A Novel Design of V-shaped Radial Forearm Free Flap Facilitates the Direct Closure of Donor Site Wound.

The radial forearm free flap (RFFF) is one of the commonly used flaps in the repair of head and neck soft tissue defects, especially small and medium-sized defects. The free skin grafts from abdomen are usually used to repair the RFFF donor site wound. This study aims to design a novel V-shaped RFFF, hoping that it might facilitate the direct closure of the forearm donor site wound. From August to December in 2019, 20 patients with oral cancers received radical surgeries, and V-shaped RFFFs were designed to repair the soft defects and the forearm donor site wound was directly closed. The patie

Wound Management Strategy for Treatment of Localized Cutaneous Leishmaniasis Using the TIME Framework.

INTRODUCTION: The current drugs available for the treatment of cutaneous leishmaniasis (CL) often cause several adverse events, and the risk-benefit ratio is low due to the risk of severe complications. Current treatment recommendations are based on data from areas endemic for leishmaniasis and are not always perfectly applicable, especially in cases of imported CL. Thus, it is crucial to assess the level of severity in each case to provide the most appropriate treatment modality. The World Health Organization recommends simple wound care (with unspecified strategies) or local therapy as first

The effects of ozone application on genotoxic damage and wound healing in bisphosphonate-applied human gingival fibroblast cells.

OBJECTIVES: Medication-related osteonecrosis of the jaws (MRONJ) is an extremely therapy-resistant disease involving the jaws especially following bisphosphonate treatment. Bisphosphonates accumulate in bone in concentrations sufficient to be directly toxic to the oral epithelium. Current therapeutic options are inadequate for the prevention and treatment of MRONJ. The aim of this study was to investigate effects of ozone gas plasma therapy on wound healing in bisphosphonate-applied human fibroblasts. MATERIAL AND METHODS: Human primary gingival fibroblasts were cultured. Cytotoxic concentrat

The influence of iron status and genetic polymorphisms in the HFE gene on the risk for postoperative complications after bariatric surgery: a prospective cohort study in 1,064 patients.

BACKGROUND: Gastric bypass surgery is a highly effective therapy for long-term weight loss in severely obese patients, but carries significant perioperative risks including infection, wound dehiscence, and leaks from staple breakdown. Iron status can affect immune function and wound healing, thus may influence peri-operative complications. Common mutations in the HFE gene, the gene responsible for the iron overload disorder hereditary hemochromatosis, may impact iron status. METHODS: We analyzed 1064 extremely obese Caucasian individuals who underwent open and laparoscopic Roux-n-Y gastric by

Is closed-suction drainage necessary for single-level lumbar decompression?: review of 560 cases.

BACKGROUND: Closed-suction drainage is commonly used for prevention of postoperative hematoma and associated neurologic compromise after lumbar decompression, but it remains unclear whether suction drainage reduces postoperative complications. QUESTIONS/PURPOSES: We evaluated the efficacy of closed-suction drainage in single-level lumbar decompression surgery. PATIENTS AND METHODS: We retrospectively reviewed 560 patients who underwent single-level lumbar decompression or discectomy. We routinely used closed-suction drainage in all spinal surgeries until July 2003, and thereafter, we did not

Fracture of subthalamic nucleus deep brain stimulation hardware as a result of compulsive manipulation: case report.

OBJECTIVE AND IMPORTANCE: Trichotillomania (TTM) is an impulse control disorder characterized by the recurrent pulling of one's hair resulting in noticeable hair loss. There has been no definite association drawn between Parkinson's disease (PD) and TTM, although there is a suggestion that obsessive-compulsive symptomatology may be more prevalent in left-side predominant PD. We believe that it is important to be aware of psychiatric comorbidities in the surgical treatment of PD, as they may significantly impact the postoperative course. CLINICAL PRESENTATION: We describe the case of a 58-year

Single-dose oral ciprofloxacin plus parenteral metronidazole for perioperative antibiotic prophylaxis in colorectal surgery.

From 1983 until 1990 our standard antibiotic prophylaxis in colorectal surgery has been 80 mg gentamicin plus 500 mg metronidazole, both administered intravenously at the beginning of anesthesia, and then 80 mg gentamicin every 8 h for 3 days. In this time period, 718 patients undergoing elective colorectal surgery by abdominal approach were treated with this prophylactic procedure. The mean wound infection rate was 13.4% with no significant differences between years. In the present study, including 100 adult patients, intravenous gentamicin has been replaced by a single oral dose of 750 mg ci

The use of a liposome-encapsulated 5-fluoroorotate for glaucoma surgery: I. Animal studies.

We have considered the wound-healing and tissue-repair process as it applies to glaucoma filtration surgery. Information derived from classical histologic studies of functioning and failed blebs has been considered in view of the new ideas based on modern cell biology concepts of the wound healing process. These evaluations provide a rationale for the use of adjunctive chemotherapy for glaucoma filtration surgery. A study is described using posterior-lip sclerectomies, an experimental monkey model, a liposomal-drug delivery system and the potent pyrimidine analog L5FO. Results of the clinical

Microbiological study of sternal osteomyelitis after median thoracotomy - a retrospective cohort study.

INTRODUCTION: Deep sternal wound infection is a rare but feared complication of median thoracotomies and is usually caused by microorganisms from the patient's skin or mucous membranes, the external environment, or iatrogenic procedures. The most common involved pathogens are Staphylococcus aureus, Staphylococcus epidermidis and gram-negative bacteria. We aimed to evaluate the microbiological spectrum of deep sternal wound infections in our institution and to establish diagnostic and treatment algorithms. METHODS: We retrospectively evaluated the patients with deep sternal wound infections at

Chronic Post-craniotomy Fistula Persisting for Seven Years in a Patient With Neurofibromatosis Type 2.

Chronic post-craniotomy draining fistulas are rare complications, particularly in patients who have received prior antibiotic therapy. Individuals with neurofibromatosis type 2 (NF2) may face heightened risk due to repeated cranial interventions, impaired wound healing, radiation exposure, and tumor burden. Persistent fistulas in this population are infrequently reported. We describe an unusual case of a chronic post-craniotomy draining fistula persisting for more than seven years in a patient with NF2 who had undergone multiple cranial surgeries and prior radiation therapy for tumor managemen

Case Report: Left Vocal Cord Palsy and Hoarseness-A Case of Mycotic Arch Aneurysm With Infective Endocarditis.

We present contrast-enhanced CT images of the neck and chest from a gentleman in his 70s who presented with fever, malaise, and left knee pain. He was diagnosed with septic arthritis and Staphylococcus aureus bacteremia. During admission, he developed new-onset hoarseness. Flexible laryngoscopy confirmed left vocal cord palsy. CT neck and thorax with contrast revealed a saccular aneurysm of the aortic arch (Standring, 2020; Netter, 2022; Paquette et al., 2012). Urgent surgical repair revealed an aortic abscess with rupture into the pericardial cavity. He was treated for infective endocarditis

Topical Oxygen Therapy and a Zinc-Containing Palatal Stent for Post-surgical Healing: A Case Report of a Large Irritation Fibroma of the Hard Palate.

Irritation fibroma is a common benign reactive lesion of the oral mucosa, typically resulting from chronic mechanical irritation. While generally small and asymptomatic, rare cases may exhibit atypical growth. This report presents a 37-year-old male patient with a 2.7 cm fibroma located on the hard palate. Complete surgical excision was performed, and histopathological examination confirmed the diagnosis of irritation fibroma with no evidence of dysplasia or malignancy. Postoperative management included the use of a chairside-fabricated zinc-infused palatal stent, combined with topical oxygen-

Managing Pyogenic Soft-Tissue Infections in People Living With HIV in a Resource-Limited Setting: Field Lessons and a Pragmatic Algorithm From South Sudan.

Severe pyogenic soft-tissue infections (SSTI) are a frequent cause of morbidity among people living with HIV (PLHIV) in resource-limited hospitals. Drawing on five months of frontline work in a district surgical unit in South Sudan, this field report distills practical lessons into a simple, resource-adapted algorithm for triage, source control, antibiotics, and wound care. Core steps include bedside sepsis screening with qSOFA, prompt empiric antibiotics aligned with the WHO EML/AWaRe approach, and decisive operative debridement without waiting for advanced diagnostics when necrotizing infect
