5-ALA-Induced Fluorescence in Leptomeningeal Dissemination of Spinal Malignant Glioma.

BACKGROUND: 5-Aminolevulinic acid (5-ALA)-fluorescence-guided resection of malignant glioma is well established in many neuro-oncology departments. In addition, the use of 5-ALA has been reported for cerebral metastases, meningioma, and spinal tumors. We report a case of a patient with a leptomeningeal spread of a K27M-mutated spinal anaplastic astrocytoma (World Health Organization° [WHO] III), which was detected by its faint 5-ALA-induced fluorescence. CASE DESCRIPTION: A 26-year-old female patient with an incomplete resection of a spinal astrocytoma (WHO II) at Th1-Th3 was treated with adj

Insertion of magnetically controlled growing rods in a patient with a diaphragmatic pacemaker: case report.

Phrenic stimulators offer an alternative to standard mechanical ventilation as well as the potential for ventilator independence in select patients with chronic respiratory failure. Young patients (< 10 years old) with high cervical spinal cord injuries often develop paralytic scoliosis due to loss of muscle tone caudal to their spinal cord lesion. Growing rod systems allow for stabilization of spinal deformity while permitting continued growth of the spine and thoracic cavity. Magnetically controlled growing rods (MCGRs) offer the advantage of noninvasive expansion, as opposed to the operativ

Fixed spaced stimulation restores adaptive plasticity within the spinal cord: Identifying the eliciting conditions.

Prior work has shown that neurons within the spinal cord are sensitive to temporal relations and that stimulus regularity impacts nociceptive processing and adaptive plasticity. Application of brief (80ms) shocks (180-900) in a variable manner induces a form of maladaptive plasticity that inhibits spinally-mediated learning and enhances nociceptive reactivity. In contrast, an extended exposure (720-900) to stimuli given at regular (fixed spaced) intervals has a restorative effect that counters nociceptive sensitization and enables learning. The present paper explores the stimulus parameters un

Myelin Basic Protein Cleaves Cell Adhesion Molecule L1 and Improves Regeneration After Injury.

Myelin basic protein (MBP) is a serine protease that cleaves neural cell adhesion molecule L1 and generates a transmembrane L1 fragment which facilitates L1-dependent functions in vitro, such as neurite outgrowth, neuronal cell migration and survival, myelination by Schwann cells as well as Schwann cell proliferation, migration, and process formation. Ablation and blocking of MBP or disruption of its proteolytic activity by mutation of a proteolytically active serine residue abolish L1-dependent cellular responses. In utero injection of adeno-associated virus encoding proteolytically active MB

The changing landscape of spinal cord injury.

In the past quarter century, spinal cord injury medicine has welcomed the proliferation of new medications and technologies that improve the survival and quality of life for people with spinal cord injury, but also endured the failure of strategies we hoped would salvage the cord in the acute phase. Surgical decompression and spinal stabilization should be pursued whenever indicated and feasible; however, there is no compelling evidence that early decompression facilitates neurological improvement. Methylprednisolone, the subject of over two decades of trials, has proven to be of marginal bene

[Distal hereditary motor neuropathy].

INTRODUCTION: Distal hereditary motor neuropathy (dHMN), also known as spinal muscular atrophy, represents a group of clinically and genetically heterogeneous diseases caused by degenerations of spinal motor neurons and leading to distal muscle weakness and wasting. Nerve conduction studies reveal a pure motor axonopathy and needle examination shows chronic denervation. STATE OF ART: dHMN were initially subdivided into seven subtypes according to mode of inheritance, age at onset, and clinical evolution. Recent studies have shown that these subtypes are still heterogeneous at the molecular ge

Functional regeneration of respiratory pathways after spinal cord injury.

Spinal cord injuries often occur at the cervical level above the phrenic motor pools, which innervate the diaphragm. The effects of impaired breathing are a leading cause of death from spinal cord injuries, underscoring the importance of developing strategies to restore respiratory activity. Here we show that, after cervical spinal cord injury, the expression of chondroitin sulphate proteoglycans (CSPGs) associated with the perineuronal net (PNN) is upregulated around the phrenic motor neurons. Digestion of these potently inhibitory extracellular matrix molecules with chondroitinase ABC (denot

Precision diagnostic disc injections.

Spinal pain is an important public health problem affecting the population indiscriminately. The structures responsible for pain in the spine include the vertebrae, intervertebral discs, spinal cord, nerve roots, facet joints, ligaments, muscles, atlanto-occipital joints, atlanto-axial joints, and sacroiliac joints. Even though disc herniation, facet joints, strained muscles, and torn ligaments have been attributed to be the cause of most spinal pain, either in the neck and upper extremities, upper and mid back, or low back and lower extremities, disorders of the disc other than disc herniatio

Surgical treatment and results in patients suffering from lumbar spinal stenoses.

Stenosis of the lumbar spinal canal is contributing to 3-5% of all patients operated on a lumbar nerve root compression syndrome. Morphologically, a reduction of the midsagittal diameter of the spinal canal to less than 12 mm or stenoses of the lateral recessus or foramen intervertebrale have been described. In our department 37 patients with a lumbar spinal stenosis have been surgically treated between 1982 and 1987. Spinal stenosis is a disease of aged patients mostly suffering for a long history. The main symptoms are lumbar and sciatic pains, neurological deficits and a claudicatio intermi

[Clinical experience with recently produced devices for use in spinal surgery].

The authors reported their clinical experience with devices, which have been recently introduced for spinal surgery. The main roles of such devices are to correct malalignment of the spine and stabilize the spinal instability. Although Harrington's device has been widely used since 1962, its use is gradually decreasing due to its having several weak points, such as its low resistance to distorting forces and its imperfect ability to correct malalignment of the spine. In the present manuscript, the authors describe clinical experience with three kinds of spinal devices. They are Steffee's trans

Studies of chlorpromazine-induced enhancement of the potentials evoked by peripheral stimulation of the cat cerebellum.

1) In the experiments reported here, we investigated effects of CPZ on the amplitude of cerebellar potentials using decerebrated or spinal cat; furthermore, we also examined the effects of microinjecting CPZ, DA or NE into the precerebellar nuclei on the amplitude of the cerebellar potentials. 2) Purkinje cell spontaneous discharge was either hardly changed or was depressed by CPZ. 3) CPZ did not change the potentials evoked by peripheral stimulation on the dorsal surface of the spinal cord. 4) CPZ depressed significantly the cerebellar potentials evoked by stimulation of the nucleus reticular

Glycine receptor distribution in mouse CNS: autoradiographic localization of [3H]strychnine binding sites.

Biochemical and electrophysiological studies of mammalian CNS indicate that the amino-acid, glycine, is a major inhibitory neurotransmitter whose location is, for the large part, confined to the spinal cord and brain stem. In this study, autoradiographs of C57BL/6J mouse brain sections labeled with [3H] strychnine, a potent antagonist of glycine, were used to map the distribution of glycine receptors in the CNS. Autoradiographs showed highly localized areas of grain density, which confirmed the gross distributions described in homogenate binding studies and gave a more precise regional localiz

Pathological studies on the ossification of the posterior longitudinal ligament (opll).

Ossification of the posterior longitudinal ligament (OPLL), which causes marked spinal cord compression, occurs more frequently in Japan than in other parts of the world. To investigate the mechanism of this ossification, we examined two cases of mixed type of OPLL inducing severe neurological signs due to spinal cord compression and three cases of segmental OPLL type without neurological signs. Ossification of the ligament begins at the upper end of posterior margin of the vertebral body and distends mainly downward along the posterior longitudinal ligament (PLL) and finally replaces the PLL

Extracellular matrix protein anosmin-1 regulates Schwann cell-astrocyte interaction for regenerative axon targeting in dorsal root crush injury model.

Schwann cell (SC) transplantation is considered as a promising strategy for spinal cord injury. However, SCs show less capability in assisting the regenerative axons to penetrate through astrocyte (AS)-formed scar barrier. Anosmin-1, an extracellular matrix glycosylated adhesion protein expressed in the olfactory bulb, is involved in olfactory ensheathing cells and reborn olfactory nerve axons continually penetrating the glial barrier and targeting the olfactory bulb. In this study, we employ a dorsal root crush injury model treated with anosmin-1. A vertical climbing test was used for behavio

Regulation of peripheral inflammation by the central nervous system.

In inflammatory disorders such as rheumatoid arthritis, cytokines and danger signals are sensed by the central nervous system, which adapts behavior and physiologic responses during systemic stress. The central nervous system can also signal the periphery to modulate inflammation through efferent hormonal and neuronal pathways. The brain and spinal cord are involved in this bidirectional interaction. A variety of neuronal pathways that modulate synovial inflammation have been implicated, including the sympathetic and the parasympathetic branches of the autonomic system. Another mechanism, the

Basivertebral Nerve Ablation: Part 1.

Vertebrogenic pain due to degenerated or damaged vertebral endplates is a common source of chronic low back pain.1 The pathogenesis of vertebrogenic pain is attributed to the presence of endplate nociceptors that transmit afferent signals to the basivertebral nerve (BVN), located posteriorly within the vertebral body.2,3 Chronic endplate inflammation triggers nociceptor proliferation, and associated chemical sensitization and mechanical stimulation lead to pain signals transmitted via the BVN and perceived as low back pain.1,2 Therefore, radiofrequency ablation of the BVN has emerged as a pote
