[Facial malformations and asymmetries caused by the first and second branchial arch syndrome].

The first and second branchial arch syndrome is a congenital affection, among whom one can range as well hereditary as non-hereditary entities. They always cause a badly mutilating facial asymmetry. In this article, the first and second branchial arch syndrome were divided in hemifacial and bilateral dysostoses. We pointed out the hemifacial microsomia, the most occurring entity in these series. From two case reports, we attempt to give a treatment synopsis of this syndrome, which is always done by a multi-disciplinary team, including dentists, orthodontists, maxillofacial surgeons and plastic

Not all inflammation in the right lower quadrant is appendicitis: a case report of Escherichia coli O157:H7 with a review of the literature.

Although significant work has been presented on this subject in pediatric, infectious disease, and epidemiologic literature, there is a noteworthy lack of information on Escherichia coli O157:H7 in any surgical journals. As this disease can present with signs and symptoms often ascribed to the acute abdomen, it is imperative that the general surgeon, pediatric surgeon, and colorectal surgeon are all familiar with this infection and its clinical ramifications. A case report followed by a review of the literature is presented.

Acquisition of surgical skills for endonasal skull base surgery: a training program.

INTRODUCTION: Endonasal surgery represents a radical change in the practice of cranial base surgery and requires the acquisition of new knowledge and surgical skills. The optimal training program for surgeons has not been established. METHODS: We reviewed our experience with endonasal cranial base surgery from 1998 to 2006 to develop a training plan for the acquisition of surgical skills. It consists of a modular and incremental approach to endonasal skull base surgery that is designed to train surgeons to function as a team, learn endoscopic skull base anatomy, and develop fundamental endosc

Can emergency general surgical referrals be reduced? A prospective study.

It has been proposed that early assessment by a senior surgeon would result in a significant reduction in the number of general surgical admissions. A prospective study of 290 surgical patients admitted to a busy district general hospital over a period of 1 calendar month has been performed to test this hypothesis. After admission, all patients were assessed by a senior surgeon who carried out triage for each patient. The commonest diagnoses in descending order of frequency were non-specific abdominal pain, appendicitis, diverticular disease, cholecystitis, head injury and pancreatitis. Twenty

Reconstruction and rehabilitation of short-range, high-velocity gunshot injury to the lower face: a case report.

War injuries can range from the most minor to the devastating and life-threatening. Multidisciplinary care is required for successful management of survivors. In the acute phase, care may involve emergency surgeons, anaesthetists, neurosurgeons, ophthalmic surgeons, vascular surgeons and ENT specialists in addition to the oral and maxillofacial surgeon. Afterwards, definitive treatment of facial hard and soft tissue gunshot injuries depends ultimately on the abilities and skills of the oral and maxillofacial surgeon and his appreciation of such injuries. The timing and sequence of the surgical

Clinical experience with a high definition exoscope system for surgery of pineal region lesions.

VITOM-90 (Karl Storz Endoscopy, Tuttlingen, Germany) is a new technology that can be used as an alternative to the operating microscope. We have found that this device substantially improves surgeon comfort during infra-tentorial supracerebellar approaches to pineal region masses, and now report our experiences. The VITOM-90 is a specially designed scope that is attached to a high definition (HD) digital camera and displayed on a HD video monitor. This system was utilized in five patients undergoing infratentorial supracerebellar approaches for pineal region lesions. Surgical outcomes and path

Preoperative laryngoscopy in thyroid surgery: Do patients' subjective voice complaints matter?

BACKGROUND: Although routine preoperative laryngoscopy has been standard practice for many thyroid surgeons, there is recent literature that supports selective laryngoscopy. We hypothesize that patients' preoperative voice complaints do not correlate well with abnormalities seen on preoperative laryngoscopy. METHODS: A retrospective chart review of a 3-year, single-surgeon experience was performed. Records of patients undergoing thyroid surgery were reviewed for patient voice complaints, prior neck surgery, surgeon-documented voice quality, and results of laryngoscopy. RESULTS: Of 464 patien

[Significant factors affecting the operation time for benign parotid tumor].

Tumors of the parotid gland are fairly frequent among head and neck tumors. Parotidectomy for benign tumors is necessary for diagnosis and therapy. Furthermore, acquiring the skill for this operation is a significant step in the training course of an otolaryngologist Thus, it is useful to analyze the operation time and determine the factors affecting it to evaluate the progress in the operation skill. To measure the operation time and determine the factors affecting it, 71 cases, including 43 cases of pleomorphic adenomas and 28 cases Warthin's tumor, were retrospectively reviewed to the deter

Difficulty in securing treatment for degenerative hip disease in a patient with Down syndrome: the gap remains open.

In 2002, the office of the U.S. surgeon general published a report detailing the discrepancies between the quality of healthcare afforded to persons with and without mental retardation. This article examines the case of a female resident of a developmental center with profound mental retardation due to Down syndrome and degenerative hip disease. Although she was in urgent need of a total hip replacement, the operation was denied or delayed by several different surgeons. Using a survey of physician attitudes, we examine several possible motivations behind the surgeons' reluctance to perform the

Laparoscopic cholecystectomy in situs inversus totalis: The importance of being left-handed.

Since laparoscopic cholecystectomy has become the standard procedure for the treatment of gallstone disease, several cases have been reported in patients with situs inversus. These cases require more technically demanding procedures due to the symmetrical disposition of the anatomy. Thus, handedness could influence the performance of these operations. The two of us (L.M.O.) and (J.M.B.), a right-handed and a left-handed surgeon, respectively, placed the instruments in reverse mode from that used in orthotopic patients. The right-handed surgeon felt more impairment when dissecting with his left

The beginnings of internal fixation of fractures in the pre-aseptic period 1797-1870.

The first successful internal fixation in the pre-aseptic period was performed by the Flemish surgeon Dominique Le Roy in 1796 in Antwerpen, using a gold wire in a patient with an open fracture of the lower leg. In the first half of nineteenth century, he was followed by a number of other surgeons, among them Achille Cléophase Flaubert, father of the famous novelist Gustave Flaubert, who treated both acute fractures and non-unions. Almost all operations resulted in suppuration, however, in many patients the fracture healed, while other cases ended in amputation or even death. Despite a series

A patient reads the medical literature.

A 45-year-old male with no significant past medical history presented with a lump in his right groin. Outpatient computed tomography imaging ordered by his primary care physician demonstrated moderate right inguinal hernia containing nonobstructed distal descending/proximal sigmoid colon. Surgical repair of the hernia was recommended and the patient was referred for consultation with a general surgeon. The patient then conducted a search of current medical literature and, upon reflection, refused to let the consulted surgeon operate upon him. After identifying a significantly more experienced

A Novel Method to Attain Highly Symmetric Oblique Mandibular Chin-Body Osteotomy.

UNLABELLED: Oblique mandibular chin-body osteotomy is a widely used narrowing genioplasty. However, the historic challenge is how to achieve a highly symmetric result of oblique mandibular chin-body osteotomy. Here, we present a novel and simple method to achieve a highly symmetrical mandibular chin-body osteotomy. METHOD: Ten consecutive patients underwent mandibular chinbody osteotomy used silicone triangle template technique. Patient and surgeon's satisfaction of the surgery and surgeon's confidence improvement with template during the surgery were measured. RESULT: All patients and surge

Congenital anomalies: a 25-year overview.

Expansion of the discipline of hand surgery and heightened interest in congenital problems have resulted in major advances in the treatment of congenital hand anomalies over the past 25 years. Increased experience with congenital anomalies of the hand has expanded the hand surgeon's knowledge of patterns and relationships between different anomalies resulting in new methods of classification and more logical approaches to treatment. The principles of treatment of the more common anomalies, such as syndactyly, established by prior generations of hand surgeons have been refined in details of tec

Development of convolutional neural networks for classification and characterization of proximal humerus fractures on computed tomography.

BACKGROUND: Agreement between surgeons on classification, characterization and choice of treatment for proximal humerus fractures (PHFs) is poor, leading to subjective surgical decision-making and poor intersurgeon reliability. Machine learning for classification and characterization of PHFs on radiographs performed insufficiently. More detailed three-dimensional configuration of PHFs on computed tomography (CT) scans may improve performance. This study aimed to 1) develop and internally validate a convolutional neural network on CT scans, 2) externally validate the model, and 3) compare chara

Can FEM-based preoperative planning for complex tibial plateau fractures improve surgical skills? A cadaver study.

BACKGROUND: Complex tibial plateau fractures continue to pose a significant challenge for surgeons. In recent years, the widespread use of CT imaging has led to new insights leading to novel classifications that facilitate 360° stabilization techniques. Visualization in 3D has improved both fracture reduction and surgical outcomes. This study investigated whether preoperative planning of complex tibial plateau fracture fixation via finite element modeling (FEM) could enhance the fixation performance achieved by experienced surgeons and potentially improve outcomes for less experienced surgeons
