Predictive and prognostic role of Neutrophil-to-Lymphocyte ratio in patients with advanced thyroid cancer treated with targeted therapy.

BACKGROUND: Thyroid cancer represents the most common endocrine malignancy. While targeted therapies have improved clinical outcomes in advanced stages, substantial interpatient heterogeneity in treatment response highlights the critical need for robust predictive biomarkers. The neutrophil-to-lymphocyte ratio (NLR), a hematological parameter reflecting systemic inflammatory status and immune homeostasis, has emerged as a promising prognostic indicator across various solid tumors. In this study, we conducted a comprehensive evaluation of NLR’s predictive capacity for targeted therapeutic effic

Combined analysis of GAD65 and ICA512(IA-2) autoantibodies in organ and non-organ-specific autoimmune diseases confers high specificity for insulin-dependent diabetes mellitus.

There is evidence that insulin-dependent diabetes mellitus (IDDM) may develop in association with other non-beta-cell-specific autoimmune diseases. We aimed to assess whether autoantibodies to the islet cell antigens glutamic acid decarboxylase (Mr 65,000 isoform) (GAD65) and ICA512(IA-2), present alone or in combination, are limited to IDDM or also occur in other organ- or non-organ-specific autoimmune disorders. We determined the frequency of these autoantibodies by radioimmunoassay in 199 sera from patients with autoimmune thyroid diseases (AITD), rheumatoid arthritis (RA), systemic lupus e…

[Erythrocyte carbonic anhydrase I and zinc concentrations in thyrotoxicosis reflect integrated thyroid hormone levels over the previous few months].

In the present review, the clinical utility of determining red blood cell (RBC) carbonic anhydrase I isozyme (CA1) and zinc(Zn) concentrations in patients with various forms of thyroid disease is discussed. RBC CAI and Zn concentrations were both decreased in patients with hyperthyroid Graves' disease. After treatment, the normalization of RBC CA1 and Zn lagged two months behind the normalization of plasma thyroxine (T4) and triiodothyronine (T3) levels. Furthermore, the highest correlation coefficients were observed between RBC CA1 and Zn levels, and plasma thyroid hormone levels measured eig

Indium-111-labelled octreotide scintigraphy in the diagnosis and management of non-iodine avid metastatic carcinoma of the thyroid.

Treatment of differentiated thyroid cancer is a success of modern medicine with the use of radioiodine ((131)I). However, a significant proportion of thyroid cancers may be non-iodine avid. Thyroid tumours are known to express somatostatin receptors. Octreotide, an analogue of somatostatin, can be combined with a radioactive isotope, such as (111)In-DTPA(0) to visualise tumours with high concentrations of somatostatin receptors. We assessed 18 patients with histologically proven metastatic or locally recurrent non-iodine avid thyroid carcinoma to determine the usefulness of (111)In-DTPA(0) oct

Thyroid disorders in children from birth to adolescence.

Thyroid hormones play a crucial role as a regulator of growth, of nervous system myelination, of metabolism, and of organ functions. Disorders affecting the thyroid gland represent the most common endocrinopathies in childhood. The etiology and clinical presentation of thyroid disorders in children and adolescents substantially differ from that in adults. Thus, pediatric medical care requires an appreciation of distinct characteristics of thyroid function and dysfunction in childhood and adolescence. Early diagnosis and treatment are essential to prevent irreversible and permanent nervous syst

Update on interventional techniques for thyroid and parathyroid pathologies.

Over the past two decades, interventional techniques in cervical endocrine pathology have evolved significantly, offering less invasive alternatives to conventional surgery. This update reviews the latest advances in non-surgical approaches, including percutaneous ethanol injection (PEI) and thermal ablation (TA), with a focus on their indications, efficacy, and safety profiles. PEI remains the treatment of choice for cystic thyroid nodules. The technique involves ethanol-induced tissue necrosis, leading to significant nodule shrinkage. PEI is simple to perform, with a low risk of major compli

Double Filtration Plasmapheresis for Thyroid Storm With Encephalopathy: A Case Report.

Thyroid storm is a life-threatening endocrine emergency with a mortality rate of up to 17%. Standard treatment includes antithyroid drugs (ATD), β-blockers, and glucocorticoids. However, some patients may require blood purification therapies, such as plasmapheresis, due to the ineffectiveness of standard treatments or contraindications like agranulocytosis. We report the case of a 38-year-old female with Graves' disease who was admitted for thyroid storm (Burch-Wartofsky score of 70) combined with encephalopathy, presenting with impaired consciousness, seizures, and leukopenia (1.82 × 109/L).

Fetal thyroid function: diagnosis and management of fetal thyroid disorders.

The fetal hypothalamic-pituitary-thyroid axis develops independently of the maternal axis, but it is dependent on the maternal-placental system for adequate supply of iodide substrate. This iodide is supplied by direct transfer of maternal plasma iodide and by placental deiodination of T4. In addition, although placental transport of iodothyronines is limited, significant maternal-fetal transfer of T4 occurs, accounting for approximately 30% of the average 10 ug/dL serum-T4 concentration in fetal-cord blood at term. Current information suggests that this maternal contribution to the fetal-T4 l

Thyroid hormone therapy in a population cohort over two decades indicates persistent overtreatment.

OBJECTIVE: Thyroid hormones (THs) are among the most prescribed medications worldwide, and regular monitoring of thyroid function is required to ensure optimal management. Both over- and undertreatment with THs is associated with health risks, including increased cardiovascular morbidity and mortality, as well as higher socioeconomic costs. We investigated how changes in treatment concepts and guideline recommendations over two decades have influenced the frequency and quality of TH treatment in a high-income country. METHODS: Data from 7,711 participants in the German population-based Heinz

[Immunologic and cytologic aspects of Hashimoto's thyroiditis during therapy with thymopentin].

Hashimoto's thyroiditis is an autoimmune disease characterized by the presence of thyroid autoantibodies and frequent coexistence of other autoimmune disorders. The object of our research was to examine the peripheral blood and fine-needle cytology modifications in patients with this disease during therapy with Timopentina. Our results suggested a possible therapeutic effect of Timopentina, as an alternative to traditional cortisone treatment.

Bidirectional negative relationship between thyrotropin and kidney function during alcohol intoxication in males.

INTRODUCTION: Despite a well-established direct toxic effect of alcohol on renal cells, there is a salutary dose-dependent effect of alcohol consumption on common laboratory parameters related to kidney performance. Alcohol also impacts thyroid hormones, while thyroid status modulates kidney function. The modulation of kidney parameters with thyrotropin (TSH) and thyroid status indicates a possible interaction between alcohol, kidney, and thyroid functions. This retrospective study was conducted to test the hypothesis that the positive effect of alcohol use on the estimated glomerular filtrati

Preoperative embolization of thyroid arteries in a patient with a large cervicomediastinal hyperfunctioning goiter.

We present the case of a 61-year-old male patient with chronic obstructive pulmonary disease and a large cervicomediastinal multinodular hyperfunctioning goiter that made thyroidectomy a daunting task, especially considering the risk of intraoperative bleeding. The preoperative computed tomographic (CT) scan showed the right thyroid lobe very deeply rooted in the mediastinum, below the horizontal plane passing through the aortic arch. The thyroid mass imprinted the arterious and venous innominate trunks. To avoid a median sternotomy and remove the enlarged thyroid through a cervical approach a

The Effect of Apalutamide on Thyroid Function in Prostate Cancer Patients.

CONTEXT: Apalutamide (APT) is a nonsteroidal antiandrogen medication used to treat metastatic castrate-sensitive and nonmetastatic castrate-resistant prostate cancer. Early clinical trials of APT identified thyroid dysfunction as a common adverse effect of therapy, but the clinical presentation and management of APT-induced hypothyroidism has not been studied. OBJECTIVE: The objective of our study is to elucidate the clinical presentation and treatment approach of APT-associated thyroid dysfunction in prostate cancer patients. METHODS: We report a case series of 16 patients with APT-associat

Papillary thyroid carcinoma arising in ectopic thyroid tissue within sternocleidomastoid muscle: a review of current literature.

The prevalence of ectopic thyroid tissue as consequence of an aberrant migration of thyroid during embryogenesis ranges up to 10% in autopsy studies. The differential diagnosis between the relatively rare occurrence of a primary carcinoma arising in ectopic thyroid tissue and the more frequent presence of cervical lymph node metastasis from papillary thyroid carcinoma (PTC) might represent a difficult challenge in the clinical practice. The clinical relevance of these lesions lies in their risk of hidden primary thyroid cancer. Our intention is to provide in this review the current limited dat

Burkitt-like lymphoma infiltrating a hyperfunctioning thyroid adenoma and presenting as a hot nodule.

BACKGROUND: Most solitary hyperfunctiong regions on thyroid scan consist of benign tissue. Here we report a patient with a Burkitt-like lymphoma that was infilterated into a region containing a hyperfunctioning nodule. SUMMARY: A 56-year-old man was referred to our Endocrine Unit in May 2009 due to the incidental discovery of a large left thyroid lobe nodule by a computed tomography study. This had been performed to search for a primitive tumor in a patient with bone metastasis. He was clinically and biochemically thyrotoxic with no evidence of humoral thyroid autoimmunity. The nodule had a d

Thyroid hormones and energy metabolism in amyotrophic lateral sclerosis.

Weight loss, partially caused by hypermetabolism, represents a well-documented and therapeutically relevant feature of the amyotrophic lateral sclerosis phenotype worldwide. In this study, we retrospectively analysed the association between thyroid function and clinical, prognostic and metabolic parameters in a cohort of patients with amyotrophic lateral sclerosis in an experienced centre in Germany (n = 1754). Specifically, we examined the relationship between thyroid stimulating hormone levels, age, glucose and body mass index and-in subgroups-phosphorylated neurofilament heavy chain levels

DICER1 and DGCR8 in thyroid tumorigenesis: miRNA biogenesis and histopathologic diversity.

This review examines the emerging roles of DICER1 and DGCR8, key components of the miRNA biogenesis pathway, in thyroid pathogenesis, with a particular focus on their association with oncocytic morphology. Recent findings have expanded our understanding of DICER1 syndrome and DGCR8-related thyroid disorders, revealing a broader spectrum of thyroid lesions associated with mutations in these genes than previously recognised. We analyse the current literature on DICER1 and DGCR8 mutations in thyroid pathology, synthesising data from both basic science and pathological studies. The review explores
