INTRODUCTION: Liver abscesses are rare in neonates with the majority resulting from an ascending infection via the umbilical and portal veins, haematogenous spread, or via the biliary tree, or via direct contiguous spread from neighbouring structures. They may present in unusual ways often presenting with ongoing sepsis and resulting in diagnostic difficulties. We present the clinical and radiological findings on six neonates with hepatic abscesses and underline the association with misplacement of umbilical line, association with hypertonic glucose infusions and TPN.
METHODS: A retrospective

To assist in making prognoses for patients with interstitial pneumonia (IP) who were treated with cyclosporin A (CsA), we conducted a review of forty-nine patients (32 men and 17 women with a mean age of 62 yrs) with progressive IP during the period from 1997 through 2001. All patients were steroid-resistant or acutely exacerbated cases. They received a low dosage of CsA (100-130 mg/day) combined with corticosteroids. Before and after the CsA therapy, blood gas analysis and HRCT scans were evaluated. Twenty-five patients underwent video-assisted thoracoscopic surgery (VATS) or autopsy for a hi

BACKGROUND: Pulmonary metastasectomy remains the standard of care for resectable metastatic osteosarcoma. However, controversy persists regarding minimally invasive surgery versus thoracotomy for occult nodule detection, the optimal use and duration of preoperative systemic therapy and prognostic factors associated with long-term survival. This study aimed to evaluate surgical approaches, assess the use and duration of preoperative systemic therapy, and identify prognostic factors associated with long-term survival in this population.
METHODS: At a single center, we retrospectively reviewed 1

OBJECTIVES: Malignant pleural effusions (MPEs) present a significant clinical challenge and are associated with a poor prognosis, frequently observed in patients with advanced malignancies. Conventional diagnostic techniques for identifying MPEs exhibit limitations in both accuracy and sensitivity. To differentiate between benign and MPEs, the clinical applicability of the ErNP@SiO2-ICG rare earth nano-nano near-infrared (NIR) fluorescence probe was investigated.
METHODS: Through solution chemistry processes, vacuum treatment with heat, and the synthesis of a core-shell design, an exceptional

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is currently the most common approach for pulmonary lobectomy in early-stage lung cancer. Reported advantages include less pain, fewer complications, faster recovery, and improved postoperative quality of life. The widespread adoption of VATS lobectomy is principally based on non-oncological benefits. Its oncological equivalence to open surgery remains assumed as no single trial has been powered for survival. To address this important question, we sought to conduct an individual patient data meta-analysis of eligible randomised trials.

BACKGROUND: Lymphatic dissemination is widely recognized as an important pathway in the progression of non-small cell lung cancer (NSCLC), and nodal involvement often accompanies more advanced thoracic spread (Rawal N. Eur J Anaesthesiol. 2016;33(3):160-71). Pleural involvement represents a distinct form of intrathoracic dissemination and is generally classified as M1a disease; when a primary tumor extends to or invades the pleura, tumor cells may gain access to the pleural cavity (Gehling M, Tryba M. Anaesthesia. 2009;64(6):643-51). Nevertheless, pleural lesions may occasionally be discovered

BACKGROUND: First-line treatment strategy for advanced non-small cell lung cancer (NSCLC) patients with concurrent driver mutations, high PD-L1 expression, and active autoimmune disease poses a significant challenge with limited guidance.
CASE PRESENTATION: A 41-year-old male with newly diagnosed active rheumatoid arthritis (RA) was found to have right lower lobe adenocarcinoma with ipsilateral interlobar pleural nodules (cT4N0M1a, stage IVA). Molecular profiling revealed a non-classical EGFR exon 18 mutation (E709_T710>del) with TP53 co-mutation and high PD-L1 expression (TPS 55%). Initial t

OBJECTIVES: Management of epithelial ovarian cancer presenting with moderate to large pleural effusion at first diagnosis is a clinical challenge. Several options are utilized with limited evidence including initial thoracoscopic evaluation, neoadjuvant chemotherapy (NACT) or some combination. We sought to evaluate the disease course and patterns of recurrence after triage to NACT.
METHODS: We included all clinical Stage IVA patients having moderate to large pleural effusions without radiologic evidence of a Stage IVB metastasis presenting to our institution between 2016 and 2021. Clinical ou

BACKGROUND: In recent years, robot-assisted thoracoscopic surgery has seen major advances. The feasibility and safety of this new surgical procedure have been widely recognized. However, only a few studies have investigated the short-term postoperative outcomes of lobectomy in early-stage non-small cell lung cancer (NSCLC) patients using different numbers of ports in Da Vinci robot-assisted surgery. This study aimed to evaluate the clinical value of robot-assisted lobectomy by comparing the perioperative data of NSCLC patients who underwent three-port and four-port surgical methods.

Pectus excavatum, the most common congenital chest wall deformity, is conventionally corrected using the minimally invasive Nuss procedure; however, postoperative pain remains a significant clinical challenge. Thoracic epidural analgesia (TEA) has traditionally been considered the gold standard, but it carries potential complications and technical challenges, particularly in patients with thoracic deformities. Alternative strategies, including the erector spinae plane block (ESPB) and intercostal nerve cryoablation, are therefore being explored to optimize pain control, reduce opioid consumpti

PURPOSE: Adolescent Idiopathic Scoliosis negatively impacts chest wall development. Bracing and fusion surgery have shown varied effects on pulmonary outcomes. Vertebral Body Tethering presents a growth-sparing alternative that might mitigate these effects by reducing biomechanical disruptions. Aim was to evaluate changes in pulmonary functions during the course of the follow-up after VBT surgery.
METHODS: Retrospective cohort study including patients who underwent Thoracic, Thoracolumbar and Bilateral VBT surgery with a minimum 24 months follow-up. Forced Vital Capacity (FVC%), Forced Expira

BACKGROUND: Spontaneous ventilation video-assisted thoracoscopic surgery (SV-VATS) has been propagated for nearly two decades without a prospective in-depth analysis of anaesthetic management and anaesthetic processing times. This would be important as anaesthetic management of SV-VATS imposes fundamental changes to standards in thoracic anaesthesia and may increase anaesthetic risks. Therefore, this study aimed to provide such in-depth analysis and compare the results to data from matched intubated VATS (I-VATS) patients. 3D-reconstruction of bronchial airways helped to estimate the risk redu

BACKGROUND: Patients of interstitial lung disease (ILD) combined with pulmonary lesions are increasingly common in clinical practice. Patients with ILD are at significantly higher risk for complications after pulmonary resection (including lobectomy and sublobar resection), especially acute exacerbations of ILD (AE-ILD). The purpose of this study is to summarize the short-term and long-term outcomes after pulmonary resection in ILD patients and to analyze the clinical factors affecting surgical safety.
METHODS: From January 2004 to January 2022, a total of 78 patients who were diagnosed with
