Health workers' experiences of coping with the Ebola epidemic in Sierra Leone's health system: a qualitative study.

BACKGROUND: The 2014 Ebola Virus Disease epidemic evolved in alarming ways in Sierra Leone spreading to all districts. The country struggled to control it against a backdrop of a health system that was already over-burdened. Health workers play an important role during epidemics but there is limited research on how they cope during health epidemics in fragile states. This paper explores the challenges faced by health workers and their coping strategies during the Ebola outbreak in four districts - Bonthe, Kenema, Koinadugu and Western Area - of Sierra Leone. METHODS: We used a qualitative stu

Changes in Health-Seeking Behavior Did Not Result in Increased All-Cause Mortality During the Ebola Outbreak in Western Area, Sierra Leone.

Little is known about the residual effects of the west African Ebola virus disease (Ebola) epidemic on non-Ebola mortality and health-seeking behavior in Sierra Leone. We conducted a retrospective household survey to estimate mortality and describe health-seeking behavior in Western Area, Sierra Leone, between May 25, 2014, and February 16, 2015. We used two-stage cluster sampling, selected 30 geographical sectors with probability proportional to population size, and sampled 30 households per sector. Survey teams conducted face-to-face interviews and collected information on mortality and heal

HIV postexposure prophylaxis in an urban population of female sex workers in Nairobi, Kenya.

OBJECTIVES: To assess biological and behavioral factors in HIV-uninfected female sex workers (FSWs) accessing postexposure prophylaxis (PEP) and to characterize the circumstances preceding PEP, time to access, and adherence. METHODS: Participants were HIV-uninfected FSWs enrolled in an HIV care and prevention program in Nairobi, Kenya. Those accessing PEP between 2009 and 2010 were enrolled and compared with controls. Multiple logistic regression models were used to compare PEP-related biological and behavioral parameters. RESULTS: PEP users (n = 326) had been involved in sex work for a shor

Trait anger, cynical hostility and inflammation in Latinas: variations by anger type?

Anger and hostility are psychological factors that appear to play a salient role in relation to cardiovascular disease (CVD) risk; however, their association with risk within the Latino population remains relatively unexplored. The current study examined associations between overall trait anger, anger subdimensions (i.e., anger temperament and anger reaction) and cynical hostility with sICAM-1, a marker of cellular adhesion and systemic inflammation related to CVD risk, in a sample of 294 middleaged Mexican-American women. Results showed no association between trait anger or anger temperament

The complex gastrointestinal patient and Jean Watson's Theory of Caring in nutrition support.

The care of the patient with gastrointestinal disease is complex and challenging. The reasons for the complexity are varied and different for each patient. Any of these variables can affect the nutritional health of the patient, an essential element of care that supports healing, recovery, and improved quality of life. A nutritional assessment, an evaluation of the patient's nutritional status, can be used to establish the patient's weight history, dietary habits, tolerances, and likes and dislikes. Intake and output values from this assessment provide information relating to the patient's abi

Cardiac health: relationships among hostility, spirituality, and health risk.

This study examined hostility, spirituality, and indices of health risk among 100 young, primarily males of Hispanic background. Over half of the subjects were prehypertensive or hypertensive; one third had at least 2 objective risk factors for cardiac disease; and younger participants had lower spirituality scores and higher cynical distrust scores. Body mass index, spirituality, and glucose accounted for 29% of variance in systolic blood pressure; body mass index and age accounted for 39% of variance in diastolic blood pressure. The tools to assess cardiac risk (blood pressure, history, capi

Men who batter intimate partners: a grounded theory study of the development of male violence in intimate partner relationships.

Intimate partner violence is a serious and pervasive problem in U.S. society, with 25% of women and 7.6% of men reporting physical abuse by an intimate partner each year. Understanding the risk factors for development of violence is essential toward the development of interventions to reduce partner violence. Much of the understanding about the development of partner violence is based on research with victims rather than perpetrators. The study was conducted with men convicted of assault on an intimate female partner. Grounded theory was the method used to analyze data from interviews with 16

Role of Structural Marginalization, HIV Stigma, and Mistrust on HIV Prevention and Treatment Among Young Black Latinx Men Who Have Sex with Men and Transgender Women: Perspectives from Youth Service Providers.

Young black and Latinx men who have sex with men (YBLMSM) and transgender women (YBLTW) are disproportionately impacted by HIV. Structural and social marginalization, the social barriers, and structures that unevenly distribute benefits and burdens to different groups, may contribute to inability for youth to access prevention and treatment care services. Yet, few reports have examined the community and health care experiences of social marginalization among youth service providers who have multiple roles in the community (i.e., serve as a service provider and are a member or prior member of t

Listening to women's voices: the experience of giving birth with paramedic care in Queensland, Australia.

BACKGROUND: Unplanned out-of-hospital birth is generally assumed to occur for women who are multiparous, have a history of a short pushing phase of labour or are experiencing a precipitate birth. However, there is little research that examines the woman's perspective regarding factors that influenced their decision on when to access care. This research aimed to explore women's experience of unplanned out-of-hospital birth in paramedic care. Due to the size of the data in the larger study of 'Women's experience of unplanned out-of-hospital birth in paramedic care' [1], this paper will deal dire

Early performance in a humanistic medicine course as a predictor of dental students' later clinical performance.

The purpose of this study was to test the hypothesis that dental students' early ability to demonstrate a humanistic approach with patients is associated with later clinical performance. A first-year humanistic medicine course, Patient Doctor I (PDI), at Harvard School of Dental Medicine combines training in conducting the medical interview with human values, placing a high value on strengthening relationships with patients and emphasizing empowerment, respect, and strong communication skills. Retrospective data were collected in the following domains: PDI course evaluations, admissions inform

Patients' attitudes towards oral health and experiences of periodontal treatment: a qualitative interview study.

PURPOSE: The aim of the present qualitative study was to analyse patients' attitudes towards oral health and experiences of periodontal treatment. MATERIALS AND METHODS: Patients who were referred to a specialist clinic in periodontics were subjected to open-ended interviews before (Abrahamsson et al, 2008), as well as after, the phase of cause-related periodontal therapy. An interview guide was used with themes of interest. The time interval between the interviews was about 6 months. Because the study focused on psychosocial interactions, the qualitative method used for the analysis was Grou

Childhood chronic pain and health care professional interactions: shaping the chronic pain experiences of children.

Children with chronic pain meet numerous healthcare professionals during their search to understand their pain. Through semi-structured interviews, this qualitative study sought to understand the experiences of five children with chronic pain as they encountered healthcare professionals. In the majority of these interactions, children reported feeling misunderstood, disbelieved and abandoned. The findings of this study demonstrate that children's experiences with professionals influence their approach towards current and future healthcare encounters. All children discussed their guarded relati

Engaging with digital health: a cross-sectional study of older adults' use of online health information exchange.

BACKGROUND: Online health information exchange (OHIE) increasingly shapes how individuals interact with healthcare providers and peers. Yet, despite older adults' often high need for health information, their engagement with OHIE remains insufficiently understood, given persistent age-related differences in digital access and skills. METHODS: This study draws on a population-based survey of older adults aged 60 + in Switzerland (N = 1,261) to analyze three types of OHIE: interactions with doctors, other healthcare providers, and peers. Logistic regression models were used to assess how sociod

"Sisters Influencing Sisters:" Findings from a Community-Engaged Process to Develop a Social Media-Based, Influencer-Delivered Intervention to Increase PrEP Awareness, Knowledge, and Uptake Among Transgender Women.

Transgender women (TW) shoulder a disproportionate burden of HIV diagnoses, yet are less likely to use PrEP, due to intersectional social and structural barriers to accessing services. TW utilize social media to share health information and foster community, yet to date, most studies leveraging the power of these platforms for PrEP interventions have focused on gay men. To address this gap, we partnered with TW social media influencers and engaged a Community Advisory Board (CAB) to develop an Instagram-based intervention to increase PrEP awareness, knowledge, and use among TW. Over quarterly

Experiences of being diagnosed with unruptured intracranial aneurysm in a population-based health survey: A qualitative interview study.

INTRODUCTION: Most unruptured intracranial aneurysms (UIA) are small and carry a low rupture risk. Many patients are therefore not treated with occlusion but managed conservatively. Evidence on the psychological impact of living with an untreated UIA is limited and conflicting. RESEARCH QUESTION: To explore experiences of receiving a diagnosis of a UIA and having to live with it over time. MATERIAL AND METHODS: The Tromsø7 health survey examined 1862 participants with magnetic resonance angiography and 85 diagnosed with conservatively managed UIAs were eligible for inclusion. We invited 24 b

Words That Heal: A Systematic Review of Healthcare Providers-Patient Communication to Enhance Chronic Care Management and Practice Recommendations.

Chronic diseases represent a major global health burden, and suboptimal treatment adherence remains a key obstacle to long-term management. Healthcare provider (HCP)-patient communication is a modifiable determinant of adherence and psychological functioning, yet evidence remains fragmented across conditions and communication domains. This review builds on patient-centered communication scholarship and established models of communication functions and pathways linking clinician-patient communication to health outcomes. This systematic review synthesizes empirical evidence on HCP-patient commun
