New technique for finding the ruptured bile duct into the liver cysts: scope in the cave technique.

Communication with the biliary tree is the most frequent complication of hepatic hydatid disease. This may result in fistula formation after surgical management of liver hydatidosis. Although frank ruptures are usually diagnosed preoperatively and managed surgically, simple openings may result in biliary fistula formation. It is very difficult to accurately diagnose a small hole in the cyst during surgery in most patients. We describe an easy and reliable technique for finding the point of communication via direct visualization using a telescope during conservative surgery for hepatic hydatido

Bronchoesophageal fistula in an 11-month-old boy.

We report a case of a rare type of bronchoesophageal fistula that communicated with the patient's normal bronchial tree, but had systemic arterial supply. Although included within the spectrum of bronchopulmonary foregut malformations, only three other similar cases have been reported in the literature. At the time of presentation, the 11-month-old male patient was afebrile with coarse breath sounds at the left lung base. A barium swallow demonstrated the connection between the distal esophagus and a left lower lobe bronchus. CT also helped to delineate the course of the fistula.

Carcinoid tumors of the orbit: a dilemma of diagnosis and treatment.

Carcinoid tumors arise from the gastrointestinal tract or the bronchial tree, and tumors of either origin produce symptoms locally or secrete amines that produce a carcinoid syndrome systemically. Metastatic carcinoid tumors to the orbit are rare and, thus, present a special problem in diagnosis and treatment. We describe a case of metastatic orbital carcinoid that presented itself 6 years prior to the discovery of the primary ileal lesion. This case report describes the unusual presentation, reviews previously published data, and addresses controversial opinions regarding therapy.

Pathophysiology of congenital diaphragmatic hernia.

The patient born with congenital diaphragmatic hernia (CDH) presents with a multitude of problems. The pulmonary hypoplasia associated with CDH results in an inadequate surface area for gas exchange, decreased cross sectional area of the pulmonary vascular tree, precocious muscularization of the pulmonary vessels, and probable surfactant deficiency. Elevated levels of circulating vasoactive mediators, as well as a heightened sensitivity to these mediators, further compounds the pulmonary hypertension associated with CDH. We have reviewed the pathophysiology of CDH, with specific reference to h

[Digestive hemorrhage undiagnosed by conventional endoscopy].

Digestive bleeding undiagnosed by conventional endoscopies occurs in about 5% of the patients with gastro-intestinal bleeding. Vascular ectasias are the most common cause. More than half of these tumors are benign. Meckel's diverticulum is the cause of bleeding in two thirds of men under the age of 30. Hemorrhage from the biliary tree or pancreatic duct occurs more often in patients with a suggestive history. According to the abundance and the cause of hemorrhage numerous investigations can be undertaken and the diagnostic value of new endoscopic methods (enteroscopy, endoscopic ultrasonograph

Five liability claims involving the cornea and how they could have been prevented.

Five recent professional liability claims involving the cornea are reviewed, arising from five unusual conditions: fungal infection following corneal abrasion with a tree branch; herpes simplex-induced neurotrophic keratitis resulting from contact lens wear; corneal necrotization caused by lime particulate matter splashed into the eye; siderosis-related retinal detachment secondary to a penetrating iron foreign body; and fluctuating acuity, halos, and flare resulting from radial keratotomy. Emphasis is on how these claims might have been avoided.

[Giant bronchopyocele. General and radiological characteristics. Apropos of a case].

A case of giant bronchocele is reported. This lesion is due to a mucus and pus filled dilatation of the bronchial tree proximal to a stenosis. The latter can be either congenital or acquired, or in the context of a diffuse bronchopathy, which can be responsible for a functional stenosis. Both plain radiography and computed tomography findings are diagnostic. The etiology of the abnormality however represents an important problem to be solved. Further, a thoractomy is often necessary, except in those case arising in the context of a diffuse bronchopathy, which usually resolve either spontaneous

[Imaging of aneurysms and dissections of the aorta. Part 1--Aneurysms of the abdominal aorta].

The authors review the main imaging findings of aneurysms and dissections of the aorta. Based on the experience and imaging techniques available in their institution (Conventional Radiology, Sonography and Computed Tomography), they try to make a decision tree of radiologic procedures in ambulatory and emergency patients. Nowadays, in the majority of situations, these techniques allow the Radiologist to make the correct diagnosis, detect the complications and evaluate the patients in the pre and post-surgical phases. Other imaging techniques not available in their institution, such as Magnetic

Rare association of tetralogy of Fallot and aortic valve stenosis. Autopsy findings after failed balloon aortic valvuloplasty.

We report the case of a neonate with a rare association of tetralogy of Fallot and aortic valve stenosis, who died following percutaneous balloon valvuloplasty. Postmortem examination confirmed a tetralogy of Fallot morphology with a markedly hypoplastic pulmonary tree, hypertrophy of both ventricles, and a thickened and dysplastic aortic valve. Valvuloplasty was not effective despite the presence of a fissure in the cusp. The findings suggest that the left ventricular impairment and associated anomalies, with or without a dysplastic valve may contribute to the failure of balloon valvuloplasty

[Bronchogenic cyst. Presentation of 2 cases and review of the literature].

The bronchogenic cyst is a congenital malformation of the tracheobronchial tree. It is not frequently seen in the pediatric population and had its origin around the sixth week of gestation when the bronchial development stops. Two cases of intraparenchymal bronchogenic cyst with lobar involvement of the right lung is presented. It was suspected after carefully examining and X-ray of the thorax and it was confirmed with ultrasonography and CT scan. There were no other associated malformations. The clinical and radiological diagnosis is extremely important for achieving the correct surgical proc

Novel approach to iatrogenic bile peritonitis.

Bile peritonitis after injury to the biliary tree is a serious complication that requires exploratory laparotomy. Our patient had an obstructing ampullary carcinoma, and generalized bile peritonitis developed from attempted percutaneous transhepatic cholangiography. The patient's condition was managed by peritoneal lavage and endoscopic transampullary stenting, with immediate relief of pain and toxicity. Exploratory laparotomy was avoided, and an eventual pylorus-sparing Whipple resection was the definitive treatment. We believe this to be the first report of successful nonoperative treatment

Epilepsy in a mitochondrial disorder.

In a large family with maternally inherited mitochondrial disease, a mild defect in the NADH-ubiquinone oxidoreductase step (complex 1) in the respiratory chain was found. Epilepsy was seen in nine (22%) of the 37 family members. Five of them, belonging to one branch of the family, had myoclonus epilepsy and EEG abnormalities consistent with this. The remaining four patients, belonging to other branches of the family tree, had partial epilepsy. Neurological symptoms also varied in different parts of the family. Possible explanations for the differences in phenotypic expressions are discussed.

Pseudoaneurysm of the radial artery as a cause of a late compartment syndrome. A case report.

Pseudoaneurysm has been reported in the literature as a cause of compartment syndrome. The onset of the subsequent compartment syndrome is usually early. Often, while the compartment syndrome is diagnosed and treated, the pseudoaneurysm is missed. A second operative procedure is then necessary. A 40-year-old woman illustrates an unusual case of a late compartment syndrome, following the development of a radial artery pseudoaneurysm three weeks after a penetrating injury. It is important to investigate the vascular tree in compartment syndromes after a penetrating injury, no matter how late the

Esophageal atresia, distal tracheoesophageal fistula, and an air shunt that compromised mechanical ventilation.

Infants with esophageal atresia and a distal tracheoesophageal fistula are predisposed to respiratory failure on the basis of prematurity, respiratory distress syndrome, aspiration of saliva, and reflux of gastric contents into the tracheobronchial tree. Thoracotomy and primary repair may be delayed to allow time for complete evaluation of the infant and respiratory stabilization. Poorly compliant lungs and a large distal fistula can result in selective passage of ventilatory gases into the gastrointestinal tract with resultant hypercarbia. Fogarty balloon occlusion of the distal esophageal se

Pulmonary manifestations of the collagen vascular diseases.

The collagen vascular diseases are a heterogeneous group of immunologically mediated inflammatory disorders. The organs and tissues that compose the respiratory system are frequently affected by these diseases. Potential targets of the inflammation and injury include the lung parenchyma, tracheobronchial tree, pulmonary vasculature, pleura, larynx, and respiratory muscles. In this article, the spectrum of respiratory disease caused by systemic lupus erythematosus, rheumatoid arthritis, scleroderma, polymyositis/dermatomyositis, mixed connective tissue disease, ankylosing spondylitis, relapsing

Reactive hepatopathy. Liver signs without primary liver disease.

"Reactive hepatopathy" is the term that defines a common clinical syndrome seen in seriously ill patients with disorders other than primary hepatobiliary disease. A variety of factors, including circulatory changes, metabolic stress, increased bilirubin formation, and especially uncontrolled sepsis, may lead to marked changes in parameters of liver function and frequently to frank jaundice. It is important for the practitioner to be aware of this syndrome and to avoid being distracted into spending undue time and resources in investigating the liver and biliary tree. Otherwise, there may be cr

Sports related injuries: incidence, management and prevention.

The incidence of injury related to various sports is reviewed according to sport, area of injury, number of participants and hours per week spent at the sport. Organized sports accounted for fewer injuries than unsupervised recreational activities like tree climbing, skateboarding and running. The knee is the most commonly injured site. Sensitivity to patients' commitment to their sport is necessary: sometimes instead of rest, they can substitute a less hazardous form of exercise. Principles of prevention involve proper familiarity with the sport, proper equipment and proper conditioning.

A practical reappraisal of the anatomy of the extrahepatic bile ducts and arteries.

Two hundred and five dissections of the extrahepatic biliary tree have revealed that although the anatomy of this area is remarkably variable there are obly three important vascular and five important ductal anomalies (the former occurring much more frequently than the latter). Each anomaly is, however, easily recognized at operation. A higher incidence of anatomical anomalites was found in patients with gallstones compared with those without and in women than in men, and there was a remarkably low incidence of accessory hepatic ducts overall. The significance and practical implications of the

The genome sequence of the Goose Foot Starfish, Anseropoda placenta (Pennant, 1777) (Valvatida: Asterinidae).

We present a genome assembly from an individual Anseropoda placenta (Goose Foot Starfish; Echinodermata; Asteroidea; Valvatida; Asterinidae). The assembly contains two haplotypes with total lengths of 533.27 megabases and 454.27 megabases. Most of haplotype 1 (70.67%) is scaffolded into 22 chromosomal pseudomolecules. Haplotype 2 was assembled to scaffold level. The mitochondrial genome has also been assembled, with a length of 16.42 kilobases. This assembly was generated as part of the Darwin Tree of Life project, which produces reference genomes for eukaryotic species found in Britain and Ir

Diagnosis and Evaluation of Treatment Response in Relapsing Polychondritis Using 18F-FDG PET/CT.

A 58-year-old man presenting with dyspnea, weight loss, and night sweating underwent 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) because of a suspicion of malignancy. 18F-FDG PET/CT demonstrated mild to moderate uptake on nasal, cricoid, and tracheobronchial tree cartilages and costovertebral junctions. The diagnosis was relapsing polychondritis, which is a rare multisystem disease characterized by inflammation of cartilage. In addition, subsequent 18F-FDG PET/CT after treatment showed complete metabolic response.
