The chronic vascular and haemodynamic response after permanent bilateral common carotid occlusion in newborn and adult rats.

Vascular growth and redistribution of flow can compensate for arterial occlusion and possibly reduce the effects of hypoperfusion. As yet there is limited information on the age-dependent nature of vasculature remodelling. In this study, we have monitored the vascular and morphologic changes using magnetic resonance imaging and histology in a chronic bilateral common carotid artery occlusion (BCCAO) model in both newborn and adult rats. Acutely, cerebral blood flow (CBF) decreased immediately after BCCAO, producing a state of oligemic hypoperfusion. At 6 months after BCCAO in both adult and ne

Staged dorsolateral decompression and posterior stabilization for severe ventral odontoid compression in basilar invagination: illustrative case.

BACKGROUND: Basilar invagination (BI) with ventral cervicomedullary compression remains surgically challenging. Although transoral and endoscopic endonasal odontoidectomy are established approaches, some anatomical variations may favor alternative surgical corridors. OBSERVATIONS: A patient with BI developed progressive quadriparesis after cervical trauma. Imaging demonstrated severe ventral cervicomedullary compression from a retroverted, left-lateralized odontoid process, displacing the cervicomedullary junction contralaterally. CT angiography revealed absence of the left vertebral artery (

Moyamoya disease presenting with repeated ruptures of a lateral posterior choroidal artery aneurysm: illustrative case.

BACKGROUND: Peripheral aneurysms may develop along collateral pathways in patients with moyamoya disease (MMD); however, their optimal management remains unclear. The authors report a case of a lateral posterior choroidal artery aneurysm associated with MMD that ruptured three times within a short period, along with a review of the literature. OBSERVATIONS: A 54-year-old woman with MMD, who had remained asymptomatic for 19 years, developed an intraventricular hemorrhage 1 month after periventricular aneurysm dilation was detected on annual imaging follow-up. Rebleeding occurred on days 7 and

The other Babinski sign in Hemifacial Spasm: a clinical marker of severity and its predictive association with η-like vA-PICA compression.

OBJECTIVE: To investigate the clinical significance of the other Babinski sign in hemifacial spasm (HFS), focusing on its association with symptom severity and specific neurovascular compression (NVC) patterns. METHODS: This prospective observational study evaluated 69 consecutive HFS patients undergoing microvascular decompression. The association between the other Babinski sign and HSGS scores was assessed using the chi-square test for independence, with Cramer's V quantifying association strength. Subsequently, univariate and multivariate logistic regression analyses were performed to iden

The critical role of non-contrast chest CT in avoiding thrombolysis catastrophe: A case of acute aortic dissection masquerading as ischemic stroke.

INTRODUCTION AND IMPORTANCE: Aortic dissection (AD) complicating acute ischemic stroke poses significant therapeutic challenges, particularly regarding thrombolytic contraindications that may substantially increase mortality risk. This underscores the critical need for expedited differential diagnosis in such clinical scenarios. CASE PRESENTATION: An 80-year-old male presented with acute-onset left-sided hemiplegia and unresponsiveness. CLINICAL DISCUSSION: Initial cerebral neuroimaging demonstrated a substantial ischemic penumbra (136.2 ml Tmax >6 s) in the right middle cerebral artery (MCA

De Novo Heterozygous ZFX Frameshift Variant in a Female With an X-Linked Neurodevelopmental Disorder.

Germline ZFX variants are associated with an X-linked neurodevelopmental disorder, with 14 males and 16 females reported to date. We describe a 20-year-old female with a heterozygous ZFX frameshift variant, p.(Met666Valfs*2), identified by genome sequencing, previously reported in an affected male. She exhibited motor and speech delays with hypotonia in early childhood, and was later diagnosed with congenital heart defects, autism spectrum disorder, mild intellectual disability, and absence seizures. She further developed sensorineural hearing loss, skin hyperpigmentation, and ophthalmoplegia.

Basilar artery thrombosis and Wallenberg syndrome in a patient with uncontrolled hypertension.

Presented here is a compelling case of a patient with a history of untreated hypertension, highlighting symptoms indicative of Wallenberg syndrome, including acute-onset dizziness, visual disturbances, continuous vomiting, difficulty walking, and an altered level of consciousness. This case's significance lies in its clinical presentation and in the diagnostic journey undertaken to elucidate its underlying pathology. Throughout the patient's hospitalization, a comprehensive assessment incorporating clinical, laboratory, and imaging techniques was conducted to delineate the extent of their cond

Bow Hunter's syndrome with clicking sounds: A rare etiology of transient loss of consciousness with tonic-clonic seizure.

We present the case of a young male patient experiencing a transient loss of consciousness and manifesting a seizure when he tilted his head backward. Transcranial Doppler ultrasound (TCD) and carotid artery ultrasound (CAU) examination were normal when the patient's neck was in the neutral position. However, the CAU revealed vertebral artery (VA) transient occlusion during neck rotation or backward movement. Electroencephalogram (EEG) monitoring was performed with multiple neck rotation-induced tests. The patient developed dizziness, which was the same as the prodromal symptoms of the first s

Recanalization of Inferior Vena Cava and Bilateral Iliac Veins 15 Years after May-Thurner Syndrome-Related Occlusion.

May-Thurner syndrome (MTS) is a relatively rare condition involving mechanical compression of a vein between an artery and a bone which may result in venous stenosis, reflux, occlusion, or deep vein thrombosis (DVT). The most common location for MTS to occur is the left iliocaval confluence, specifically where the left common iliac vein crosses under the right common iliac artery and becomes compressed against a vertebral body. Our case represents a unique presentation of MTS where a missed diagnosis of MTS during a presentation of acute LLE DVT over 15 years ago which would later progress to

Cerebral aneurysms and cervical artery dissection: Neurological complications and genetic associations.

Dissections and aneurysms are two of the more common nonatherosclerotic arteriopathies of the cerebrovascular system and a significant contributor to neurovascular complications, particularly in the young. Specifically, ruptured intracranial aneurysms (IA) account for nearly 500,000 cases of subarachnoid hemorrhage annually with a 30-day mortality approaching 40% and survivors suffering often permanent neurologic deficits and disability. Unruptured IAs require dedicated assessment of risk and often warrant serial radiologic monitoring. Cervical artery dissection, affecting the carotid and vert

Left distal radial access in patients with arteria lusoria: insights for cerebral angiography and interventions.

BACKGROUND: The transradial approach (TRA) is frequently used for neurointerventional procedures as it is safer, improves patient comfort, and decreases costs and procedural time in comparison with the transfemoral approach (TFA). Patients with arteria lusoria, or an aberrant right subclavian artery (ARSA), provide a unique challenge for cerebral angiography and interventions when using the TRA. OBJECTIVE: To examine the hypothesis that the extreme angulation encountered while accessing the great vessels from the right TRA could be overcome by reversing the approach to the left distal TRA (dT

Anatomic Variation and Hemodynamic Evolution of Vertebrobasilar Arterial System May Contribute to the Development of Vascular Compression in Hemifacial Spasm.

BACKGROUND: Hemifacial spasm (HFS) is caused by vascular compression of the facial nerve. The definitive mechanism of offending vessel formation remains unclear. The aim of this study was to explore whether the anatomic and hemodynamic characteristics of the vertebrobasilar artery play a role in problematic vessel formation in HFS. METHODS: Imaging data of 341 patients with HFS who underwent microvascular decompression were reviewed retrospectively and compared with 360 control subjects. Hemodynamics of typical anatomic variations of the vertebral artery (VA) were analyzed using computational

[Cervical spinal dural arteriovenous fistula with rapidly progressive brainstem dysfunction due to venous congestion: a case report].

Spinal dural arteriovenous fistulas(S-dAVFs)are rare vascular malformations of the spine. We experienced a case that presented with rapidly progressive brainstem dysfunction due to venous congestion of cervical dAVFs. A 56-year-old man diagnosed with cervical dAVF four years prior presented with gait disturbance and abnormal thermal nociception on his right side. In addition to the high-intensity lesion from the lower pons to the medulla oblongata on T2-weighted magnetic resonance imaging, diffusion-weighted imaging demonstrated cerebral infarction of the left ventrolateral medulla oblongata.

Fibrocartilaginous embolism--an uncommon cause of spinal cord infarction: a case report and review of the literature.

Fibrocartilaginous embolism is a rare cause of spinal cord infarction. It is postulated that an acute vertical disk herniation of the nucleus pulposus material can lead to spinal cord infarction by a retrograde embolization to the central artery. An increased intradiskal pressure resulting from axial loading of the vertebral column with a concomitant Valsalva maneuver is thought to be the initiating event for the embolus. We present a previously healthy 16-year-old boy with sudden onset of back pain and progressive paraparesis within 36 hours after lifting exercises in a squat position. His cl

[The youngest case of isolated interruption of the aortic arch without associated cardiac anomalies].

A five-month-old girl was admitted to our institute because of the systolic blood pressure gradient of 42 mmHg between upper and lower extremities. Cineangiography revealed that the brachiocephalic artery originated from ascending aorta, while the left subclavian and left vertebral arteries took their origin from the descending aorta. However, the left common carotid artery was not visualized either on the angiogram or on the digital subtraction angiogram. No other associated cardiovascular anomalies, such as patent ductus or ventricular septal defect, were found. Therefore the diagnosis of is
