Theoretical model of the effect of combined glenohumeral bone defects on anterior shoulder instability: a finite element approach.

The presence of either a Hill-Sachs or a bony Bankart defect has been indicated as a possible cause of subluxation and anterior shoulder dislocation. Previous studies investigated only the effects of isolated humeral or glenoid defects on glenohumeral instability. We investigated the effects on shoulder stability of both glenoid and humeral defects in the glenohumeral joint. A computer-based finite element approach was used to model the joint. A generic model was developed for cartilage and bone of the glenoid and humerus, using previously published data, and experiments were analyzed using st

The effect of the conjoined tendon of the short head of the biceps and coracobrachialis on shoulder stability and kinematics during in-vitro simulation.

The kinematics and stability of the shoulder during in-vitro simulation are affected by the muscles chosen for simulation and their loads. Existing simulators have commonly actuated the rotator cuff and deltoids; however, the contribution of secondary muscles, such as those which form the conjoined tendon, are not well understood. The conjoined tendon consists of the origins of the short head of the biceps and coracobrachialis (SH&C), and is thought to produce an anterior stabilizing effect. This study investigated the effect of SH&C tension at four loading levels: 0, 5, 10, 15N. Our primary o

Testing procedures for SLAP lesions of the shoulder involving contraction and torsion of biceps long head and glenohumeral glides.

Testing procedures for SLAP lesions of the shoulder can combine resisted elbow flexion, forearm pronation and supination, and glenohumeral glides. These procedures reproduce symptoms by increasing biceps long head active tension or passive torsion, and by placing the shoulder in an unstable position. We compared activation of biceps long head and pain intensity, between supinated and pronated forearm positions, between different glides, and between individuals with and without shoulder impairment. A case control study. Twelve participants with suspected SLAP lesions and twelve with no history

A prospective study of shoulder pain in primary care: prevalence of imaged pathology and response to guided diagnostic blocks.

BACKGROUND: The prevalence of imaged pathology in primary care has received little attention and the relevance of identified pathology to symptoms remains unclear. This paper reports the prevalence of imaged pathology and the association between pathology and response to diagnostic blocks into the subacromial bursa (SAB), acromioclavicular joint (ACJ) and glenohumeral joint (GHJ). METHODS: Consecutive patients with shoulder pain recruited from primary care underwent standardised x-ray, diagnostic ultrasound scan and diagnostic injections of local anaesthetic into the SAB and ACJ. Subjects who

The influence of arm and shoulder position on the bear-hug, belly-press, and lift-off tests: an electromyographic study.

BACKGROUND: Clinical testing for the integrity of the subscapularis muscle includes the belly-press, lift-off, and bear-hug examinations. While these tests have been widely applied in clinical practice, there is considerable variation in arm positioning within each clinical examination. HYPOTHESIS: To determine the ideal arm and shoulder positions for isolating the subscapularis muscle while performing the bear-hug, belly-press, and lift-off tests. STUDY DESIGN: Controlled laboratory study. METHODS: The activity of 7 muscles was monitored in 20 healthy participants: upper and lower division

Referred pain from muscle trigger points in the masticatory and neck-shoulder musculature in women with temporomandibular disoders.

UNLABELLED: Our aim was to describe the referred pain patterns and size of areas of trigger points (TrPs) in the masticatory and neck-shoulder muscles of women with myofascial temporomandibular disorders (TMD). Twenty-five women with myofascial TMD and 25 healthy matched women participated. Bilateral temporalis, deep masseter, superficial masseter, sternocleidomastoid, upper trapezius and suboccipital muscles were examined for TrPs by an assessor blinded to the subjects' condition. TrPs were identified with manual palpation and categorized into active and latent according to proposed criteria.

Position and duration of immobilization after primary anterior shoulder dislocation: a systematic review and meta-analysis of the literature.

BACKGROUND: Immobilization after closed reduction has long been the standard treatment for primary anterior dislocation of the shoulder. To determine the optimum duration and position of immobilization to prevent recurrent dislocation, a systematic review of the relevant literature was conducted. METHODS: Of 2083 published studies that were identified by means of a literature review, nine Level-I and Level-II studies were systematically reviewed. The outcome of interest was recurrent dislocation. Additional calculations were performed by pooling data to identify the ideal length and position

Advanced shoulder joint tuberculosis treated with débridement and closed continuous irrigation and suction: a report of 2 cases.

Tuberculosis of the shoulder is rare and its diagnosis is likely to be delayed because its symptoms often resemble those of the primary frozen shoulder or rotator cuff syndrome. The standard treatment is principally antituberculous chemotherapy plus débridement, if necessary. In this report, we describe 2 patients with advanced shoulder joint tuberculosis that were treated by débridement and closed continuous irrigation and suction using normal saline followed by early range-of-motion exercise with satisfactory results. We emphasize that surgical débridement and closed continuous irrigation an

[Prediction of shoulder dystocia--combining foetal weight estimation by ultrasound and maternal risk factors--a solution for the dilemma?].

BACKGROUND: Although foetal macrosomia is a known risk factor for shoulder dystocia, foetal weight estimation by ultrasound is imprecise. METHODS: We analysed in a retrospective, single-centre study whether the combination of foetal sonographic parameters and maternal parameters could improve the prediction of foetal macrosomia and whether on the basis of this findings the prediction of shoulder dystocia in foetal macrosomia could be enhanced. Likelihood ratios for sonographic parameters have been assessed and have been used to calculate post-test-probabilities for fetal macrosomia. All vagin

Anterior dislocation of the shoulder associated with Bankart lesion in a patient with Poland's syndrome: a case report and review of the literature.

Poland's syndrome is a rare congenital entity characterized by unilateral partial or total hypoplasia of the major pectoralis muscle, breast and/or ipsilateral hand abnormalities. It has been reported in association with various structural and functional abnormalities. We report about a 23-year-old male kick-boxer with Poland's syndrome who presented in our department the history of two traumatic anterior shoulder dislocations due to boxing and self-reductions. Physical examination showed an instability of the left shoulder, and the MRI scans demonstrated a Bankart lesion. The patient had been

Pediatric lumbar Chance fractures in British Columbia: chart review and analysis of the use of shoulder restraints in MVAs.

Chance fractures of the skeletally immature spine classically occur in frontal motor vehicle accidents (MVAs) when the occupants are restrained by a lap belt only and undergo traumatic hyperflexion of the torso during the impact. We retrospectively examined all MVA-related Chance fractures at British Columbia's Children's Hospital since 1986, by collecting injury and seat-belt use information from chart data and imaging studies. Twenty-six patients were included in the study, 14 wore a lap belt only, seven wore a three-point restraint properly, and five were reportedly misusing the shoulder po

Rotator cuff tear associated with a posterior dislocation of the shoulder in a young adult: a case report and literature review.

Posterior glenohumeral dislocations are rare, and an accurate diagnosis is often difficult to make. Although rotator cuff tears are a known risk in the common traumatic anterior glenohumeral dislocation, they are an extremely rare event in association with posterior glenohumeral dislocation, regardless of patient age. There have only been 2 documented cases of rotator cuff tear following posterior shoulder dislocation previously reported in the literature. We report the case of a rotator cuff tear associated with a closed traumatic posterior dislocation of the shoulder in a 22-year-old male in

Neuromodulation of effects of upper limb motor function and shoulder range of motion by functional electric stimulation (FES).

Upper extremity motor impairment is a major contributing factor to functional disability of stroke patients. Functional electric stimulation (FES) is one of the therapeutic regimens for the management of upper extremity dysfunction after stroke. This review shows that therapeutic FES intervention on supraspinatus and posterior deltoid muscles for 6 weeks is effective to speed up upper limb motor recovery in hemiplegia of short-duration after stroke or less severely affected symptoms. The positive effect of FES could be attributable to neural mechanisms including: an enhanced information flow f
