Dental problems and chronic diseases in mentally ill homeless adults: a cross-sectional study.

BACKGROUND: Dental problems (DPs) and physical chronic diseases (CDs) are highly prevalent and incident in people with low socioeconomic status such as homeless individuals. Yet, evidence on the association between DPs and physical CDs in this population is limited. In the present study, we assessed the association between DPs and type and number of CDs in individuals experienced chronic homelessness and serious mental health problems. METHODS: We analyzed cross-sectional baseline data from 575 homeless adults with serious mental health problems participating in the Toronto site of the At Hom

Patient predictors of health-seeking behaviour for persons coughing for more than two weeks in high-burden tuberculosis communities: the case of the Western Cape, South Africa.

BACKGROUND: This study aimed to analyse the patient predictors of health-seeking behaviour for persons coughing for more than 2 weeks to better understand this vulnerable and important population. METHODS: The study analysed data from a cohort study (SOCS - Secondary Outcome Cohort Study) embedded in a community randomised trial ZAMSTAR (Zambia and South Africa TB and AIDS Reduction Study) in eight high-burden TB communities in the Western Cape, South Africa. These datasets are unique as they contain TB-related data as well as data on health, health-seeking behaviour, lifestyle choices, emplo

An overview of insulin therapy for the non-specialist.

Nearly all health professionals, whatever their practice or speciality, now have contact with a significant number of insulin-using people with diabetes. People with type 1 diabetes are nearly universally managed on more complex insulin regimens, increasingly with complex support technology, and with some understanding of the concepts underlying these needed by anyone with responsibility for other aspects of their health care. People with type 2 diabetes usually come to insulin therapy in time, now with a prevalence for insulin administration of ≈50%, thanks to improved management of associate

School food policy in secondary schools in England and its impact on adolescents' diets and dental health: the FUEL multiple-methods study.

BACKGROUND: School food standards are a legal requirement for state-funded schools in England and are designed to promote healthy eating in pupils. However, state-funded academies/free schools established between 2010 and 2014 are exempt from this legislation. To complement the school food standards, the government launched the School Food Plan in 2013, which outlines voluntary actions that schools can take to support healthy eating and increase school meal uptake. There has been little evaluation of the school food standards and School Food Plan in secondary schools. OBJECTIVES: To compare i

G6PD testing and radical cure for Plasmodium vivax in Cambodia: A mixed methods implementation study.

INTRODUCTION: Cambodia aims to eliminate malaria by 2025, however tackling Plasmodium vivax (P.v) presents multiple challenges. The prevalence of glucose-6-phosphate dehydrogenase (G6PD) deficiency has prevented the deployment of 8-aminoquinolones for "radical cure", due to the risk of severe haemolysis. Patients with P. vivax have therefore continued to experience recurrent relapses leading to cumulative health and socioeconomic burden. The recent advent of point of care testing for G6PD deficiency has made radical cure a possibility, however at the time of the study lack of operational exper

Iron-Rich Food Consumption and Predictors Among Children Aged 6 to 23 Months in Ethiopia: A Frequentist and Bayesian Hierarchical Approach.

Iron deficiency is a major public health issue in Ethiopia, especially in children aged 6 to 23 months. The 6 to 23 months period marks the introduction of complementary feeding alongside breast milk, when nutritional needs surge but intake often falls short, leading to high vulnerability. This study aimed to assess the weighted iron-rich food consumption, trends, and predictors among children aged 6-23 months old in Ethiopia. This study used Ethiopian Demographic and Health Survey (EDHS-2005-2019) data with a total weighted sample size of 7445 among children aged 6-23 months old. Bayesian mul

PRECISION study: impact of personalised cardiac anaesthesia and cerebral autoregulation on neurological outcomes in patients undergoing cardiac surgery - protocol for an international, multicentre, prospective cohort study.

INTRODUCTION: Adverse neurological complications, including postoperative delirium (POD) and stroke, remain one of the major risks after cardiac surgery. A lack of comprehensive knowledge about their causes and neuroprotective strategies has hindered the development of effective interventions to reduce these events. Personalised cerebral autoregulation (CA)-oriented blood pressure monitoring aims to identify blood pressure targets tailored to each individual patient, thereby reducing brain injury. The PRECISION study aims to assess whether perioperative duration and magnitude of mean arterial

The impact of spinal pain on family members' burden: a cross-sectional study.

OBJECTIVE: Spinal pain is highly prevalent and associated with substantial individual burden. Qualitative studies suggest that family members of individuals with spinal pain may also experience burden; however, quantitative data on family-member burden remain limited. This study had two linked objectives. First, we conducted a pragmatic cognitive debriefing study to evaluate the content validity of the Family-Reported Outcome Measure-16 (FROM-16) in a spinal pain context. Second, we conducted an exploratory cross-sectional survey using FROM-16 to estimate family-member burden in a Danish secon

Index Procedure Length of Stay and Achievement of Patient Acceptable Symptom State Predict Outcomes of Contralateral Total Hip Arthroplasty Within One Year.

BACKGROUND: Many patients who have bilateral hip osteoarthritis eventually undergo contralateral total hip arthroplasty (THA), yet limited data exist on how recovery differs between staged procedures performed within one year. This study evaluated differences in patient-reported outcomes and health care utilization following staged bilateral THA and identified predictors of suboptimal outcomes after the second surgery. METHODS: We retrospectively reviewed a prospective institutional registry to identify patients who underwent staged bilateral primary THA within one year (n = 680) and complete

Reducing the impact of diabetic foot ulcers (REDUCE): study protocol for an effectiveness and cost-effectiveness randomised controlled trial with embedded process evaluation.

INTRODUCTION: Diabetic foot ulceration represents a prevalent, persistent and resource-intensive complication of diabetes. These ulcers are slow to heal, prone to recurrence and impose a substantial burden on both patients and healthcare providers. The reducing the impact of diabetic foot ulcers (REDUCE) intervention has been designed as a multifaceted approach targeting psychological and behavioural determinants linked to diabetic foot ulcer (DFU) outcomes. Following a successful pilot trial, the REDUCE trial has been designed as a pragmatic, multicentre randomised trial to compare the effect

Using radiological brain health to predict recurrence after ischemic stroke and transient ischemic attack: A population-based study.

BACKGROUND AND OBJECTIVES: Approximately one-fourth of all strokes are recurrent, which is associated with a higher risk of morbidity/death. Identifying stroke survivors at greatest risk of recurrence could help direct secondary prevention efforts, but prior models based on clinical data have limited predictive ability. Neuroimaging markers of brain health may be uniquely able to capture the duration and severity of traditional and nontraditional risk factors for recurrence, and we examined whether they could improve the prediction of stroke recurrence beyond clinical factors alone using advan
