The authors report the long term results of His bundle ablation for supraventricular tachycardia in a series of 49 patients. This retrospective study was based on a patient population of 27 men and 22 women with an average age of 59 at the time of ablation, between 1984 and 1993. The results of radiofrequency catheter ablation of ventricular tachycardia (VT) in patients without structural heart disease are reported. We applied transcoronary chemical ablation of the atrioventricular (AV) node to a patient with uncontrollable paroxysmal supraventricular tachycardia (PSVT). Adenosine (adenine riboside), administered either as the free base or as the 5'-triphosphate (ATP) by rapid intravenous bolus, depresses atrioventricular (AV) nodal conduction, resulting in transient AV block. Adenosine is the active agent and ATP is rapidly converted to adenosine after exogenous administration. The occurrence of atrial fibrillation in patients with paroxysmal supraventricular tachycardia (PSVT) has been well documented when PSVT is secondary to atrioventricular reentry, but not when PSVT is secondary to atrioventricular nodal reentry (AVNRT). The purpose of this paper is to report our experience with endocardial catheter ablation in the recurrent episodes of paroxysmal supraventricular tachycardia in ten patients. All cases were referred because previous drug trials had failed to control arrhythmia. In four patients with Wolff Parkinson White syndrome, the ablation was used in the accessory pathway, in five patients in the his bundle region, and one patient in the AV perinodal region. The follow-up ranges from 4 to 15 months. We had a complete success rate of 40% and a partial success in other 40%. We did not have any complications

A 35-year-old female alcohol addicted ingested with suicidal intentions a single dose of 500 mg thioridazine. The course of intoxication was complicated by the development of polymorphous ventricular tachycardia of the torsade de pointes type, about six hours after ingestion. The dysrhythmia was immediately suppressed by stimulation from the right atrium. After 36 hours the stimulation could be abolished. Because of delirium tremens the patient with a normal ECG tracing was returned to the psychiatric clinic.

Twenty-four patients who underwent transvenous catheter ablation of the atrioventricular (A-V) junction between November 1982 and February 1987 were followed from 18-72 months (mean 47.9) to assess the long term efficacy and safety of the procedure.
