The effect of associated injuries, blood loss, and oxygen debt on death and disability in blunt traumatic brain injury: the need for early physiologic predictors of severity.

Studies of 4590 patients with blunt trauma injuries admitted to a Level I Trauma Center, have shown that 37% had a blunt traumatic brain injury (BTBI). Of these brain injured patients 60% has an associated other injury. Examination of mortality has shown that those with an isolated brain injury had an 11% mortality compared with 21.8% in those with an associated systemic injury. Further investigation demonstrated that the cause of the increased mortality was related to the blood loss associated with the injuries and that when hypovolemic shock resulted, mortality rose from 12.8 to 62%. The sev

Prophylactic vena cava filter insertion in patients with traumatic spinal cord injury: preliminary results.

Pulmonary embolism (PE) is a devastating complication in patients with traumatic spinal cord injury (SCI). Prophylactic measures such as venous compression hose or low-dose heparin are only partially protective in reducing the risk of venous thromboembolism and are contraindicated in some patients. Because of extended perturbations in fibrinolytic activity, catecholamine effects on platelet aggregation, increased activity of complement and acute phase reactants, abnormally high factor VIII concentrations, and persistent venous stasis with ongoing endothelial damage, the patient with an SCI rem

Isolated head injuries versus multiple trauma in pediatric patients: do the same indications for cervical spine evaluation apply?

Although the recommendations to evaluate possible cervical spine injuries in patients with isolated head injuries are identical to those with multiple trauma, to date, no such study has confirmed that risk factors are the same for both injuries. We reviewed the charts of 268 pediatric patients with isolated head injuries admitted to the Intensive Care Unit at Children's Hospital Medical Center (1985-1990) to determine which risk factors were present. In this retrospective study, we divided the patients into two groups: low risk and high risk. The low-risk patients (n = 135) were those capable

Are arteriograms necessary in penetrating zone II neck injuries?

The evaluation and management of potential arterial injuries in penetrating neck trauma are controversial. Routine surgical exploration or arteriography can be very expensive and time-consuming and can overburden available resources if used in all patients. We reviewed the records of 4035 patients seen in our trauma center during a 20-month period and identified a total of 110 patients (2.7%) with penetrating wounds to zone II of the neck; 50 were from gunshot wounds, 43 from stab wounds, 7 from shotgun injuries, and 10 from lacerations. In 42 (39%) patients there was no arteriogram or surgery

Incidence and treatment of deviation of nasal septum in newborns.

Four thousand and ninety consecutive newborns were investigated for nasal septal deviations over a two-year period in the Bnai Zion Medical Center, Haifa. An incidence of 0.93% of anterior nasal septal cartilaginous dislocation was found. No statistically significant correlation was observed between the newborns' weight and the nasal deformities. Definite correlation between the type of delivery and the nasal deformity was noted, however. The importance of early treatment during the first three days after birth is stressed. The infants were followed for a three-year period and no evidence of r

Defining Posttraumatic Sepsis for Population-Level Research.

IMPORTANCE: Multiple classification methods are used to identify sepsis from existing data. In the trauma population, it is unknown how administrative methods compare with clinical criteria for sepsis classification. OBJECTIVES: To characterize the agreement between 3 approaches to sepsis classification among critically ill patients with trauma and compare the sepsis-associated risk of adverse outcomes when each method was used to define sepsis. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used data collected between January 1, 2012, and December 31, 2020, from patients

Reversal of trauma-induced coagulopathy using first-line coagulation factor concentrates or fresh frozen plasma (RETIC): a single-centre, parallel-group, open-label, randomised trial.

BACKGROUND: Effective treatment of trauma-induced coagulopathy is important; however, the optimal therapy is still not known. We aimed to compare the efficacy of first-line therapy using fresh frozen plasma (FFP) or coagulation factor concentrates (CFC) for the reversal of trauma-induced coagulopathy, the arising transfusion requirements, and consequently the development of multiple organ failure. METHODS: This single-centre, parallel-group, open-label, randomised trial was done at the Level 1 Trauma Center in Innsbruck Medical University Hospital (Innsbruck, Austria). Patients with trauma ag

Time Course and Diagnostic Accuracy of Glial and Neuronal Blood Biomarkers GFAP and UCH-L1 in a Large Cohort of Trauma Patients With and Without Mild Traumatic Brain Injury.

IMPORTANCE: Glial fibrillary acidic protein (GFAP) and ubiquitin C-terminal hydrolase L1 (UCH-L1) have been widely studied and show promise for clinical usefulness in suspected traumatic brain injury (TBI) and concussion. Understanding their diagnostic accuracy over time will help translate them into clinical practice. OBJECTIVES: To evaluate the temporal profiles of GFAP and UCH-L1 in a large cohort of trauma patients seen at the emergency department and to assess their diagnostic accuracy over time, both individually and in combination, for detecting mild to moderate TBI (MMTBI), traumatic

Psoas impingement causing labrum tear: a series from three tertiary hip arthroscopy centers.

BACKGROUND: The term hip impingement is usually associated with psoas impingement after arthroplasty or femoroacetabular impingement (FAI). A recently, less commonly described mechanism of impingement and labrum pathology is the psoas tendon applying pressure to the acetabular labrum more medial to the typical FAI labrum lesion. Much is still unkown about the anatomy, pathology, and treatment of this entity. PURPOSE: This paper describes the successful arthroscopic treatment of a series of patients with a recently recognized cause of hip pain in the young athletic population without significa

Alzheimer's disease: etiologies, pathophysiology, cognitive reserve, and treatment opportunities.

Alzheimer's disease (AD) can be diagnosed with a considerable degree of accuracy. In some centers, clinical diagnosis predicts the autopsy diagnosis with 90% certainty in series reported from academic centers. The characteristic histopathologic changes at autopsy include neurofibrillary tangles, neuritic plaques, neuronal loss, and amyloid angiopathy. Mutations on chromosomes 21, 14, and 1 cause familial AD. Risk factors for AD include advanced age, lower intelligence, small head size, and history of head trauma; female gender may confer additional risks. Susceptibility genes do not cause the

Traumatic Brain Injury in Female Intimate Partner Violence Survivors: Incidence, Sociodemographic Disparities, and Clinical Outcomes.

Intimate partner violence (IPV) represents a public health crisis in the United States. Experiencing IPV is associated with various adverse health outcomes and injuries, including traumatic brain injury (TBI). Improving screening and care for individuals affected by concurrent IPV and traumatic brain injury (IPV-TBI) requires understanding its incidence, sociodemographics, risk factors, and clinical outcomes. Using standardized multicenter data from the US National Trauma Data Bank years 2018 to 2022, this study aimed to answer the following questions: (a) What is the incidence of IPV-TBI amon

Fresh frozen plasma-first approach is independently associated with improved survival in severely injured patients undergoing massive transfusion.

BACKGROUND: Despite evidence supporting high fresh frozen plasma (FFP)-to-packed red blood cell (PRBC) ratios, the optimal initial blood product in severe injury is unknown. We hypothesized that an FFP-first approach is associated with improved survival. METHODS: An observational Trauma Quality Improvement Program (2013-2021), including patients who received at least 5 units of PRBCs and 1 unit of FFP within four hours of arrival, was performed. Nonsurvivable injury patterns and pre-existing coagulopathy were excluded. Treatment effects were estimated with propensity score-weighted risk adjus

A retrospective review of patients admitted to a tertiary intensive care unit following the February 6th earthquake in Türkiye.

BACKGROUND: The Kahramanmaraş earthquake that occurred in Türkiye in 2023 affected 14 million people, resulting in 53,537 deaths and more than 100,000 injuries. In large-scale disasters, it is crucial to rapidly initiate complex treatment processes in hospitals equipped with tertiary intensive care units following the initial on-site response. In this study, we aimed to retrospectively analyze the types of trauma and the associated surgical and medical treatments of patients affected by the earthquake who were admitted to our level 3 intensive care unit. Our objective was to contribute to futu

Jael syndrome, or the nasty arrow.

The term "Jael syndrome" is a symbolic analogy referring to penetrating craniofacial trauma caused, intentionally or accidentally, by blade or similar weapon in which the foreign body is kept in-situ during initial management. It is rare in clinical and medicolegal practice, but is potentially life-threatening or with neurologic and functional impact. Its name comes from an episode in the Book of Judges in the Hebrew Bible (Old Testament) in which Jael, a woman, kills the Canaanite general Sisera by hammering a tent peg through his temple while he is asleep, thus ensuring the liberation of the

Operative time in acetabular fracture fixation: predictors and impact on postoperative outcomes.

BACKGROUND: Surgical fixation of acetabular fractures is technically demanding, and operative time is often treated as a surrogate for surgical complexity and a presumed driver of complications. Whether prolonged operative time reflects identifiable preoperative factors, and whether it independently worsens outcomes, remains unclear. We aimed to identify factors associated with prolonged operative time in acetabular fracture fixation and to determine whether longer operative time is associated with worse postoperative outcomes. METHODS: We retrospectively reviewed patients undergoing surgical

Temporal Effects on Intra-operative Bacterial Contamination of Surgical Gloves in Non-Infection Orthopaedic Trauma Cases.

INTRODUCTION: Surgical site infections (SSIs) are among the most serious complications in Orthopaedic trauma surgery, contributing to patient morbidity, mortality, and substantial financial costs. With direct inoculation from intra-operative bacterial contamination being a large contributor to these infections, the temporal dynamics of intra-operative bacterial colonization on surgical gloves in Orthopaedic trauma cases was investigated. METHODS: Surgical glove bacterial cultures were taken by firmly pressing the operating room staff members gloved fingertips onto a sheep's blood agar plate.
