrenaline-induced hypertension and caused a dose-dependent fall in blood pressure in normotensive rats, whi

Exercise-induced airways constriction.

Airway conductance was measured in a body plethysmograph at different lung volumes before and after graded exercise. In 14 out of 19 patients, mostly asthmatics, airway conductance fell significantly after exercise. These subjects also showed other signs of an increased bronchial reactivity to different stimuli, including forced breathing, hyperventilation, and cold air, but they had no exogenous allergy. The exercise-induced bronchoconstriction could be blocked by atropine in six of the nine patients tested. Exercise-induced bronchoconstriction in patients with clinical and physiological evid

Primary neurogenic orthostatic hypotension.

Eight further cases of neurogenic orthostatic hypotension are described together with a necropsy study on one case. Three cases showed evidence of autonomic dysfunction in isolation, while in five cases this was accompanied by evidence of more diffuse central nervous system degeneration. (Parkinsonism, cerebellar ataxia, dementia, pyramidal signs, bulbar weakness, and muscular wasting were all seen in varying proportions.)The various clinical presentations, investigations, pathology, treatment, and prognosis are discussed. Vestibular evoked myogenic potentials in practice: Methods, pitfalls and clinical applications.

Vestibular evoked myogenic potentials (VEMPs) are a useful and increasingly popular component of the neuro-otology test battery. These otolith-dependent reflexes are produced by stimulating the ears with air-conducted sound or skull vibration and recorded from surface electrodes placed over the neck (cervical VEMPs) and eye muscles (ocular VEMPs). VEMP abnormalities have been reported in various diseases of the ear and vestibular system, and VEMPs have a clear role in the diagnosis of superior semicircular canal dehiscence. However there is significant variability in the methods used to stimul

Pyramidal signs in a Caucasian patient with spinal muscular atrophy: a case report.

Spinal muscular atrophy (SMA), an autosomal recessive disease, is characterized by the selective loss of spinal motor neurons due to reduced levels of the survival motor neuron (SMN) protein. The clinical symptoms of SMA are progressive proximal muscle weakness and paralysis. Here we describe a 20-year-old Turkmenistan male with SMA who presented with uncommon pathological reflexes and asymmetric onset of weakness. The diagnosis after genetic analysis revealed a homozygous deletion of SMN1 exons seven and eight. The copies of SMN2 exon seven were normal. Although pyramidal signs are not a comm

Combination of AIDP and pyramidal signs associated with antecedent hepatitis A infection: a rare (co)occurrence.

Guillain-Barré syndrome (GBS) is a postinfectious illness and commonly occurs in association with infective agents such as Campylobacter jejuni, cytomegalovirus, Epstein-Barr virus, Mycoplasma pneumoniae, HIV, shigella, clostridium, Haemophilus influenzae; occasionally with acute hepatitis B, C, E; and more rarely with hepatitis A, as documented in various case reports. Classically, GBS presents with hyporeflexia or areflexia, but preserved and brisk reflexes have been described in context with GBS variant, AMAN (acute motor axonal neuropathy), in approximately 33% of the cases. Clinical symptoms of bilateral anterior cerebral artery territory infarction.

Bilateral anterior cerebral artery (ACA) territory infarction is rare and its associated symptoms are still not well understood. We evaluated the clinical symptoms of four patients with bilateral ACA infarction. The common clinical features were various primitive reflexes and parkinsonian symptoms including akinesia, rigidity and hypophonia. Frontal release signs were present in all patients with ACA infarction even without direct involvement of the prefrontal cortex. Enhanced glabellar tap response, previously not reported in patients with ACA infarction, was the most consistent finding, and

[Effects of intrauterine alcohol intoxication on the function of the central nervous system in the progeny].

Behavior of mature rats, who had been exposed antenatally to intrauterine alcoholic intoxication, was studied in the open field settings, unavoidable swimming and Simonov's emotional resonance situations; their conditioned reflexes of passive and active avoidance were trained, arterial blood pressure was measured, and sleeping patterns examined. It was demonstrated that animals' orientation/exploration activity was reduced, while passive behavior and fear-associated emotions became more prominent, conditioned reflexes were more difficult to form, vegetative autoregulation was disturbed, and in

A tentative classification of recessively inherited ataxias.

We present a working classification of recessively inherited ataxic syndromes based on the use of simple tools available to every clinician: a good history (particularly pinpointing the age of onset) and a good neurological examination (simplified to the verification of the presence of ataxia, deep tendon reflexes in the knee, optic nerve, retinal and/or 8th nerve signs). In the three groups of disorders (non progressive, intermittent or progressive) patients can be hyper/normo reflexic, or they can be hypo/areflexic. Six principal types of progressive ataxic disorders are further delineated b

Cardiovascular effects of apnoea in preterm infants.

Cardiovascular reflex responses have been studied in 9 newborn preterm infants with apnoeic episodes and in 2 preterm infants with periodic breathing. Respiration, blood pressure, heart rate, and peripheral blood flow were simultaneously recorded. Peripheral blood flow was measured in the leg by venous occlusion plethysmography. During apnoea, bradycardia and peripheral vasoconstriction occur. There is little change in blood pressure though pulse pressure increases. No cardiovascular changes were seen before the onset of apnoea. Periodic breathing had little effect on peripheral blood flow. Pr

A clinical case of neosporosis in a 4-week-old holstein friesian calf which developed hindlimb paresis postnatally.

A 4-week-old female Holstein Friesian calf presented with hindlimb paresis. Neurologic examination of spinal reflexes revealed depressed or absent reflexes of the hindlimbs. Menace responses on both sides disappeared on examination of cranial nerves. The calf was finally diagnosed with Neospora caninum infection by pathological findings including nonsuppurative inflammation associated with cysts in the cerebrum and spinal cord. High levels of antibody against recombinant surface antigen 1 of N. caninum (NcSAG1) were detected by ELISA from both serum and cerebrospinal fluid (CSF) samples. This

Enteric glia regulate gut motility in health and disease.

The enteric nervous system, often referred to as the second brain, is the largest assembly of neurons and glia outside the central nervous system. The enteric nervous system resides within the wall of the digestive tract and regulates local gut reflexes involved in gastrointestinal motility and fluid transport; these functions can be accomplished in the absence of the extrinsic innervation from the central nervous system. It is neurons and their circuitry within the enteric nervous system that govern the gut reflexes. However, it is becoming clear that enteric glial cells are also actively inv

Airway protective mechanisms: current concepts.

There are at least eight mechanisms identified that result either in volume clearance of the pharynx and esophagus (secondary peristalsis and pharyngeal swallow) or prevent entry of the gastric content into the esophagus and pharynx (LES and UES), accentuate these barriers (esophago-UES and pharyngo-UES contractile reflexes), or induce closure of the vocal cords and introitus to the trachea (esophagoglottal and pharyngoglottal reflexes). The sum effect of various combinations of these mechanisms is suggested to help prevent retrograde aspiration. In other words, airway protective mechanisms ag

Are albino rats night blind?

Based on single-unit recordings from the superior colliculus and optic nerve, albino rats recently were reported to have dark-adapted thresholds that are 2 log units higher than those of pigmented rats. To confirm this result, electroretinograms (ERG) were recorded with pupillary light reflex thresholds from the same strains of albino (CD) and pigmented (Long-Evans hooded) rats. Neither ERG nor pupil measurements showed higher dark-adapted thresholds for albino relative to pigmented animals. Both groups had dark-adapted thresholds close to the thresholds found for hooded animals in the reporte

Methyl bromide myoclonus: an electrophysiological study.

We report a case of myoclonus from overnight exposure to methyl bromide. Myoclonus was either spontaneous or induced by somatosensory stimulation or voluntary movements, multifocal and sometimes generalized. Median SEP showed normal size P14-N20, but giant parietal P25, N33 and frontal P22-N30 waves. Back-averaging showed a biphasic EEG spike of maximal amplitude at the central region contralateral to the corresponding myoclonic jerk recorded from abductor pollicis brevis and preceding it by a short interval consistent with conduction in corticospinal pathways. Friedreich's ataxia: a clinical review of 20 childhood cases.

The authors report the clinical review of 20 childhood cases with Friedreich's ataxia. The mean age at onset of symptoms was 6.1 years. The main presenting symptom was abnormal gait (100%). Ataxia of gait and limbs and depressed or absent tendon reflexes were found in all cases. Clinical findings are in accordance with the findings of Harding and Werdelin. Neurophysiological studies (especially sensory) are important in the confirmation of the diagnosis. Of the 10 cases in which sensory nerve conduction velocity measurements were performed, 7 had absent sensory action potentials, 2 had decreas

Four comatose patients were found to have the Wernicke-Krosakoff syndrome. All had a history of alcoholism, previous alcoholic neurological disease, and poor nutrition. Intravenous or nasogastric tube feeding without vitamin supplements precipitated coma in three. Examination showed a diffuse encephalopathy with intact pupillary light reflexes, no focal neurological signs, and absent doll's eye and caloric responses. The tendon reflexes were uniformly absent.
