The intravenous urograms of 80 patients aged 16 to 29 (mean 23) with urinary bilharziasis were reviewed to determine the frequency of striation of the renal pelvis and ureter. Another 345 patients aged 16 to 29 (mean 22) with various abdominal and urological symptoms, had intravenous urography during the same period and were used as a reference group. Striation of the upper urinary tract was demonstrated in 21% of the patients with urinary schistosomiasis but in only 3% of the reference group. Striation constitutes an early radiological change in this disease.

Torsion of the testis is a common urological emergency. Blunt scrotal trauma also may precipitate an emergency. We recently treated 2 adolescent boys in whom blunt scrotal trauma induced testicular torsion. A retrospective review of the charts of 138 patients explored for acute scrotal pain at our hospital revealed a history of scrotal trauma in 3 of 57 patients (5 per cent) with testicular torsion and 7 of 73 (10 per cent) with torsion of the testicular appendages. The possibility of torsion must be considered in cases of scrotal trauma.

In 6 patients with diabetes mellitus and 6 non-diabetic "controls" referred for urological assessment, sacral electromyelography (SER) and electrical stimulation of the anal reflex were compared with pressure/flow videocystourethrography (VCU) for the detection of sacral neuropathy, whilst electrocardiography (ECG) was used to screen for generalised autonomic neuropathy. Five diabetics had evidence of a neuropathic bladder on VCU and on electrical stimulation all 5 had an absent anal reflex, which may thus prove to be a simple, reliable, non-invasive screening test for sacral neuropathy.

The spectrum of symptoms and findings associated with posterior urethral valves can vary and mimic other conditions. Three cases of congenital posterior urethral valves in adult males are presented. Two patients were on dialysis with end-stage renal failure at the time of diagnosis after many earlier urologic evaluations. One patient subsequently received two cadaver renal transplants. The remaining patient presented with typical symptoms of prostatitis and was found to have posterior valves with a dilated posterior urethra and intraprostatic reflux on voiding cystourethrography.

A 51 years old patient underwent 3 urological operations within a short space of time. Isotope renography has been carried out in 32 babies with neurogenic bladder due to spina bifida or with other urological lesions. The tracings can be interpreted according to the standards applicable to renography in older children. OBJECTIVE: Traditionally, patients presenting with haematuria undergo a series of investigations, including urine cytology, cystoscopy, i.v. urography (IVU) and renal tract ultrasound (US).
