Clinical Profile and Treatment Response of Patients with Ocular Inflammation due to Presumed Ocular Tuberculosis: A Retrospective Study.

OBJECTIVES: Ocular tuberculosis is an extrapulmonary tuberculous infection and has varying manifestations which pose a huge challenge to diagnosis and treatment. The purpose of this study is to describe the various clinical manifestations of ocular inflammations due to tuberculosis and to assess the response to treatment following antituberculous therapy (ATT) and corticosteroids in these patients. MATERIALS AND METHODS: We performed a retrospective analysis of 29 patients with presumed ocular tuberculosis who were started on ATT and completed follow-up of at least 6 months after ATT was init

Jejunal multiple perforations for combined abdominal typhoid fever and miliary peritoneal tuberculosis.

Typhoid fever and tuberculosis, considered rare diseases in western countries, is still considered a notable problem of health issue in developing countries. The gastrointestinal manifestations of typhoid fever are the most common and the typhoid intestinal perforation (TIP) is considered the most dangerous complication. Abdominal localization of tuberculosis is the 6th most frequent site for extra pulmonary involvement, it can involve any part of the digestive system, including peritoneum, causing miliary peritoneal tuberculosis (MPT). This is the case report of a 4 years old girl with multip

[Advances in the development of new vaccines against tuberculosis. 100 years after the introduction of BCG].

The BCG vaccine used in the world for nearly 100 years protects children against the most severe forms of tuberculosis, but its effectiveness in preventing the most commonly occurring tuberculosis and the one burdened with the highest risk of transmission in adults is very diverse. Contraindications for BCG vaccination include HIV infection and other conditions of immunosuppression. Tuberculosis is a global problem difficult to control because of three main reasons: poor diagnostics in developing countries, long-term therapy or discontinuation of treatment resulting in the emergence of drug-re

Delay in diagnosis of generalized miliary tuberculosis with osseo-articular involvement: a case report.

INTRODUCTION: Diagnosis of atypical tuberculosis is difficult. Therefore, it is important that physicians are aware of rare presentations of tuberculosis to avoid diagnostic delays. CASE PRESENTATION: We present the case of a 17-year-old Sri Lankan man who presented to our facility with an ill-defined large induration over the skin of his left buttock and thigh. A cause could not be found despite extensive investigations. He also complained of chronic knee pain, but this was not investigated further at the time due to spontaneous resolution. Three years later his knee disease flared up again,

Association of tibial osteomyelitis and pneumonitis due to miliary tuberculosis in a patient with systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease, which has great prevalence and uncommon manifestations of opportunistic infectious diseases, mainly due to the multiple abnormalities of the immune system and the immunosuppressive effect of the medications used in its treatment. Patients whit SLE have an increased incidence of tuberculosis, and osteoarticular involvement occurs in 1%-3% of the cases, manifesting as pain, reduction in mobility, and increased osteoarticular volume. The radiographic findings are often nonspecific. Magnetic resonance imaging (MRI) is an usefu

[Characteristics of tuberculosis in a general hospital during the period 1993-1998. Analysis of resistance and HIV coinfection].

AIM: The epidemiologic and clinical characteristics, presence of HIV coinfection, and sensitivity to tuberculostatic drugs were analyzed in a series of tuberculosis patients attended in our center. PATIENTS AND METHODS: Retrospective study of tuberculosis cases attended in a third- level hospital from 1993 to 1998. RESULTS: During the study period, 268 cases of tuberculosis were diagnosed in our center. A progressive decrease in the incidence of this disease has occurred since 1995. Among the jailed population, we also found a decrease in cases of tuberculosis and there were no cases of resi

Abdominal tuberculosis presenting as a massive lower GI bleed in a cerebral palsy patient-a case report.

Abdominal tuberculosis is a rare extrapulmonary manifestation of tuberculosis (TB), one that is increasingly encountered at hospitals serving a growing immigrant population. Since the COVID-19 pandemic, the incidence of TB has continually increased. The diagnostic differential often overlaps with inflammatory bowel disease, with diagnostic laparoscopy remaining the most effective modality of diagnosis. Additional diagnostic difficulties are encountered in patients unable to report symptoms. We present the case of a 52-year-old woman with cerebral palsy who presented initially with pulmonary sy

Tuberculosis of the Lacrimal Gland: A Case Report.

Tuberculosis of the lacrimal gland is a rare manifestation of extrapulmonary tuberculosis. It may occur in isolation or in association with pulmonary tuberculosis. The diagnosis is strongly supported by histopathological examination. Treatment relies on anti-tuberculous therapy. We report the case of a 53-year-old woman with no significant medical history who presented with chronic right-sided epiphora associated with the progressive appearance of a swelling in the region of the right lacrimal gland. A biopsy of the mass was performed, and histopathological analysis confirmed the diagnosis of

Tuberculosis meningitis coexisting with HIV Infection: a comprehensive review.

Mycobacterium tuberculosis (Mtb) is the causative agent of tuberculosis (TB) in humans, Although Mtb is primarily considered a respiratory pathogen, its ability to spread to and affect the central nervous system (CNS) is of particular interest due to its clinical importance. Tuberculosis meningitis (TBM) is described as the manifestation of Mtb infection in the meninges, leading to inflammation and disease. Individuals with a weakened immune system, particularly those infected with human immunodeficiency virus (HIV), are more susceptible to both pulmonary and extrapulmonary Mtb infection. HIV

When to start treatment? Dilemma illustrated by a paediatric case of extensively drug-resistant tuberculosis of the central nervous system.

An HIV-positive mother infected her daughter with extensively drug-resistant Mycobacterium tuberculosis. Despite adhering to the then current guidelines for prevention, the infant was diagnosed with extensively drug-resistant pulmonary tuberculosis at the age of 4 months and developed tuberculous meningitis. After a short delay, appropriate treatment was initiated, followed by an inhospital stay at a specialised hospital. The infant became generally well, but had delayed neurological development. Secondary hydrocephalus due to tuberculous meningitis required a ventriculoperitoneal shunt. After

The current status, challenges, and future developments of new tuberculosis vaccines.

Mycobacterium tuberculosis complex causes tuberculosis (TB), one of the top 10 causes of death worldwide. TB results in more fatalities than multi-drug resistant (MDR) HIV strain related coinfection. Vaccines play a key role in the prevention and control of infectious diseases. Unfortunately, the only licensed preventive vaccine against TB, bacilli Calmette-Guérin (BCG), is ineffective for prevention of pulmonary TB in adults. Therefore, it is very important to develop novel vaccines for TB prevention and control. This literature review provides an overview of the innate and adaptive immune re

Mycobacterium tuberculosis infection and vaccine development.

Following HIV/AIDS, tuberculosis (TB) continues to be the second most deadly infectious disease in humans. The global TB prevalence has become worse in recent years due to the emergence of multi-drug resistant (MDR) and extensively-drug resistant (XDR) strains, as well as co-infection with HIV. Although Bacillus Calmette-Guérin (BCG) vaccine has nearly been used for a century in many countries, it does not protect adult pulmonary tuberculosis and even causes disseminated BCG disease in HIV-positive population. It is impossible to use BCG to eliminate the Mycobacterium tuberculosis (M. tb) infe

INTRODUCTION: Tuberculosis is an infectious disease that has displayed increasing incidence in the last decades. It is estimated that up to 20% of tuberculosis cases affect extra-pulmonary organs. In the ENT area, soft palate and tongue are the least probable locations. CASE REPORT: A 62-year-old female with a history of rheumatoid arthritis and treatment with corticosteroids and Adalimumab, developed a foreign body sensation in the pharynx accompanied by a sore throat and halitosis. The laryngoscopy with a 70 degree rigid telescope showed an ulcerated hypertrophic lesion in the right vallecu

Primary tuberculosis in soft palate: case report of a rare entity.

A 40-year-old male patient presented to our clinic with history of dysphagia and ulceration in the palate for two months. After history-taking and thorough clinical examination, investigations like routine blood parameters, chest skiagram, sputum for acid-fast bacilli, ultrasonography of the abdomen, and biopsy from the palatal lesion were performed. No evidence in support of pulmonary or abdominal tuberculosis was found. Histopathological examination of the biopsy revealed granulomatous inflammation with Langhans giant cells and caseation necrosis. Diagnosis of primary tuberculosis of soft pa

Contact tracing of tuberculosis in a school setting and the relevance of BCG in this population.

Tuberculosis (TB) remains a worldwide health problem. Treatment, contact tracing and screening of at-risk populations continue to be important issues even in countries with low rates of tuberculosis. In contact tracing, prior BCG vaccination can make interpretation of Mantoux positivity difficult. We had the opportunity to evaluate contacts of a student diagnosed with pulmonary tuberculosis. In the index case's class, 23 of 50 (46%) students screened had TB infection and a further 2 (4%) students had TB disease. Screening was offered to all students in the school (307 students) and 244 student

[Clinical and epidemiological changes in tuberculosis in children in the last 5 decades(1950-2000) ].

This study analyses over a 48 years period (1953-2000) the incidence of various clinical forms of TB in children aged 0-8, admitted in several ftizio-pediatric institutions. Follow-up included: primary complex, pleuro-pulmonary involvement, bronchial adenogenic involvement, extra-respiratory forms and major sequelae of primary tuberculosis. The results show a marked decrease of incidence of all TB forms in children and disappearance of chronic extensive primary tuberculosis and major sequelae of primary tuberculosis. The incidence tappered in steps: a sudden decrease in 1964-65, another in 197

[A case of miliary tuberculosis presenting as fever and jaundice with hepatic failure looking like the course of fulminant hepatitis].

A 79-year-old male was admitted to the Metropolitan Hiroo Hospital with chief complaints of icterus and fever. A few weeks prior to admission, he developed fever and swelling of right side of the neck and was seen at a local hospital where an anti-inflammatory agent was prescribed. The fever subsided in a few days, but recurred together with development of icterus a few weeks later, precipitating this hospitalization. After admission, hepatic failure progressed rapidly, indicating a fulminant hepatitis. Renal failure also developed and he died. Autopsy revealed diffuse caseous necrosis with de

Chest X- ray findings in HIV patients in relation to the CD4 count.

BACKGROUND: CD4 count measures the degree of immunosupression in HIV-infected patients. Immunosupression results in lack of ability of the body to fight infections thus predisposing the individual to infection; lung is one of the most susceptible organs. An important diagnostic tool in assessing the respiratory complications as well as the manifestations of HIV infection is the chest radiograph. OBJECTIVE: This study is aimed at determining the radiological features seen on chest radiographs of HIV/AIDS patients in relation to their corresponding CD4 count which is a measure of immunosupressi
