Grief, traumatic stress, and posttraumatic growth in women who have experienced pregnancy loss.

OBJECTIVE: Pregnancy loss is common and can be devastating for those who experience it. However, a historical focus on negative outcomes, and grief in particular, has rendered an incomplete portrait of both the gravity of the loss, and the potential for growth in its wake. Consistent with contemporary models of growth following bereavement, this study explored the occurrence of posttraumatic growth following pregnancy loss and further assessed the role of core belief disruptions and common loss context factors across perinatal grief, posttraumatic stress symptoms, and posttraumatic growth. On the Road to Translation for PTSD Treatment: Theoretical and Practical Considerations of the Use of Human Models of Conditioned Fear for Drug Development.

The use of quantitative, laboratory-based measures of threat in humans for proof-of-concept studies and target development for novel drug discovery has grown tremendously in the last 2 decades. In particular, in the field of posttraumatic stress disorder (PTSD), human models of fear conditioning have been critical in shaping our theoretical understanding of fear processes and importantly, validating findings from animal models of the neural substrates and signaling pathways required for these complex processes. Here, we will review the use of laboratory-based measures of fear processes in huma

The relationship between negative expressivity, anger, and PTSD symptom clusters.

More investigation is needed to understand how specific posttraumatic stress disorder (PTSD) symptom clusters relate to the internal experience of anger and overt negative behaviors in response to anger (negative expressivity). We investigated whether anger mediated relations between PTSD symptom clusters and negative expressivity. Multiple regression revealed lower PTSD intrusion symptoms associated with higher levels of negative expressivity. Anger mediated this relationship. Higher avoidance symptoms related to higher negative expressivity. Clinical implications, limitations, and strengths

Tonic immobility differentiates stress responses in PTSD.

BACKGROUND: Tonic immobility (TI) is a state of physical immobility associated with extreme stress and the development of posttraumatic stress disorder (PTSD). However, it is unknown whether TI is associated with a distinct actual stress response, i.e., objective immobility measured by a stabilometric platform. This study made a first step in exploring this as well as differences in body sway responses between PTSD patients and healthy controls. We hypothesized that PTSD would be related to increased body sway under stress, whereas TI would be related to decreased body sway under stress. METH

Psychosocial care to affected citizens and communities in case of CBRN incidents: a systematic review.

Disasters are associated with a substantial psychosocial burden for affected individuals (including first responders) and communities. Knowledge about how to address these risks and problems is valuable for societies worldwide. Decades of research into post-disaster psychosocial care has resulted in various recommendations and general guidelines. However, as CBRN (chemical, biological, radiological, nuclear) events form a distinctive theme in emergency planning and disaster preparedness, it is important to systematically explore their implications for psychosocial care. The aim of this study i

Does self-blame moderate psychological adjustment following intimate partner violence?

This study explored whether self-blame moderates the relationship between exposure to specific types of abuse and both poor general psychological adjustment (i.e., self-esteem) and specific symptomatology (i.e., posttraumatic stress disorder [PTSD]) among women who had experienced intimate partner violence (IPV). Eighty female IPV survivors were involved in this study. Results indicated that self-blame was negatively associated with self-esteem for physical, psychological, and sexual abuse. Self-blame moderated physical abuse, such that high levels of physical abuse interacted with high levels

Implicit and explicit memory in survivors of chronic interpersonal violence.

We investigated the relationship of implicit and explicit memory to a range of symptoms in a sample of 27 women with exposure to chronic interpersonal violence (IPV). Participants viewed the first 3 letters ("stems") of trauma-related, general threat, and neutral words; valenced words were matched with neutral words with the same stem. Free recall and a word-stem completion task were used to test explicit and implicit memory, respectively. Participants exhibited increased implicit memory for trauma-related words as compared with both general threat words and neutral "match" words. They also sh

The syndemic illness of HIV and trauma: implications for a trauma-informed model of care.

BACKGROUND: People living with HIV infection are disproportionately burdened by trauma and the resultant negative health consequences, making the combination of HIV infection and trauma a syndemic illness. Despite the high co-occurrence and negative influence on health, trauma and posttraumatic sequelae in people living with HIV infection often go unrecognized and untreated because of the current gaps in medical training and lack of practice guidelines. OBJECTIVE: We set out to review the current literature on HIV infection and trauma and propose a trauma-informed model of care to target this

Mental health response to community disasters: a systematic review.

IMPORTANCE: Exposure to a disaster is common, and one-third or more of individuals severely exposed may develop posttraumatic stress disorder or other disorders. A systematic approach to the delivery of timely and appropriate disaster mental health services may facilitate their integration into the emergency medical response. OBJECTIVE: To review and summarize the evidence for how best to identify individuals in need of disaster mental health services and triage them to appropriate care. EVIDENCE REVIEW: Search of the peer-reviewed English-language literature on disaster mental health respon

Social and academic functioning in adolescents with child sexual abuse-related PTSD.

OBJECTIVE: This study examined the relationship between posttraumatic stress disorder (PTSD) and functioning in academic and social domains. METHOD: Ninety treatment-seeking adolescent females with a history of child sexual abuse (CSA) completed a trauma history interview, the Child PTSD Symptom Scale – Interview, and the Child Behavior Checklist – Youth Self-Report. RESULTS: PTSD symptom severity was significantly related to poorer social competence, but was not significantly related to self-reported academic performance. The Avoidance subscale was significantly related to social functionin

A community study of the psychological effects of the Omagh car bomb on adults.

BACKGROUND: The main aims of the study were to assess psychological morbidity among adults nine months after a car bomb explosion in the town of Omagh, Northern Ireland and to identify predictors of chronic posttraumatic stress disorder symptoms. METHOD: A questionnaire was sent to all adults in households in The Omagh District Council area. The questionnaire comprised established predictors of PTSD (such as pre-trauma personal characteristics, type of exposure, initial emotional response and long-term adverse physical or financial problems), predictors derived from the Ehlers and Clark (2000

Girl who wrote to primo levi. The "foreign body" speaks.

Through the exposition of a clinical case, the author explores how a sexually abused woman can set up strategies for psychic survival to the feeling of catastrophe and how she can attempt to communicate and share it. The clinical case connects childhood sexual abuse trauma and the trauma of surviving concentration camps. The paper refers to two main achievements of Ferenczi's research: (1) the consequences of the traumatic experience and of the disavowal of it, such as dissociation, fragmentation, psychic agony; and (2) the focus on countertransference as emotional sharing and as receptiveness

Alterations in affective processing of attack images following September 11, 2001.

The events of September 11, 2001 created unprecedented uncertainty about safety in the United States and created an aftermath with significant psychological impact across the world. This study examined emotional information encoding in 31 healthy individuals whose stress response symptoms ranged from none to a moderate level shortly after the attacks as assessed by the Impact of Event Scale-Revised. Participants viewed attack-related, negative (but attack-irrelevant), and neutral images while their event-related brain potentials (ERPs) were recorded. Attack images elicited enhanced P300 relati

Challenges of welfare-to-work programs.

Governmental expectations of placement outcomes were often high given the severity of work barriers that were present in disabled welfare recipients in the United States [11]. The purpose of this article is to identify challenges that were inherent in welfare-to-work programs and solutions to these challenges. Although rich with opportunity, welfare-to-work programs presented the challenge of employing large numbers of difficult-to-employ individuals before their public assistance benefits expired\cite{5}. Many clients also had symptoms of Posttraumatic Stress Disorder (PTSD) secondary to chil

The patient observer: eye-movement desensitization and reprocessing for the treatment of posttraumatic stress following childbirth.

BACKGROUND: No standard intervention with proved effectiveness is available for women with posttraumatic stress following childbirth because of insufficient research. The objective of this paper was to evaluate the possibility of using eye-movement desensitization and reprocessing treatment for women with symptoms of posttraumatic stress disorder following childbirth. The treatment is internationally recognized as one of the interventions of choice for the condition, but little is known about its effects in women who experienced the delivery as traumatic. METHODS: Three women suffering from p

[Professional burden for anaesthesiologists: "life after death"].

The confrontation with lethal courses, either due to fate or treatment errors, is an inevitable factor in the professional life of anaesthesiologists working in the fields of pain and palliative medicine as well as in emergency and intensive medicine. Even those active in the central service of anaesthesia in the operating room are not strangers to this topic. Thus it is even more surprising that neither during undergraduate studies nor during continuing medical education are systematic aids or guidelines provided on how to cope with such situations. Furthermore, there are no structured recomm
