Carotid body chemoreceptors, sympathetic neural activation, and cardiometabolic disease.

The carotid body (CB) is the main peripheral chemoreceptor that senses the arterial PO2, PCO2 and pH. In response to hypoxemia, hypercapnia and acidosis, carotid chemosensory discharge elicits reflex respiratory, autonomic and cardiovascular adjustments. The classical construct considers the CB as the main peripheral oxygen sensor, triggering reflex physiological responses to acute hypoxemia and facilitating the ventilatory acclimation to chronic hypoxemia at high altitude. However, a growing body of experimental evidence supports the novel concept that an abnormally enhanced CB chemosensory i

A developmental study on types and frequency distribution of short apneas (3 to 15 seconds) in term and preterm infants.

We measured the frequency distribution and the ventilatory correlates of the various types of apneas 3 to 15 s long during sleep in eight term infants (birth weight 3.65 +/- 0.16 kg; gestational age 39.5 +/- 0.3 wk) and eight preterm infants (birth weight 2.07 +/- 0.18 kg; gestational age 34.3 +/- 0.4 wk). Each infant was studied on five to seven occasions from birth to 56 wk of postconceptual age using a modified flow-through system. Sixty-six paired epochs of quiet sleep (1163 min) and rapid eye movement sleep (829 min) were analyzed in term infants and 85 paired epochs of quiet sleep (1553

Healthy brain aging: what has sleep got to do with it?

Sleep plays an important role in learning, memory encoding, and cognition. Insufficient quantity or quality of sleep leads not only to short-term neurocognitive dysfunction but also to permanent changes to the central nervous system. Sleep disorders are common in the geriatric population. The hypoxemia and sleep fragmentation resulting from obstructive sleep apnea are the most likely pathophysiology responsible for damage to the brain. Because treatment of these sleep disorders can lead to improved cognitive function, it is becoming increasingly important for physicians to be able to correctly

Identifying respiratory events and artifacts using oscillatory impedance measurement in aPAP therapy.

Constant positive airway pressure (cPAP) is the predominant noninvasive therapy for the obstructive sleep apnea syndrome (OSAS). The average pressure thereby applied can be reduced by online-identification of respiratory events. Oscillatory impedance measurement as an indicator for airway patency can identify central and obstructive apneas and hypopneas. Our goal was to reduce the sensors needed, to auto-calibrate the signals to each individual patient and to rule out patient induced artifacts.

Radiographic Evaluation of Sleep-Disordered Breathing.

Sleep-disordered breathing and obstructive sleep apnea are becoming more prevalent in today's population. Management of these conditions can be difficult and this diagnosis is often overlooked by clinicians. An increased awareness and understanding of craniofacial structures and anatomic relationships can aid the clinician in identifying at-risk patients, and improve treatment outcomes. An airway review of 3-dimensional computed tomography imaging can identify (1) anatomic variations that contribute to obstructive airway complications, and (2) measurable dimensions to identify at risk patients

Pediatric Considerations for Dental Sleep Medicine.

Pediatric obstructive sleep apnea (OSA) is a serious medical condition with numerous health consequences. Dentists are well suited to recognize and provide medical referrals for pediatric patients at risk for OSA. Timely dental sleep medicine interventions may improve signs and symptoms of OSA in growing children. Orthodontic and dentofacial orthopedic treatment may decrease obstructive respiratory events in some pediatric patients. Palatal expansion may be part of a comprehensive orthodontic treatment plan to correct a malocclusion and treat OSA. Orthognathic surgery, mandibular advancement d

Postoperative sleep-disordered breathing in patients without preoperative sleep apnea.

BACKGROUND: Recently published data show that postoperative apnea-hypopnea index (AHI) is significantly increased in some patients without preoperative sleep apnea. These patients may be at risk of developing perioperative adverse events related to sleep-disordered breathing (SDB). The objective of this study was to investigate the incidence and predictors of postoperative moderate-to-severe SDB (AHI > 15 events/h) in patients without sleep apnea preoperatively. METHODS: In a prospective observational fashion, patients were invited to undergo sleep studies with a portable device (Embletta X10

Sleep Disordered Breathing in Children with Neuromuscular Disease.

Sleep disordered breathing (SDB) in children with neuromuscular disease (NMD) is more prevalent compared to the general population, and often manifests as sleep-related hypoventilation, sleep-related hypoxemia, obstructive sleep apnea, central sleep apnea, and/or disordered control of breathing. Other sleep problems include, sleep fragmentation, abnormal sleep architecture, and nocturnal seizures in certain neuromuscular diseases. The manifestation of sleep disordered breathing in children depends on the extent, type, and progression of neuromuscular weakness, and in some instances, may be the

Vigilance and automobile accidents in patients with sleep apnea or narcolepsy.

Patients with obstructive sleep apnea or narcolepsy report difficulty remaining alert and attentive. To detect impaired vigilance, we designed Steer Clear, a computer program simulating a long and monotonous highway drive that presents 780 obstacles in 30 min. Sixty-two patients with sleep apnea hit a higher percentage of obstacles (4.3 +/- 0.6% [SEM]) than 12 age- and sex-matched subjects without sleep apnea (1.4 +/- 0.3%; p < 0.05) and 10 age- and sex-matched volunteers (1.2 +/- 0.3%; p < 0.05). Ten patients with untreated narcolepsy hit a higher percentage of obstacles while performing on S

"Not Your Grandfather's CPAP": Improving Sleep Apnea Therapy Adherence With Emerging Technology.

Continuous positive airway pressure (CPAP) therapy is an effective treatment for obstructive sleep apnea (OSA), a condition that may affect up to 1 billion people worldwide. But discomfort with CPAP machines has kept many patients from complying with therapy. Now, companies are investing in machine learning, personalized pressure delivery algorithms, fabric masks, and digital health strategies to increase therapy adherence and help patients sleep more soundly.

Management of acute cardiovascular consequences of sleep disorders: clinical implications for improved patient outcomes.

INTRODUCTION: Sleep disorders such as insomnia, restless legs syndrome (RLS) and sleep disordered breathing such as obstructive sleep apnea (OSA) and central sleep apnea (CSA) are increasingly recognized as independent risk factors for acute cardiovascular events. AREAS COVERED: This review highlights the key mechanisms linking sleep disorders to acute cardiovascular events, including autonomic dysregulation, intermittent hypoxia, systemic inflammation, and endothelial dysfunction. These contribute to arrhythmias, acute coronary syndromes, heart failure exacerbations, and strokes. OSA stands

Association between sleep disorders and low muscle mass based on body composition metrics.

Investigating the relationship between body composition and sleep disorders, this cross-sectional study analyzed 990 patients (427 males, 563 females) at the Samsung Medical Sleep Center using polysomnography and bioelectrical impedance analysis. Sleep disorders were classified as primary insomnia (PI), comorbid insomnia and sleep apnea (COMISA), and obstructive sleep apnea (OSA) using ICSD-3 criteria, while sarcopenia was defined using gender-specific Skeletal Muscle Index (SMI) thresholds. The PI group showed the lowest SMI values across both sexes. Analysis revealed that among PI patients,

Cardiovascular outcomes in patients with COPD-OSA overlap syndrome: A systematic review and meta-analysis.

Chronic obstructive pulmonary disease (COPD) and obstructive sleep apnoea (OSA) are prevalent respiratory conditions that are independently associated with increased cardiovascular disease (CVD). It is not clear from current evidence whether COPD-OSA overlap syndrome confers an additive risk. This systematic review and meta-analysis investigated whether CVD was more prevalent in patients with overlap syndrome compared to either condition alone. We searched four electronic databases, screened 1826 records against the inclusion criteria. After screening, 18 retrospective, observational studies i

Sleep disordered breathing in the elderly.

Sleep disordered breathing (SDB), i.e., obstructive, central or mixed sleep apneas, has been recognized as a common occurrence in the elderly. Aging is per se associated with a decrease in the quality of sleep; SDB may further disrupt the sleep architecture in older subjects. The prevalence of obstructive sleep apnea (OSA) increases with aging; available studies report prevalence rates of 11-62%. Furthermore, OSA has been associated with increased mortality in older adults. Central apneas and periodic breathing occur with increased frequency either in subjects with neurological disorders such

Bedwetting--not always what it seems: a sign of sleep-disordered breathing in children.

ISSUES AND PURPOSE: Nocturnal enuresis (bedwetting) can linger long into childhood. Sleep research has documented that nocturia and bedwetting are symptoms of obstructive sleep apnea (OSA) in adults and that bedwetting is predictive of OSA in children. CONCLUSIONS: Nocturnal polyuria is a cardiovascular response to negative pressure breathing (inspiration against a closed glottis), which is characteristic of OSA. PRACTICE IMPLICATIONS: Evidence of nocturnal polyuria and sleep-disordered breathing are important signs of OSA, a serious but treatable condition.
