A total of 829 subjects (females: 364 aged < 36 years and 360 aged > 35 years; males: 52 aged < 36 years and 53 aged > 35 years) employed on assistance and handling of patients in 42 geriatric residences in the Province of Trento underwent physical and anamnestic examination using a standardized method proposed by the EPM Research Unit for identification of shoulder disorders. The subjects who had reported the existence of pain lasting at least one week in the previous 12 months and occurring at least once a month and not due to trauma of the scapulo-humeral joint were defined as cases with po

Operative release for entrapment of the suprascapular nerve was carried out in 35 patients. They were assessed at an average of 30 months (12 to 98) after operation using the functional shoulder score devised by Constant and Murley. The average age at the time of surgery was 40 years (17 to 67). Entrapment was due to injury in ten patients and no cause was found in three; 34 had diffuse posterolateral shoulder pain. The strength of abduction was reduced in all the patients. The average Constant score, unadjusted for age or gender, before operative release was 47% (28 to 53). In 25 of the patie

BACKGROUND: In the United States, value-based purchasing has created the need for healthcare systems to prospectively identify patients at risk for high healthcare utilization beyond a physical therapy episode for musculoskeletal pain. The purpose of this study was to determine predictors of pain-related healthcare utilization subsequent to an index episode of physical therapy for musculoskeletal pain.
METHODS: This study assessed data from the Optimal Screening for Prediction of Referral and Outcome (OSPRO) longitudinal cohort study that recruited individuals with a primary complaint of neck

BACKGROUND: Chronic postoperative pain is a well-recognised problem. The incidence of severe incapacitating pain is about 3-5% after various types of surgery such as thoracotomy, repair of inguinal hernias and mastectomy. Sternotomy causes considerable postoperative pain and patients with chronic post-sternotomy pain are often referred to pain clinics. Epidemiological studies on chronic post-sternotomy pain are scarce, however. The aim of this paper was to study the incidence and possible risk factors of chronic pain following sternotomy operations performed for coronary bypass grafting or thy

Hepatitis E is an increasingly recognized cause of acute hepatitis in the Western world and is often complicated by extrahepatic manifestations, particularly neurological ones such as neuralgic amyotrophy (NA). NA is a disease of the brachial plexus, characterized by sudden onset of severe pain and muscle weakness, typically affecting the shoulders and upper limbs and can even extend to the diaphragm. Recent data suggest that its presence is linked to a HEV infection in 10% of the cases. Due to its rarity, there are currently no established treatment guidelines. Here we present a case of a 44-

Acupuncture treatment in local areas is commonly used to treat pain or soreness; however, acupuncture around the neck or shoulder may be a risk factor for pneumothorax. Herein, we report two cases of iatrogenic pneumothorax after acupuncture. These points indicate that physicians should be aware of these risk factors through history-taking before acupuncture. Chronic pulmonary diseases, such as chronic bronchitis, emphysema, tuberculosis, lung cancer, pneumonia, and thoracic surgery, may be associated with a higher risk of iatrogenic pneumothorax after acupuncture. Even if the incidence of pne

Polymyalgia rheumatica (PMR) is an inflammatory joint disease that presents in patients older than 50 years with prolonged morning pain and stiffness in the shoulder and hip joints and neck. The lack of specific clinical findings, laboratory signs, biomarkers and established imaging methods makes it difficult to diagnose patients with this disease. 18F-FDG PET/CT is a functional imaging technique that is an established tool in oncology and has also proven useful in the field of inflammatory diseases. The aim of this paper is to present literature evidence on the use of molecular imaging method

A 30-year-old healthy man presents after a fall with diffuse weakness and pain and recent onset left eye ptosis in the setting of three weeks of progressive bilateral leg weakness, gait instability, and difficulty washing his hair due to upper extremity weakness. He had also developed stiffness in his neck and shoulders and uncontrollable muscle spasms. Exam was notable for ptosis, fatiguing weakness in extremities, increased tone, hyperreflexia with clonus, and spastic gait. A mediastinal mass was found on chest CT (Fig. 1), and biopsy confirmed an invasive thymoma with positive nodes. Serum

The first aim of this study was to determine the prevalence of musculoskeletal symptoms. The second aim was to study the association between musculoskeletal symptoms and factors in radiation workers from the active lists of clinics and hospitals and then to provide basic data for a systematic and effective resource management of radiation workers' musculoskeletal problems in the future. A questionnaire survey was conducted on radiation workers in clinics, general hospitals, and Dong-A university hospitals around the Korean city Buscan from July 10 to 31, 2011. The results showed that the rate

The interscalene block (ISB) of the brachial plexus is a common technique for anaesthesia and the treatment of postoperative pain in shoulder surgery. Despite the well-known advantages of this technique, it is not without risks that need to be identified and taken into account. The most frequent complications associated with it are of a neurological nature and most are transient. It has been shown that the use of nerve stimulation does not guarantee a safe block. Ultrasound guidance now makes it possible to visualise potentially dangerous structures, thus allowing us to reduce the risk of asso

Polymyalgia rheumatica (PMR) is an inflammatory disorder that affects people over 50 years of age, characterized by pain and stiffness in the neck and shoulder and pelvic girdles. PMR may occur as an isolated condition or concomitantly with giant cell arteritis. Similar to other inflammatory rheumatic disorders, PMR is associated with an increased cardiovascular risk due to inflammatory changes in large arterial vessels, which result in an increased chance of developing aneurysms and dissections. International guidelines do not provide specific indications about the management of PMR patients

A 38-year-old man presented with pain in the left shoulder following a fall. A rotator cuff tear was suspected but roentgenograms revealed a lytic, expanded, multiseptate lesion of the scapula adjacent to glenoid. This was compatible with a giant cell tumor, aneurysmal bone cyst or simple bone cyst. The MRI showed rim enhancement and clinched the diagnosis of simple bone cyst. An infraspinous approach to scapula was undertaken and open biopsy confirmed the diagnosis of simple bone cyst. Definitive treatment in the form of curettage and calcium phosphate cementation was carried out. The postope

In the last decade a significant rise in the prevalence of tuberculosis as well as in its extrapulmonary manifestations is detected worldwide. The central nervous system, the genitourinary tract, the organs of the abdomen and the skeletal system, are common sites of infection. Misdiagnosis and delay in treatment are common events. Herein, we present a case of a 78-year-old man non-smoker, with miliary tuberculosis complicated with tuberculous spondylitis. The patient presented with anemia and a left shoulder pain, accompanied by rigor and fever 37.5 degrees C-38 degrees C of one month duration

Botulinum toxin (BoNT) has been used for over a quarter of century for the treatment of well over 100 different indications. Many of the symptoms for which BoNT has been found to be effective occur in a variety of neurological disorders. One neurodegenerative disorder in which BoNT has been used extensively to treat various symptoms is Parkinson's disease (PD). This review will highlight the following therapeutic applications of BoNT in conditions associated with PD: limb dystonia, blepharospasm and lid apraxia, bruxism, cervical dystonia (anterocollis), camptocormia, hand and jaw tremor, rigi

Elastofibroma dorsi, named for its characteristic location, is a benign soft tissue tumour occurring most often in the infrascapular region of elderly women. It has been infrequently reported in other anatomic locations. Elastofibroma is a rare non-encapsulated benign tumour characterised by the proliferation of elastin fibres in a stroma of collagenous and fatty connective tissue. This presents as an obvious swelling deep to the scapula, making the scapula prominent, causing periscapular pain, discomfort and loss of range of motion in the shoulder. This article presents a typical case and a b

A man aged 51 for the last 3 months had displayed general malaise, epigastric pain, nausea, vomiting and constipation. Also, he had a pseudo-athletic appearance with symmetrical large accumulations of fat on the front of the trunk, the lower back, the shoulders and the proximal extremities, characteristic of 'benign symmetrical lipomatosis'. He died of embolism of the aortic bifurcation and autopsy revealed an extensive adenocarcinoma in the upper abdomen, probably originating from the pancreas or the stomach. Benign symmetrical lipomatosis mostly occurs in middle-aged men. The pathogenesis is

Among the so-called cervical spine syndromes cervical disc prolapse represents a well defined disease entity which should be kept in mind considering treatment resistant painful neck and shoulder as well as neurological dysfunctions of this region. Differential therapy consists of conservative measures and especially a variety of surgical procedures for cervical disc syndromes. Under all circumstances decision for surgery has to take into consideration the individual psychological situation (e.g. expectations of the patient) and social factors (e.g. the patient's occupation). Therefore a close

AIM: This observational cohort study aimed to investigate the extent to which physical pain increases the long-term risk of suicidal behavior among construction workers.
METHODS: In 1989-93, 79,099 men from the Swedish construction workers cohort completed a questionnaire on working conditions and physical symptoms, including pain, in addition to their routine health examination. The participants assessed how often they had experienced pain in nine anatomical areas over the past year; this was summarized into four categories describing pain in the neck, shoulders and upper back; the lower bac

Cervical conjoined nerve root is rare, and medical imaging, such as magnetic resonance imaging and computed tomography, cannot give an accurate preoperative diagnosis.1 Treatment of cervical radiculopathy with root anomaly can be challenging. We report here a case of cervical conjoined nerve root with a 2-dimensional video. A 41-year-old woman without systemic disease presented with a 2-month history of neck and bilateral shoulder pain, upper back tightness, and left upper limb painful numbness, especially of the first to third fingers. The visual analog scale scores of the neck and left upper

A prevalent overuse injury among athletes, especially to those participating in sports like volleyball that demand repeated overhead motions, is biceps tendinopathy. The painful condition known as biceps brachii tendinopathy is characterized by changes in the structure of the tendon together with chronic degeneration. Furthermore, the biceps aid in the acceleration and deceleration of the arm in numerous overhead sports. The biceps may experience excessive strain as a result of poor training or exhaustion. It is commonly known that the long head of the biceps tendon plays a significant role in
